Alcohol may have caused swelling or infections in the liver. For this, you should get an abdominal USG and kidney function test done. I am giving you some medicine. Take medication upto 4 to 5 months • 1 glass of water,1 teaspoon of grated ginger,½ teaspoon of turmeric powder,½ teaspoon of cinnamon powder,1 teaspoon of lemon juice,1 teaspoon of apple cider vinegar,Boil one glass of water in a small pan,Add the grated ginger, turmeric, and, cinnamon powder. • Let it simmer until the water reduces slightly and the aroma becomes sharp.Strain it into a cup. When it turns warm (not hot), mix in the lemon juice and apple cider vinegar.Drink slowly on an empty stomach. ●Avipattikar Churna – 1 tsp at bedtime with warm water Reduces acid reflux, improves liver and bowel movement ●Triphala + Isabgol 1 tsp with warm water at night Relieves constipation, gently detoxifies gut ●Arogyavardhini Vati – 2 tablets after meals, twice daily Liver cleanser, improves fat metabolism ●Punarnavadi Mandur – 1 tablet twice daily before meals Reduces water retention, bloating, and helps the liver function. ●Guduchi Satva – with warm water, 2 times Boosts digestion, immunity, ●Drinks that will help your gut and metabolism to boost ●Cumin + Coriander + Fennel Tea (Boil 1 tsp of each in 2 cups water → reduce → sip warm) ●Ginger + Tulsi Tea (if not acidic)

Your symptoms (leg swelling + right upper abdomen pain + long alcohol history) need proper medical evaluation also. Low alkaline phosphatase is not typical liver damage marker, so don’t rely only on that. Possible causes Early liver disease / fatty liver changes Protein deficiency Fluid retention (check kidney & heart also) Bhumyamalaki churna – 3 grams, twice daily after food with water Kalmegh tablet – 1 tablet twice daily after food For swelling (edema) Punarnava mandoor – 1 tablet twice daily after food Punarnava kwath – 20 ml twice daily before food Improve digestion & metabolism Arogyavardhini vati – 1 tablet twice daily after food Diet advice Avoid oil, fried, heavy, non-veg for few weeks Take light diet rice gruel, green gram, boiled vegetables Reduce salt (important for swelling) Drink warm water

Hello, Given your history of chronic alcohol use and the current presentation of leg swelling (edema) and right upper quadrant pain, this suggests the liver is still recovering from long-term congestion or structural changes, even though you have been sober for 18 months. In Ayurveda, this is viewed as a disturbance of Yakrit (Liver) and Vyan Vayu, leading to Shotha (swelling) due to the accumulation of toxins (Amavisha). Low alkaline phosphatase, while less common than high levels, can sometimes correlate with nutritional deficiencies or specific metabolic shifts during liver regeneration. The goal is to improve liver micro-circulation, reduce water retention, and provide “Medhya” (cooling/healing) effects to the hepatic tissues. Start this meodicne : 1. Punarnavasavam: This is the primary liquid for edema. It acts as a natural diuretic and liver tonic. Dose: 20 ml with equal quantity of lukewarm water, twice daily after food. 2. Heposem Tablet: A specialized formulation for liver protection and correcting functional imbalances. Dose: 1 to 2 tablets twice daily before food. 3. Sahacharadi Thailam (External): To address the leg swelling directly without putting a load on the liver. Application: Gently massage onto legs in an upward direction (towards the heart) to improve lymphatic drainage. Diet: Strictly limit salt intake, as sodium worsens leg swelling. Focus on easy-to-digest foods like mung dal soup and bottle gourd. Clinical Correlation: Since you are a medical professional, ensure you rule out cardiac or renal involvement (check Serum Albumin and Creatinine), as chronic liver issues can sometimes lead to secondary hypoalbuminemia, causing that edema. Monitoring: If the abdominal pain becomes sharp or the swelling increases rapidly, please consider an abdominal ultrasound to check for portal hypertension or fatty changes.

Hello I can understand your concern You have already taken the most important and positive step that is stopping alcohol But now you are having swelling in legs with pain in right upper abdominal pain along with fatigue This has to be carefully evaluated as there may be stress in liver ▪️Important The symptoms mentioned may be due to early liver damage or fluid imbalance Even though your bilirubin is normal We need further test to come to a final diagnosis Please get it done as early as possible -Liver function test -Serum Albumin ( important as you are having swelling) -Ultrasound of abdomen and pelvis -PT INR This all will help us to rule out underlying liver stress ▪️Ayurvedic plan of treatment Phyllantus syrup 10 ml twice daily after food Arogyavardini vati 1 tablet twice daily after food Guduchyadi kwath 10 ml twice daily after food Punarnavasa kwath tablets 2 tabkets twice daily after food This all are liver protective, reduce the swelling and help in reducing overall liver stress Diet and lifestyle Have more of moong dal Lauki Tori In moderate coconut water Keep your legs elevated Avoid Alcohol Oily food Fermented food Red meat Your body is getting adjusted and recovered from long term alcohol intake Please do the required test so that we can come to a confirm diagnosis and help to prevent further liver damage Wishing you a good health😊 Warm Regards Dr Snehal Vidhate

