Hi! I’m an assistant to the Ask Ayurveda doctors. I’ve carefully reviewed your question.

I will explain your symptoms through the Ayurvedic perspective so you understand what is happening in your body as a whole.

Hidradenitis suppurativa (HS) and oily skin are often linked to imbalances in the Pitta and Kapha doshas. HS, characterized by inflamed skin and nodules, can be seen as a manifestation of aggravated Pitta, which governs heat and inflammation in the body. The oily nature of your skin and scalp, along with the presence of pimples and dandruff, indicates a Kapha imbalance, which tends to increase oiliness and congestion.

The root cause of these conditions in Ayurveda is often linked to internal heat, toxin accumulation (ama), and an imbalance in the body’s natural cleansing processes. An excess of Pitta can lead to inflammation and skin eruptions, while excess Kapha contributes to oiliness and sluggish elimination of toxins.

Ayurveda views these symptoms as interconnected, reflecting a systemic imbalance rather than isolated problems. The digestive system plays a crucial role here, as weak digestion can lead to toxin buildup, further aggravating both Pitta and Kapha. Stress and lifestyle factors can also exacerbate these conditions by affecting dosha balance.

To give doctors a precise and effective treatment plan, your answers to the questions below are essential. Without this information, the recommendations will be more general and less accurate.

Please answer each question as clearly as possible.

1. How severe are your HS symptoms currently? Please specify if they are mild, moderate, severe, or very severe, and if they fluctuate with certain triggers like stress or diet. 2. How often do you experience oily skin, pimples, and dandruff? Are these symptoms more frequent during certain times, such as after eating specific foods or during stress? 3. Describe your digestion. Do you experience any signs of weak digestion such as bloating, gas, or sluggishness, particularly after consuming oily or spicy foods? 4. Have you noticed any specific triggers that worsen your skin conditions, such as weather changes, stress, or certain dietary habits? 5. What is your current diet like in terms of food choices? Please specify if it includes processed, oily, or spicy foods, and how often these are consumed. 6. How would you rate your stress levels, and have you noticed any correlation between stress and flare-ups in your skin conditions? 7. Have you tried any Ayurvedic remedies or lifestyle changes before? If so, please describe what you have tried and any effects you noticed.

Your detailed responses will help in tailoring a more precise Ayurvedic treatment plan for you.

Hidradenitis Suppurativa Is infectious boil leading to sinus. To avoid deep sinus and surgical methods it should be treated early. Apply paste of neem, Haridra and tulsi over affected area, wash area with Triphala Kashaya or dashmoola kashaya. For oily face apply Multani mitti ever alternate day mixing with water, it will help to soak oil over skin. Use Anti dandruff shampoo (Sri Sri). Wash hair with neem water twice a week. Take recommended medicines for 15 days and do follow up. 1. Triphala Guggulu 1-1-1 after food along with prescribed medications. 2. Raktashodhini 10 ml -10ml -10ml after food 3. What you are experiencing in the form of hidradenitis suppurativa under the arm along with oily scalp acne around nose and dandruff is not separate problems but one internal imbalance expressing in different ways through the skin your body is showing a clear pattern of excess heat and oil along with toxin accumulation which in Ayurveda we understand as pitta kapha aggravation with rakta dushti and ama formation when digestion and metabolism are not functioning optimally toxins start accumulating in the body and instead of getting eliminated they get lodged in skin channels especially in areas like underarms where sweat glands and hair follicles are dense leading to painful swellings pus formation and recurrence along with this the same imbalance reflects as oily skin dandruff and acne so the root cause is internal not just local infection The medicines you are taking like Doxycycline and Isotretinoin help in controlling infection and reducing oil secretion temporarily but they do not correct the underlying imbalance so your decision to shift towards a deeper healing approach is valid but it requires consistency and patience because this condition heals gradually as the internal system gets corrected From an Ayurvedic perspective the focus should be on improving digestion clearing toxins purifying blood and balancing pitta and kapha so that the tendency to form boils reduces from the root you should keep your diet light warm and easy to digest avoid curd cheese fried food bakery items excess sugar non vegetarian heavy meals and cold refrigerated food as these increase both kapha and toxin formation take warm water throughout the day which helps in channel cleansing include simple foods like rice moong dal cooked vegetables bitter foods like neem based preparations and greens which help in blood purification Mahamanjishtadi Kashayam 15 ml mixed with equal quantity of warm water twice after food Amruthaaristha 15 ml with equal water twice daily after food Nimbadi kwath twice daily after food with warm water to reduce Kaishora Guggulu 1 tablet twice daily after food Triphala churna half teaspoon at night with warm water use Neelibhringadi keram apply lightly once or twice a week and wash with mild herbal shampoo avoid heavy oiling For local underarm care keep the area clean dry and free from friction you can apply a paste of turmeric and neem avoid squeezing or pressing the lesions as it worsens the tract formation wear loose cotton clothes to reduce sweating and friction This condition requires around three to six months of consistent treatment to show stable results as the body slowly corrects metabolism and clears accumulated toxins also do not stop isotretinoin suddenly if you are on it taper gradually while starting Ayurvedic medicines to avoid flare up

