Since your periods have been absent for the last 6 months and you now have pain in the right side of the abdomen with a past history of ovarian cyst surgery, it is very important to get a Pelvic Ultrasound (USG abdomen + pelvis) done immediately.

This test will help check: Whether there is any recurrence of ovarian cyst. If there are complications like torsion, rupture, or adhesions after surgery Condition of the uterus and ovarian function Please do not start any medicine until this scan is done. Once the report is available, treatment can be planned safely.

HELLO PRAJAKTA,

-You are 27 years old, had ovarian cyst surgery in 2020. -periods have stopped for 6 months (secondary amenorrhea) -Pregnancy is ruled out (test negative) -Now you also have right-sided abdominal pain for 5 days.

In Ayurveda, when menstruation is absent, it is called Artava kshaya (decrease of menstrual flow) or Nastartava (complete absence of menstruation). -This usually happens because of imbalance in Vata and kapha dosha -Vata responsible for movement gets obstructed by kapha responsible for growth, cyst like formulations, leading to blockage of reproductive channels -surgery may remove cysts but doesn’t correct the underlying imbalance, so the problem so the problem can come back

Before starting treatment, the following tests help rule out other causes -Pelvic ultrasound= check uterus, ovaries (for cyst recurrence, endometrium thickness) -Hormone tests- TSH, FSH, LH, Prolactin, Estradiol, Progesterone, AMH -CBC and Blood sugar= to rule out anemia and insulin resistance (common in PCOS)

TREATMENT GOALS -Clear obtruction in reproductive channels -Regulate hormones naturally -strengthen uterus and ovaries to maintain healthy cycles -prevent recurrence of cysts -relieve abdominal pain and improve digestion

INTERNAL MEDICATIONS

1) RAJAHPRAVARTINI VATI= 1 tab twice daily with warm water after meals =stimulates uterus, helps initiate periods

2) ASHOKARISHTA= 20 ml + equal water after meals twice daily =regulate menstrual cycle, reduces pain

3) KUMARYASAVA= 20 ml with equal water after meals twice daily =supports ovarian function, acts as mild uterine tonic

4) PHAL GHRITA= 1 tsp with warm milk in morning =nourishes reproductive tissues, balances vata

5) SHATAVARI CHURNA= 1 tsp with warm milk at night =rasayana for female health, improves fertility and hormonal balance

DURATION= usually 3 months of continuous therapy

EXTERNAL THERAPIES

1) OIL MASSAGE with warm sesame oil =relaxes body , reduces vata

2) STEAM TO ABDOMEN AND LOWER BACK -releives pain, removes channels blockages

3) LEPA (herbal paste on lower abdomen ) =reduces pain and kapha obstruction

LIFESTYLE CHANGES -Sleep= maintain regular regular sleep, avoid staying up late -Exercise= moderate daily exercise like walking, yoga, avoid over exertion -Stress management= meditation, pranayama-since stress directly affects menstrual hormones -Avoid= excess cold exposure, cold water baths, excessive fasting, overwork

DIET -warm, freshly cooked foods -sesame seeds, jaggery, ginger, ajwain, cumin- naturally stimulate periods -ghee in modertion= balance vata -fruits= pomegranate, pineapple, papaya -green leafy vegetables= moong dal, whole grains

AVOID -cold,heavy, stale, junk food -excess dairy (curd, cheese, cold milk ) -oily, deep fried, fast food -excess tea, coffee, alcohol

YOGA AND PRANAYAM Helps balance hormones, relive stress, and improve pelvic blood flow

ASANAS -bhujangasana -Baddha konasana -Malasana -Supta baddha konasana -Dhanurasana

PRANAYAM -Anulom vilom -Bhramari -Kapalbhati

Practice daily for 15-20 min

HOME REMEDIES -Seasame + jaggery= 1 tsp roasted sesame seeds with small piece of jaggery daily -Ginger tea= boil fresh ginger in water, drink water twice daily -Ajwain water= boil 1 tsp ajwain seeds in a glass of water, reduce to half, drink warm -Papaya= regulate consummation helps stimulate menstruation