can you please share your full reports including your CBC. are you under any other medications like diabetes, hypertension, thyroid dysfuction? if yes share those reports and the medicines you are currently on. how old are you? whats your body weight and bmi? any nausea or vomiting? sharp pain in abdomen? zinc and magnesium levels?how is your urine flow? all these questions need to be answered before moving on to complex internal medicatios. please follow the medications advised and use the kashayya mentioned above like 60 ml kashaya added in 2 liters of hot water and immerse your foot in that . also helpful if yopu can boil the water using moringa leaves, castor, arka or tamarind leaves, use the available leaves among these. the water can also be poured down on legs over the swelling in a stream.

The symptoms which you provide above is pointing towards shotha and yakrit dalodara Due to past alcohol related complication 👇👇 So our focus is to protect the liver and reduce the swelling while your bilirubin is normal the low alkaline phosphatase and physical swelling pointing that we need to improve liver metabolism and circulation 👇👇👇👇👇👇👇👇 1) punarnavadi Kashayam- 15 ml with 45 ml lukewarm water 30 minute before breakfast and dinner 2) aarogyavardhini Vati- one tablet with lukewarm water after breakfast and dinner 3) kumaryasavam- 20 ml with 20 ml water after lunch and dinner 3) patolkaturohinyadi kashayam- 15 ml with 45 ml lukewarm water 30 minute before lunch 👇👇👇👇👇👇👇 What to Eat: Minimizing salt intake is the most critical step to reducing leg swelling. Focus on Mudra Yusha (Moong dal soup), Laja (puffed rice), and bottle gourd (Lauki). Include bitter gourd (Karela), papaya, and drumstick (Sahjan) in your meals. Drink water boiled with Coriander seeds, but keep total fluid intake moderate as per your thirst. What to Avoid: Avoid curd, pickles, papads, and fermented foods (Idli/Dosa). Stay away from deep-fried foods, heavy sweets, and buffalo milk products. Avoid sleeping during the day, as it slows down metabolism and increases swelling. 👇👇👇👇👇👇 When sitting or sleeping, keep your legs elevated above the level of your heart using pillows to help the swelling subside. Avoid long periods of standing or sitting still. Gentle walking for 10-15 minutes 👍👍👍 Since you have successfully quit alcohol (which is a great step for your health), consistency with these herbs will help the liver tissues regenerate more effectively.

With a history of alcohol intake for many years even though you have stopped since one and half years the liver may still be in a recovery phase and when you are now noticing swelling in the legs along with pain in the right upper abdomen it should be taken seriously even if bilirubin is normal and appetite is fine Low alkaline phosphatase is not a typical finding in common liver damage but it can be seen when there is weakness in metabolism poor nutrition or reduced liver activity so it is important to look at the complete picture with other reports and not depend on a single value Swelling in the legs can happen when there is low protein in the body or fluid retention so it is better to check serum albumin total protein kidney function and also get an ultrasound abdomen done to understand the liver condition clearly From an ayurvedic understanding this shows imbalance in liver function along with disturbance in body fluids and digestion so the approach should be to support the liver improve metabolism and reduce fluid accumulation in a gentle way You can start bhumyamalaki churna half teaspoon twice daily after food with warm water for liver support along with punarnava mandoor one tablet twice daily after food to help reduce swelling and improve fluid balance arogyavardhini vati one tablet twice daily after food can be added if digestion is good to support liver function and metabolism and dashamoola kwath fifteen ml twice daily after food with equal warm water can help in reducing inflammation and balancing the system Keep the diet simple and light avoid oily fried spicy and processed foods completely continue to avoid alcohol take food like rice moong dal boiled vegetables with a little ghee and drink warm water through the day avoid sleeping immediately after meals Regular monitoring is important if swelling increases or if you notice breathlessness tiredness or increase in abdominal size then do not delay further evaluation with proper care and discipline the liver can improve gradually over time