–HS is linked to: Blocked hair follicles + inflammation Hormonal influence (androgens) Immune system overreaction Gut health and lifestyle factors –High stress → worsens inflammation + hormones Oily/spicy diet → increases Pitta + Kapha Weak digestion → toxin buildup Naturally oily skin type → prone to blockage 👉 So yes, your instinct is right: this is an internal issue, not just a skin problem. –treatment protocol – 1. Digestion (2–3 weeks) Jeera water: 1 glass morning Hingwashtak churna: ½ tsp after meals (2× daily) 2. Detox (Ama reduction) (3 weeks) Neem tablets: 1 tab twice daily (empty stomach) Giloy: 500 mg–1 g twice daily Triphala: 1 tsp at night with warm water 3. Main Medicines (2–3 months) Kaishore Guggulu: 2 tabs twice daily after meals Mahamanjishthadi kwath: 15 ml + equal water, twice daily before meals Gandhak Rasayan: 250mg twice daily 4. Maintenance Amla powder: 1 tsp daily Aloe vera juice: 20 ml morning empty stomach

Walk: 20–30 min daily Pranayama: 10 min – Important Continue & taper slowly (don’t stop suddenly): Doxycycline Isotretinoin

Not all Ayurvedic medicines are “heating.” It depends on the herb and the balance of the formula.

Your feeling of “heat from head to toe” + HS (Hidradenitis Suppurativa) + oily skin = high internal heat + inflammation –Morning (empty stomach) Aloe vera juice: 20 ml Amla juice/powder: 1 tsp –Coconut water (3–4 times/week) Drink normal or slightly cool water (not ice cold) Avoid sun exposure + overheating –Add daily: Anulom Vilom (10 min) Bhramari (5 min) –You don’t need more “heating detox” right now You need cooling + gentle detox + stress control

1.Guggulu tikta ghrita 1 tsp with warm milk or water empty stomach in the morning 2.Arogyavardhini vati 2 tab twice daily with water after meals 3.Neem capsules 1 cap twice daily with water after meals 4.Mahamanjisthadi kwath 20 ml with 20 ml water twice daily after meals 5.Triphala churna 3/4 tsp at bedtime with warm water External Care---- - Wash underarms with neem decoction or turmeric water. - For scalp: apply neem oil + coconut oil mix twice a week. - For dandruff: apply neem oil twice weekly 3-6 hrs before hairwash. Diet & Lifestyle---- - Avoid oily, spicy, fried foods (they aggravate Kapha-Pitta). - Prefer light, cooling foods: moong dal, lauki, tori, green leafy vegetables. - Take warm water throughout the day to flush toxins. - Reduce milk + curd combination (causes Kapha blockages). - Practice stress management: pranayama (Anulom Vilom, Bhramari), meditation.

Namaste, Looking at your symptoms—hidradenitis suppurativa (HS), oily skin, pimples, dandruff, and indigestion—they are all connected. In Ayurveda, this is mainly due to pitta + kapha imbalance along with ama (toxins) and weak digestion. When digestion is not proper and internal heat increases, the skin becomes oily, inflamed, and prone to infections. That is why you are seeing pimples, dandruff, and HS, especially in areas like the underarms where sweat and friction are more. Your approach is correct—focusing on root cause and internal correction is important for long-term improvement. Start with diet and lifestyle: Avoid spicy, oily, fried food, bakery items, excess non-veg, and sugar. Keep food simple—home-cooked meals, green vegetables, bottle gourd, ridge gourd, bitter gourd, and amla. Sip warm water through the day. Stress plays a big role, so try 10–15 minutes of daily deep breathing or meditation. Simple home care: Keep the underarm area clean and dry. You can gently wash with neem water and apply a thin layer of aloe vera gel to reduce irritation. For scalp: Use a mild neem or tea-tree based shampoo 2–3 times a week. Avoid heavy oiling since your scalp is already oily. Ayurvedic medicines with dose and timing: Arogyavardhini Vati – 1 tablet twice daily after food Mahamanjisthadi Kwath – 15 ml with equal water, twice daily Gandhak Rasayan – 1 tablet once daily Triphala Churna – 1 teaspoon at night These help in improving digestion, reducing inflammation, and supporting skin healing from within. You may start noticing improvement in pimples and dandruff within 3–4 weeks. HS flare-ups usually take longer and need consistent care for a few months. If any swelling becomes painful, with pus or fever, then taking antibiotics as advised by your doctor is important—Ayurveda can be continued as supportive long-term care.