-Since your periods have been absent for 6 months and you have abdominal pain, do not ignore this- get a gynaecological heck up and ultrasound first

DO FOLLOW

HOPE THIS MIGHT BE HELPFUL

THANK YOU

DR. MAITRI ACHARYA

Missing periods can be due to various conditions, especially after experiencing ovarian cysts, and a recent negative pregnancy test further rules out that possibility. The pain you are experiencing in the right side abdomen could be associated with many reasons, possibly the cyst or another unrelated issue. However, it is important to first rule out any acute conditions, so consulting a healthcare professional for this symptom is crucial.

Concerning the absence of your menstrual cycle for six months (amenorrhea), this could be related to hormonal imbalances, stress, nutritional deficiencies, or underlying medical conditions. In Ayurveda, such issues are often linked to an imbalance in the doshas, specifically Vata and Pitta, which govern bodily rhythms and metabolism, respectively.

You can start by including warm, nourishing foods in your diet to balance Vata. Prefer cooked vegetables, whole grains like rice and quinoa, and legumes. Drink warm fluids; ginger tea is particularly benefical. Avoid cold or raw foods, excessive caffeine, and processed sugras. All of these tend to aggrivate Vata.

Adding herbs like Ashwagandha can be helpful. Its grounding properties support hormonal balance and alleviate stress. Take about 1 teaspoon of Ashwagandha powder in warm milk once a day. Ensure you have adequate rest and try to establish a regular sleep cycle, as irregular sleeping patterns can disturb menstrual regularity.

Performing some gentle Yoga poses like Bhujangasana (Cobra Pose) and Baddha Konasana (Bound Angle Pose) can help enhancing circulation and stimulate pelvic health. Aim for 15-20 minutes daily.

Triphala, a traditional Ayurvedic formulation, can aid in detoxification and digestive improvement, hence balancing Pitta and regulating hormones. Take 1/2 tsp with warm water before bed.

Regularly practicing pranayama like Nadi Shodhana (alternate nostril breathing) can calm the nervous system and address underlying stress. Evening walks, moderately paced should be pursued.

Missing proper check-ups should be avoided; please see a doctor immediately to investigate further if pain or other symptoms persist or worsen. A regular health evaluation can identify medical needs beyond the scope of lifestyle and dietary adjustments.

It’s important to address the absence of menstruation alongside your recent abdominal pain with caution. Given your history of an ovarian cyst operation, there could be a few factors at play here. First, it’s crucial to rule out any serious conditions, so I urge you to seek immediate medical attention if the pain worsens or if you experience additional symptoms like dizziness, fever, or unusual discharge.

From a Siddha-Ayurvedic perspective, the lack of menstruation, or amenorrhea, can often be related to an imbalance in the vata dosha and potentially some involvement of kapha. This can disrupt the normal function of the reproductive system. A vital focus should be on balancing these doshas and supporting healthy agni (digestive fire), as it influences all body systems including reproduction.

Diet plays a key role in managing vata imbalances. Aim to consume warm, cooked foods—think rice, whole grains, and stews—prepared with warming spices such as ginger, cumin, and turmeric. Avoid raw foods and cold drinks which can exacerbate vata disturbances. Hydrating with warm herbal teas like fennel or cumin tea can be supportive as well.

Herbal remedies known in the tradition for supporting regular menses include herbs like Shatavari and Ashoka, which can help in balancing reproductive hormones. You may take Shatavari powder, around 1 teaspoon mixed with warm milk or water, twice daily. However, be sure to consult a local Siddha-Ayurvedic practitioner to ensure these recommendations fit your unique body constitution and health status.

For abdominal pain, applying a warm sesame oil massage to the lower abdomen might offer some relief by improving circulation and calming vata. Since the pain is recent and noticeable, it’s important not to delay further evaluation by a healthcare provider, especially considering past surgery and negative pregnancy test results. Always prioritize prompt medical consultation to address potential complications.
