My mother due to diabetic complications developed stage 3a kidney decease and nuroapthy. Though her A1C is at 7.5 now, earlier it is around 10. I am attaching her current lab results. CREATININE: 1.2 EFGR (estimated glomerular filtration rate) : 51.3 ALBUMIN CREATINE RATIO 38.5 We are planning to use Aimil Amyron tablets morning 2 and evening 2 Chandra Prabha guti vati morning 2 and evening 2 gukshuradi guggulu morning 2 and evening 2 She also developed bloating and constipation. Can you please prescribe good ayurveda medicines for this ( bloating and constipation. & good bowel movements) which does not effect her kidneys, does not cause elocrolyte iussues.

For diabetic kidney disease stage 3A with neuropathy, avoid strong laxatives, excess salts, and long-term harsh bowel cleansers. Current kidney values Creatinine 1.2 eGFR 51.3 ACR 38.5 This suggests mild-moderate CKD, so medicines should be gentle. Your planned medicines Chandraprabha Vati Gokshuradi Guggulu can be used cautiously under physician supervision. Avoid too many guggulu-based medicines if acidity, dehydration, or kidney strain increases. For bloating + constipation + safe bowel movement: Avipattikar Churna 1/2 tsp at night after food with warm water Good for gas, bloating, mild constipation Gandharvahastadi Kashayam 10 ml with 20 ml warm water at bedtime Gentle bowel movement support Triphala tablet (mild dose only) 1 tablet at bedtime Avoid high doses in CKD Hingvastak Churna 1/4 tsp with first morsel of lunch and dinner Helps digestion and gas Diet Warm water frequently Bottle gourd, ridge gourd, cooked vegetables Small quantity soaked black raisins daily Avoid bakery foods, fried food, excess curd at night Important: Avoid strong virechana herbs, castor oil regularly, senna, or excessive Triphala because electrolyte imbalance and dehydration can worsen kidneys. Mild walking after every meals. Avoid dehydration. Intake more fibrous foods. Include more watery fruits and vegetables. Green leafy vegetables also give good results. – Intake 1/2 tsp of ajwain after every meals. – you can also add fried jeeraka and boil it in some water, intake after every meals. First of all as you mentioned she have kidney issues it will be my advice to stop medicines. Be it allopathy be it ayurvedic all medicines have to pass the kidney to be eliminated through the body. Include medicines only that are given by her nephrologist and donot overload her kidney with more medications. For constipation: As she has a water intake limitation the constipation is a result of low water intake. Keep her adviced water intake in check. Give her soaked 2-3 black rasines twice a day. Give her a glass of water water or warm liquid as let her do these asan that will help : Kraunchasan, Malasan, Setubandhasan, Balasan. Castor oil Mix it in her roti ka aata and use it during bed time.

Because she has Stage 3a kidney disease, we should use only gentle medicines that improve digestion and bowel movement without causing dehydration or electrolyte imbalance. ----Safer supportive options: 1) Psyllium (Isabgol) 1 tsp at night with warm water 2) Small amount of Ghee at bedtime for dry constipation 3) Low-dose Triphala 1 tab at bedtime 4) Ajwain–jeera–saunf water after meals for bloating —Rx: (1) Isabgol) – 1 tsp HS with lukewarm water – 1 tab HS if constipation persists 2) Hingvashtak Churna – 1/2 tsp with first morsel of food BD 3) Ajwain + Jeera + Saunf boiled water after meals – 1 tsp HS with warm water 4) tab amyron aimil 2 tab twice daily after meal —Advice: Warm, light, easily digestible diet Avoid cold, heavy, fermented, gas-forming foods Daily mild walking after meals Avoid strong virechana/purgative medicines due to CKD Monitor bowel habits, swelling, urine output, and kidney parameters regularly

●Triphala powder • 1 teaspoon of Triphala • 1 cup of warm water • Soak for 5-10 minutes • Consume at night If you have frequent urination at night, take Triphala in the morning on an empty stomach. ●Why is fiber important? Fiber softens and makes stools more mobile. Include in your diet: • Porridge • Oats Wheat bran • Whole grains • Fresh fruits and vegetables Fiber + water = easier bowel movements. ●Castor Oil (in special circumstances) Method – • Ginger tea • Add 2 teaspoons of castor oil • Consume at night. Do not use regularly as it can be habit-forming. ●Use of Flaxseed At night • 1 teaspoon of flaxseed • Boil in 1 cup of water • Drink with the seeds. I must urgently correct the dosage of your planned protocol. While Chandraprabha Vati and Gokshuradi Guggulu are excellent classical choices for diabetic nephropathy to protect the kidney’s filtering units, giving her 4 tablets of each per day is too heavy for a kidney functioning at 50% capacity. Overloading a compromised kidney with high doses of herbal extracts will increase her fatigue and strain her liver. Furthermore, her bloating and bowel movements occurring only every other day are classic signs of Vata aggravation, heavily worsened by the diabetic neuropathy, which slows down the nerve signals to her colon (gastroparesis/diabetic enteropathy). We need a solution that clears the bowel without drawing excessive water into the colon, ensuring zero risk of electrolyte imbalance. 1. Adjusted Kidney Protocol Gokshuradi Guggulu & Chandraprabha Vati: Reduce the dosage to exactly 1 tablet of each, twice a day (total 4 pills a day, not 8). Give them 30 minutes after meals with a sip of water. This will gently reduce the microalbuminuria without toxicity. Aimil Amyron: Reduce to 1 tablet twice a day. Taking 4 a day is unnecessary and heavy on the gut, contributing to her severe bloating. 2. Constipation and Bloating (Kidney-Safe Solution) Eranda Bhrishta Haritaki Dosage: 1/2 to 1 teaspoon. Time: At night, 30 minutes before going to sleep. Vehicle: A small glass of warm water. It is the safest, most effective bowel regulator for CKD patients. Unlike Triphala or salts (which can spike potassium or sodium), it strictly lubricates the dry colon, repairs the damaged neuropathy nerves, and relieves the trapped gas without any electrolyte shifting. ✓Crucial Care Rules Avoid High Potassium Laxatives: Do not use over the counter Ayurvedic powders like Panchasakar Churna, Lavan Bhaskar or Isabgol (Psyllium husk). They either contain high salts that will raise her blood pressure and damage the kidneys, or they act as bulk-forming laxatives that will drastically worsen her bloating. ✓Since her kidneys are at Stage 3a, do not force her to drink excessive water to clear the constipation. She should only drink to satisfy her thirst. Over-hydration will cause fluid to accumulate around her ankles and lungs. To protect her kidneys, diabetes should be kept under control She can add Glymin tablet 1-0-1 after food with water, along with other diabetes medicine taking. Chandraprbha vati 1-0-1 after food with water, Gokshura powder 1tsp -1tsp -0 after breakfast with water, Also Amyron is herbo mineral ayurvedic medicine, 2-0-2 is high dose for CKD patient .can take 1-0-0 after food with water. High dose with egfr- 51.3 is not advisable. This combination to start with. Later gradually can increase the dose if required. For bloating and constipation Start with Tablet Heposem 1-0-1 after food with water, will help improve digestion. Biogest tablet 1-0-1 after food with water will help prevent bloating Gandharvahastadi castor oil 10ml. After dinner with warm water , will relieve constipation . Restrict liquid and water to arround 1 litre. Only. Lessen intake of salt in your diet. Do pranayam lom -vilom bhastrika bhamri , kapalbhatti 5-10mins twice daily.
