Hello, Managing hypertension in a patient with diabetic nephropathy (protein leakage) requires a delicate balance to protect the kidneys while stabilizing blood sugars. In Ayurveda, this is often viewed as a complication of Prameha (diabetes) affecting the Vrikka (kidneys) and Mutravaha Srotas (urinary channels). The focus is on Mutra-Prasadana (clearing the urinary tract) and Rasayana (rejuvenation) to repair the filtration unit of the kidneys. Since there is significant protein leakage (2.91 ratio), it is vital to avoid heavy, salty, or excessively spicy foods that increase Pitta and Vata, further damaging the renal tissues. Prescription : 1. Punarnavadi Kwath: This is a primary decoction for reducing swelling and improving kidney function. It helps manage blood pressure by acting as a natural, potassium-sparing diuretic. Dose: 15 ml diluted with 60 ml of lukewarm water, taken twice daily 30 minutes before food. 2. Chandraprabha Vati: A potent formulation for urinary disorders and diabetes-related complications. It helps reduce protein leakage and tones the urinary system. Dose: 2 tablets twice daily after food. 3. Varanadi Kwath: Often used when hypertension is associated with metabolic issues, helping to clear internal blockages (Srotas). Dose: 15 ml diluted with 60 ml of lukewarm water, taken twice daily before food. Strict Sodium Control: Limit salt intake to less than 3–5 grams per day. Avoid processed “diet” snacks, as they often contain hidden sodium. Protein Management: While protein is leaking, high intake of heavy proteins (like red meat or heavy lentils) can strain the kidneys. Stick to light, easy-to-digest proteins like Moong Dal. Hydration: Water intake must be regulated based on whether he has any swelling (edema). If swelling is present, consult his doctor for a specific fluid limit. Stress Reduction: High cortisol levels spike both blood pressure and glucose. Gentle Pranayama (like Anulom Vilom) for 15 minutes daily can significantly help.

Managing your uncle’s health requires a careful, dual-action approach. His high urine protein/creatinine ratio (2.91) and uncontrolled diabetes (HbA1c 7.3) suggest Diabetic Nephropathy—a condition where the kidneys are struggling to filter waste properly. In Ayurveda, this is often viewed as a complication of Madhumeha (Diabetes), leading to Vrikka Roga (Kidney issues) due to an imbalance in Vata and Kapha doshas. I have prescribed medications take them consistently and you will se results and also you can consult me on personal consultation . 🌱Dietary Management (Crucial for Nephropathy) Since the kidneys are losing protein, the diet must be low-sodium and moderately low-protein to prevent further damage.  The “Dos” (Best Choices)✅ • Lauki (Bottle Gourd) & Turai (Ridge Gourd): These are light, easy to digest, and naturally diuretic. • Old Rice & Barley: Barley (Yava) is traditionally used in Ayurveda for urinary disorders and diabetes. • Moong Dal: The only lentil that is light enough for the kidneys during protein leakage. • Boiled/Steamed Food: Reduces the intake of complex fats and processed salts. The “Don’ts” (Avoid Strictly)❌ • Salt (Sodium): High salt is the primary driver of hypertension. Use rock salt (Saindhava Lavana) in very small quantities. • Potassium-Rich Foods: If kidney function is compromised, avoid bananas, citrus fruits, and coconut water unless a doctor approves. • High Protein: Avoid paneer, heavy dals (Rajma, Chole), and non-veg, as they increase “BUN” (Blood Urea Nitrogen) levels. • Processed Foods: No pickles, papads, or canned soups—these are hidden salt bombs.  ♻️Lifestyle & Natural Reducers • Pranayama (Sheetali & Bhramari): These cooling breathing exercises are scientifically shown to lower the “fight or flight” response, naturally reducing blood pressure.  • Foot Massage (Padabhyanga): Massaging the soles of the feet with warm sesame oil before bed can calm the nervous system. • Monitor Water Intake: You mentioned 1–2 liters; ensure he doesn’t exceed this unless recommended, as excess fluid can increase blood pressure if the kidneys aren’t filtering well.

