Doctors' responses

Hi! I’m an assistant to the Ask Ayurveda doctors. I’ve carefully reviewed your question.

I will explain your symptoms through the Ayurvedic perspective so you understand what is happening in your body as a whole.

Bald patches and scalp eczema are often interconnected issues in Ayurveda, reflecting an imbalance in your body’s doshas, primarily Pitta and Vata. The bald patches may be due to an aggravated Vata dosha, which is responsible for dryness and brittleness, leading to hair loss. On the other hand, scalp eczema or psoriasis is often a manifestation of an imbalanced Pitta dosha, causing inflammation, redness, and itchiness on the scalp.

Both conditions can indicate a deeper systemic imbalance, where excess heat (Pitta) and dryness (Vata) are affecting your scalp health. This imbalance might be linked to your digestion, stress levels, and lifestyle habits.

To give doctors a precise and effective treatment plan, your answers to the questions below are essential. Without this information, the recommendations will be more general and less accurate.

Please answer each question as clearly as possible.

1. How long have you been experiencing bald patches and scalp eczema? Please choose from: Less than 1 month, 1-3 months, 3-6 months, or More than 6 months. 2. How would you describe the severity of your scalp eczema? Is it mild with occasional itching, moderate with frequent itching and redness, severe with constant discomfort and flaking, or very severe with pain and inflammation? 3. Have you noticed any specific triggers for your eczema? Could it be certain hair products, stress, dietary choices, or are there no clear triggers? 4. Do you have any other associated symptoms such as flaking or scaling of the scalp, hair loss in other areas, redness or irritation, or no other symptoms? 5. How is your overall digestive health? Would you say it’s good with regular comfort, occasional issues, frequent digestive discomfort, or chronic digestive problems? 6. What is your current hair care routine? Do you regularly shampoo and condition, use specialized products for scalp issues, have minimal hair care, or not use any hair products? 7. Have you tried any treatments for your scalp issues before? Have you used home remedies, over-the-counter products, prescription medication, or is this the first time seeking help?

Your detailed responses will help in crafting a more targeted and effective approach to your treatment.

Treating the combination of bald patches (often related to Indralupta) and scalp eczema (Vicharchika or Darunaka) requires a dual approach: calming the immune response and clearing the “Srotas” (channels) to allow for hair regrowth. In Ayurveda, this is typically seen as a Pitta-Kapha imbalance where aggravated Pitta causes inflammation and Kapha causes the scaling and blockage of hair follicles. Ayurvedic Perspective Vicharchika (Eczema): Characterized by itching, redness, and discharge. It is considered a Kushtha roga (skin disease) primarily involving the Rakta (blood) and Twacha (skin) tissues. Indralupta (Alopecia): When the Vata and Pitta doshas together cause hair fall, and Kapha subsequently blocks the pores, preventing new hair from growing. Recommended Kerala Ayurveda Medicines & Dosage The following regimen focuses on purifying the blood and providing localized nourishment. Internal Medicines 1. Winsoria Tablet This is a proprietary formulation specifically for skin conditions like eczema and psoriasis. It helps reduce itching and scaling. Dose: 1 tablet twice daily after food. 2. Patolakaturohinyadi Kwath A potent classical formulation used for “Pitta-Rakta” purification. It is excellent for clearing inflammatory skin conditions and cooling the system. Dose: 15 ml of liquid mixed with 45 ml of lukewarm water, taken twice daily on an empty stomach (30 minutes before food). 3. Manjishtadi Kwath A well-known blood purifier that helps manage the systemic toxins (Ama) contributing to eczema. Dose: 15 ml mixed with 45 ml of lukewarm water, taken twice daily (can be taken alongside or alternated with Patolakaturohinyadi). External Applications 4. Chemparuthyadi Keratailam A traditional coconut-oil-based formula. Coconut oil is cooling (Pitta-shamaka), making it safer for eczema-prone scalps than heavy sesame oils. It helps soothe the scalp and reduce inflammation. Usage: Apply gently over the scalp 30 minutes before bathing. 5. Neelibringadi Keratailam Once the eczema inflammation (redness and oozing) has subsided, use this to target the bald patches. It is the gold standard for stimulating hair follicles. Usage: Apply to the bald patches and massage gently with fingertips to improve circulation. Important Guidelines Avoid Triggers: Limit the intake of excessively spicy, salty, and sour foods (like pickles and fermented items), as these aggravate Pitta and worsen eczema. Scalp Hygiene: Use a very mild, herbal cleanser. Do not scrub the bald patches vigorously if the skin is broken or weeping. Stress Management: Since both conditions can be psychosomatic, ensuring adequate sleep is vital for tissue repair.