Hello, Given your history of long term alcohol use, leg swelling + dull right upper abdominal pain + fatigue, even with normal bilirubin and no jaundice, can till suggest a liver biliary issue, fatty liver/ alcoholic liver injury sequelae, early portal congestion, protein deficiency, or even non liver causes like kidney, heart, venous insufficiency, or low albumin. Also, low alkaline phosphatase (ALP) is unusual in classic obstructive liver disease; it can sometimes be seen with nutritional deficinecy (zinc, magnesium), hypothyroid states, malabsorption, or rarely advanced metabolic issues. So the first point is do not assume this is “just liver heat” or only due to past alcohol- it needs evaluation From an Ayurvedic view, this can be understood as -liver functional imbalance -vitiation affecting blood and circulation -kapha accumulation + meda imbalance contributing to swelling -vata obstruction causing dull chronic pain and bloating -edema/swelling disorder -possible weakened digestive-metabolic fire after years of alcohol impact Alcohol over many years can disturb pitta (especially ranjhak pitta in liver), weaken agni, produce ama (metabolic toxins), and affect rasa/rakta dhatu (fluid and blood channels) , leading to fluid retention, fatigue, and abdominal discomfort Treatment goals -support liver function -reduce edema -improve digestion and reduce ama -support circulation and fluid metabolism -correct possible nutritional depletion -prevent progression into more serious liver or systemic disease Internal medication 1) Punarnavadi kasahaya= 15ml + equal water twice before meals =classical for edema, fluid retention, urinary support, liver support 2) Punarnavadi manduram= 1 tab twice daily after meals =low digestive sterength, and hepatosplenic congestion patterns are considered 3) Arogyavardhini vati= 1 tab twice daily after meals =used for metabolic support, sluggish digestion, ama reduction 4) Bhumiamalaki capsule= 1 cap twice daily after meals =traditionally valued for liver support 5) Avipattikar churna= 1 tsp with warm water at bedtime =helpful in bloatin and pitta agni disturbance predominate Diet -light, digestible, low salt food -warm cooked meals over heavy oily meals -bottle gourd, ridge gourd, pumpkin -pomegranate -amla if tolerated -moderate protein intake Reduce or avoid -high salt -fried food -very sour, very spicy foods -red meat in excess -packaged snacks -sugary foods -absolutely avoid alcohol permanently Home remedies -warm water through the day in small quantities -cumin-coriander-fennel light infusion -elevate legs when resting helps swelling -gentle walking unless swelling is severe -avoid proloned standing Yoga/pranayam -avoid strong abdominal pressure practices until evaluated -can perform vajrasaana after meals -pawanmuktasana -anulom vilom -bhramari Because leg swelling + right upper abdominal pain can sometimes be serious consult immediately if you have -increasing abdominal distention -breathlessness -rapidly worsening leg swelling -fever -confusion -black stools or vomiting blood -severe pain -reduced urination Do follow Hope this might be helpful Thank you

✓A 20-year history of daily alcohol intake places immense structural stress on the liver. The combination of dull right upper quadrant pain and progressive leg swelling over the last few months strongly suggests early portal hypertension and fluid retention, even if bilirubin remains normal. ✓A low alkaline phosphatase (ALP) level is unusual in liver disease but is a classic clinical marker of severe zinc or magnesium deficiency, which is extremely common after decades of heavy alcohol consumption. ✓Ayurvedic Diagnosis: Yakrut Roga (Liver dysfunction) complicated by Shotha (edema/swelling). The liver’s metabolic fire is compromised, leading to fluid pooling in the lower extremities, generalized fatigue and occasional gaseous bloating in the gut. ✓Ayurvedic Prescription Arogyavardhini Vati Dosage: 2 tablets twice daily. Time: 30 minutes after meals. Vehicle: Warm water. Punarnavadi Mandur Dosage: 2 tablets twice daily. Time: To be taken alongside the Arogyavardhini Vati. Vehicle: Warm water. Phalatrikadi Kwath Dosage: 15 ml mixed with 15 ml of warm water. Time: Twice daily, on an empty stomach (30 minutes before meals). ✓Crucial Diet & Care Protocols Diagnostic Recommendation: You must consult a hepatologist or gastroenterologist for an urgent Ultrasound or FibroScan of the liver. The physical swelling and liver capsule pain require structural imaging to rule out advanced fibrosis or early cirrhosis. Strict Sodium Restriction: To immediately assist the herbal medicines in reducing your leg swelling, salt intake must be cut drastically. Completely avoid pickles, table salt, and all processed or packaged foods. Therapeutic Sweating: Engage in mild, passive Swedana (herbal steam therapy) for your lower body if physically comfortable. Sweating helps the skin safely eliminate trapped fluid and toxins, naturally reducing the filtration burden on your liver and kidneys. Nutritional Replenishment: Since the low ALP indicates a high probability of cellular zinc deficiency, incorporate zinc-rich, easy-to-digest foods into your daily meals, such as thoroughly cooked lentils, roasted pumpkin seeds, and sesame seeds. Regards Dr Gursimran Jeet Singh MD Panchakarma
