••In Ayurveda, your situation is viewed through a specific lens: a history of severe Amla Pitta (chronic hyperacidity) and Parinama Shula (duodenal ulcer) has naturally weakened your Agni (digestive fire). Following surgery and years of depleted absorption, this has led to Pandu (anemia) and Dhatukshaya (tissue depletion leading to weakness), which is further compounded by the natural dominance of Vata dosha in this stage of life. Because you are dealing with chronic anemia and are in your 80s, it is absolutely vital to consult a qualified Ayurvedic physician in person before starting any strong herbo-mineral formulations. Here is a supportive, gentle Ayurvedic framework designed to revive your digestion and build your blood levels without aggravating your stomach. 1. Cooling & Healing the Stomach Lining Before we can successfully build your blood and iron levels, we have to make sure your stomach can absorb nutrients comfortably without triggering acidity or pain. Yashtimadhu (Licorice): This is exceptionally gentle and acts as a natural demulcent, coating and soothing the mucosal lining of the stomach and duodenum. Shatavari: Known for its cooling (Sheeta) properties, it helps pacify Pitta and promotes tissue healing while being very gentle on an elderly digestive tract. Amalaki (Indian Gooseberry): Unlike other sour fruits, Amalaki actually reduces Pitta (acidity) and is a massive natural source of Vitamin C, which is crucial for iron absorption. 2. Addressing Anemia (Pandu) Gently Traditional iron supplements often cause constipation or gastric irritation. Ayurveda uses naturally processed, bioavailable options that are combined with digestive herbs to prevent this. Dhatri Lauha: A classical formulation where iron is processed with Amalaki. It is specifically indicated for Parinama Shula (ulcer pain) and Pandu (anemia) because the Amalaki buffers the iron, making it highly absorbable and cooling to the stomach. Drakshadi Lehyam or Draksharishta: A herbal jam or mild fermented preparation made from raisins. It naturally builds hemoglobin, relieves weakness, and ensures smooth bowel movements, which prevents Vata from building up in the colon. 3. Dietary Regimen for Deep Nourishment When Agni (digestion) is weak, the food must be easy to break down, otherwise, it turns into Ama (toxins) instead of blood. Pomegranate & Black Raisins: Both are excellent Rakta-vardhaka (blood-building) foods. Soaking 10–15 black raisins in water overnight and chewing them in the morning is incredibly supportive. Manda and Yusha (Rice gruel and Moong Dal Soup): Prepared with a pinch of cumin and coriander, these give immediate energy without straining your stomach. A2 Cow’s Ghee: Taken in small quantities (half a teaspoon with warm meals), ghee heals the gut lining, stimulates the digestive fire, and calms Vata.

The patient is in his/her 80s and has been suffering from severe acidity and stomach pain for more than 30 years. A surgery for a duodenal ulcer was performed at around 60 years of age. Despite ongoing allopathic treatment, there has been no significant improvement. ----Currently, the patient reports: Severe, persistent stomach pain Acidity aggravated by spicy foods Very poor appetite Marked weakness and fatigue Low energy levels throughout the day Anemia, for which iron supplements are being taken ----There is no history of lifestyle diseases such as diabetes, hypertension, or heart disease. ----Considering the patient’s advanced age, long-standing gastrointestinal symptoms, poor appetite, anemia, and persistent weakness, a thorough re-evaluation is recommended. This may include assessment for ongoing gastrointestinal blood loss, nutritional deficiencies (iron, vitamin B12, folate), recurrence of ulcer disease, gastritis, malabsorption, or other age-related gastrointestinal conditions. -----Rx: 1) Avipattikar Churna – 3 g twice daily after meals with lukewarm water. 2) Kamdudha Ras (Muktiyukta) – 1 tablet twice daily after meals. 3) Draksharishta – 15 ml with equal water twice daily after meals. 4) Punarnava Mandur – 2 tablets twice daily after meals. 5) Shatavari Kalpa – 1 teaspoon twice daily with milk, if tolerated. ----Diet Advice (Pathya): Soft, freshly prepared food. Khichdi, rice gruel, cooked vegetables, pomegranate, ripe banana. Adequate hydration. Small frequent meals. ----Avoid (Apathya): Spicy, fried, sour, and very hot foods. Excess tea, coffee, tobacco, and alcohol. Long gaps between meals. ----Follow-up: Review after 2–4 weeks
Your long history of acidity, previous duodenal ulcer surgery, poor appetite, anemia, weakness, and persistent stomach pain at this age deserve careful attention. While Ayurveda can help improve digestion, strength, and quality of life, it is important not to assume that all symptoms are due to “acidity” alone. In an 80+ year-old patient with anemia and poor appetite, further medical evaluation is also necessary. Clinically, the most likely factors are: chronic hyperacidity and impaired digestion (Agnimandya), post-surgical digestive weakness after ulcer surgery, nutritional deficiencies causing anemia and fatigue, and possible age-related gastric mucosal changes. Persistent severe pain despite treatment warrants ruling out recurrent ulceration, occult bleeding, or other gastrointestinal conditions. From an Ayurvedic perspective, this appears to be a combination of aggravated Pitta in the stomach with Vata depletion and Dhatu Kshaya (tissue weakness), leading to both acidity and loss of strength. I would consider the following Ayurvedic support • Avipattikar Churna 3 grams after lunch and dinner with lukewarm water for 6–8 weeks to help reduce acidity and burning. • Yashtimadhu (Mulethi) Churna 2 grams twice daily mixed with honey or warm water after meals, provided there is no uncontrolled hypertension. It helps soothe the gastric lining. • Shatavari Kalpa 1 teaspoon twice daily with warm milk (or water if milk is not tolerated) to nourish tissues and support recovery. • Punarnava Mandur 1 tablet twice daily after meals may be useful if anemia is due to iron deficiency, but only after reviewing hemoglobin and iron studies. Diet is extremely important. Prefer soft, freshly cooked foods such as rice, moong dal, cooked vegetables, pomegranate, ripe banana, and thin rice gruel. Take small frequent meals rather than large meals. Avoid spicy foods, pickles, excess tea/coffee, fried foods, sour foods, long fasting, and late-night meals. A simple home remedy is 1 teaspoon of ghee in warm rice or soft food daily if digestion permits; this often reduces gastric irritation and supports nourishment in elderly patients. At your age, strong Panchakarma procedures are generally not advised unless assessed in person. Gentle Brimhana (nourishing) therapies and mild Abhyanga (oil massage) may help weakness and improve comfort. Please arrange a review of CBC, iron profile, vitamin B12, stool occult blood test, and upper GI evaluation if not done recently. Seek urgent medical attention if there is black stool, vomiting blood, unexplained weight loss, difficulty swallowing, or worsening pain. With consistent treatment and dietary correction, acidity may improve within 3–6 weeks, while strength and anemia usually require 2–3 months of focused management
