Hi! I’m an assistant to the Ask Ayurveda doctors. I’ve carefully reviewed your question.

I will explain your symptoms through the Ayurvedic perspective so you understand what is happening in your body as a whole.

The severe pain you’re experiencing can be associated with both diabetic neuropathy and sciatica, which from an Ayurvedic viewpoint, often involves an imbalance in Vata dosha. Vata governs movement and nerve impulses in the body, and its aggravation can lead to pain, tingling, and numbness. Additionally, high blood sugar levels and hypertension can contribute to this imbalance, further complicating the condition.

Diabetic neuropathy typically results from prolonged high blood sugar levels, leading to nerve damage. In Ayurveda, this is seen as a result of ama (toxins) accumulation and disturbed Vata, leading to impaired nerve function and pain. Sciatica, on the other hand, involves Vata disturbance in the lower back and legs, often exacerbated by cold or irregular lifestyle habits.

Your systemic imbalance is reflected in the combination of high blood sugar, high blood pressure, and pain. Ayurveda views these as interconnected, stemming from lifestyle, digestion, and stress, which contribute to Vata and ama imbalances.

To give doctors a precise and effective treatment plan, your answers to the questions below are essential. Without this information, the recommendations will be more general and less accurate.

Please answer each question as clearly as possible.

1. How would you describe the nature of your pain? Is it sharp and stabbing, dull and aching, or more of a burning sensation? 2. Where exactly do you feel the pain most intensely? Is it predominantly in the lower back, legs, feet, or a combination of areas? 3. What seems to relieve your pain? Do resting, heat application, or cold application provide any relief? 4. How well is your blood sugar managed currently? Is it well controlled with medication, occasionally high, or consistently high? 5. How often do you experience episodes of high blood pressure, and how are they managed? 6. What medications are you currently taking for diabetes and blood pressure? Are you on oral medication, insulin, antihypertensive medication, or a combination of these? 7. How would you rate your overall energy levels during the day? Do you experience high energy, moderate energy, or low energy?

Your detailed responses will help tailor a more specific Ayurvedic approach to your condition.

Namaste, Your situation needs careful attention because the pain you are describing can be due to aggravated nerves, either from uncontrolled diabetes or sciatica, and right now your sugar level (260) is quite high, which itself can worsen nerve pain and delay recovery. First priority should be to bring blood sugar under better control, otherwise any treatment for pain will give only temporary relief. Try to monitor sugar regularly and avoid sweets, refined foods, excess rice, and heavy meals. Keep your diet light, with more vegetables, whole grains, and a little ghee. Eat on time and avoid long gaps. Sharp, stabbing pain with radiation can be more towards sciatica, while burning, numbness, or tingling is more neuropathy. In many cases, both can overlap, especially in diabetes. Avoid sitting or standing in one position for long time. If pain increases on walking or bending, give proper rest to the back. At the same time, complete bed rest is also not advised—gentle movement within comfort is better. For immediate relief, you can apply warm sesame oil or Mahanarayana taila on lower back and legs, followed by mild heat (hot water bag). This helps reduce stiffness and nerve irritation. If the pain is more in one leg starting from lower back going down, try to avoid forward bending and lifting weight. Sleeping on a firm surface can help. Simple home support: Taking a pinch of dry ginger (sonth) with warm water after meals can help reduce vata and pain. Also, turmeric milk at night can support inflammation control. Ayurvedic medicines with dose and timing: Yograj Guggulu – 1 tablet twice daily after food Mahayograj Guggulu (if pain is severe) – 1 tablet twice daily after food Ashwagandha Churna – 1 teaspoon at night with warm milk Dashmool Kwath – 15 ml with equal water, twice daily after food These can help reduce nerve pain, inflammation, and support strength gradually. However, since your pain is recent and severe, and sugar is high, it is very important to consult your physician soon to adjust diabetes medication and rule out acute nerve compression if it is sciatica. You should start feeling some relief in pain within a few days to a week, but proper control of sugar is essential for lasting improvement.