Not only natural therapy but also supportive allopathy medications will be required for his condition to heal. At this moment controlling diabetes to reduce further damage will be the main goal. However ALLOPATHY and ayurveda both have to play amjor role in the treatment. Lifestyle that has to be strictly followed: ?? Do ?? Drink buttermilk Shift to millet roti Dinner at 7.30 or 8 100 steps daily after both meals Best to have lunch at 11.30-12 and dinner at 7.30-8 ?Don’t? Anything white like wheat, refined flour, sugar, salt, potato, sweet potato Oily food Packed food Processed food Ready to eat food Dairy products Curd Dried Fish Medications: Syp. Varunadi kwath 3 tsp twice a day before food Tab.Zpter 2 tabs twice a day before food Tab. Chandraprabha Vati 2 tabs twice a day before food. Follow up after 1 month to see hoe bosy reacts.

Your uncle’s reports suggest early diabetic kidney involvement with hypertension, so along with regular physician/nephrologist follow-up, Ayurveda can help support blood pressure, kidney health, and sugar balance naturally. Ayurvedic Understanding This condition is mainly related to: Vata + Kapha imbalance Weak Mutravaha srotas (urinary/kidney channels) Long-standing high sugar affecting circulation and kidneys The goal is to: Control blood pressure naturally Reduce kidney strain and protein leakage Improve diabetes control Protect heart and kidney function Natural Ayurvedic Measures Diet (Most Important) Strictly reduce: Salt, packaged foods, pickles, chips, bakery items White sugar, sweets, cold drinks Excess tea/coffee Deep fried and heavy foods Prefer: Lauki (bottle gourd), tori, karela, parwal Barley water, old rice, moong dal Garlic in small quantity Fresh fruits in moderation (guava, apple, papaya) Avoid excess protein supplements and red meat because kidneys are already stressed. Helpful Ayurvedic Herbs Arjuna – supports heart and BP balance Punarnava – supports kidney function and swelling control Gudmar + Meshashringi – helps sugar metabolism Gokshura – supports urinary system Amla – antioxidant and diabetic support Common classical combinations often used: Punarnavadi kwath Chandraprabha vati Arjunarishta Nishamalaki (Use only after checking compatibility with his current medicines and kidney status.) Lifestyle Advice 30–40 minutes slow walking daily Gentle yoga: Anulom Vilom Bhramari pranayama Vajrasana after meals Sleep on time and avoid stress/anger Maintain healthy weight Water Intake Do not force excessive water. Around 1.5–2 liters/day is usually reasonable unless his doctor has given fluid restriction. Very Important Medical Advice Because the urine protein/creatinine ratio is 2.91, kidney damage risk is significant. He should: Regularly monitor BP, creatinine, eGFR, and urine protein Keep HbA1c ideally below 7% Continue prescribed BP and diabetes medicines regularly Consult a nephrologist if swelling, reduced urine, breathlessness, or rising creatinine occurs Simple Daily Routine Morning: Warm water Light walk + pranayama Meals: Low salt, diabetic-friendly diet Night: Early dinner Avoid heavy or salty foods

Your uncle’s reports suggest diabetic kidney involvement with significant protein leakage, so controlling blood pressure and sugar strictly is very important to protect kidney function further. Natural support can help, but prescribed medicines should not be stopped. Main focus should be: Tight diabetes and BP control Kidney-friendly diet Salt restriction Regular monitoring Avoid: Excess salt, packaged foods, pickles, fried foods Excess protein supplements or heavy gym diets Painkiller overuse Encourage: Regular walking Proper sleep Stress reduction Controlled carbohydrate intake Water intake should be according to kidney status and doctor advice, not excessive forceful drinking. At home: Barley water in moderation and bottle gourd soup can support kidney health if digestion is comfortable. Punarnava mandoor – 1 tablet twice daily after food Chandraprabha vati – 1 tablet twice daily after food Arjuna tablet – 1 tablet twice daily after food These may support kidney function, BP balance, and metabolism gradually. Regular monitoring of creatinine, urine protein, BP, and HbA1c is very important. A nephrologist follow-up should continue regularly. Regards, Dr Raghuveer (Ayurvedacharya)