What you’re describing—bald patches along with severe itching, flaking, and scaling—points toward a combination of: Alopecia areata (patchy hair loss) → autoimmune involvement Chronic scalp dermatitis → could be eczema or even early Psoriasis These conditions often overlap and are not just “local scalp problems”—they reflect internal imbalance, especially in immunity, blood quality, and metabolism. 🧠 Ayurvedic Understanding This condition correlates with: Indralupta (hair loss disorder) Kapalgata Kushtha (scalp skin disorder) Main pathology involves: Pitta + Rakta dushti (impure blood + inflammation) Vata involvement → causing hair fall and patch formation Important Reality Topical oils, shampoos, or even medicines alone won’t give fast or sustained regrowth. If you don’t correct the internal pathology,: New patches may appear Itching and scaling will persist Hair regrowth will be slow or incomplete Core Treatment Approach (Must Understand This) 1. Shodhana (Detox) – Most Important Step Without purification, results are delayed and incomplete. Virechana (Purgation Therapy) Clears Pitta + Rakta toxins Reduces inflammation at root level Helps control itching, scaling, redness Raktamokshana (Bloodletting therapy) Directly purifies vitiated blood Very effective in: Psoriasis-like scaling Chronic eczema Patchy hair loss with inflammation These therapies act at the root cause level, not just symptoms. After Shodhana (Maintenance + Regrowth Phase) 2. External Treatment (Lepa) Application of medicated pastes on scalp Helps: Reduce scaling & itching Stimulate hair follicles Improve local circulation 3. Internal Medicines (Doctor-guided) Commonly used formulations include: Mahamanjisthadi Kwath → Best for blood purification & skin healing Khadirarishta → Useful in chronic skin conditions with itching Arogyavardhini Vati → Corrects metabolism + liver function (very important in skin diseases) These support long-term healing and prevent recurrence. Diet & Lifestyle (Very Important) Avoid: Spicy, oily, fermented foods Curd, bakery, junk food Excess sugar Include: Neem, turmeric (in diet if suitable) Warm, light, easily digestible food About Homeopathy Alone Since you specifically asked: Homeopathy may help in some cases, but in your condition: Severity is high (constant itching + patches) There is clear internal pathology (Rakta + Pitta dushti) So relying only on homeopathy may: Delay results Not stop progression effectively

Namaste, Your condition suggests two things happening together—bald patches (which can be alopecia) and scalp eczema with itching and flaking. This usually indicates a combination of pitta (inflammation, irritation) and vata (hair fall, patchy loss) imbalance, along with some scalp sensitivity. First important thing to understand is that unless the scalp inflammation and itching are controlled, hair regrowth will be difficult. So the focus should be on calming the scalp first, then supporting hair growth. Avoid scratching the scalp, even if itching is severe, because it can worsen both eczema and hair loss. Also avoid very hot water head bath, harsh shampoos, and frequent chemical products. Diet plays a big role here. Try to reduce spicy, oily, fried, and processed foods. Include cooling and nourishing foods like amla, coconut water, green vegetables, soaked almonds, and a little ghee. If digestion is sometimes disturbed, that also needs correction, as it reflects on skin and scalp. Simple home care: You can apply fresh aloe vera gel on the scalp 2–3 times a week and leave it for 20–30 minutes before washing. It helps soothe itching and reduce inflammation. Neem leaves boiled in water can be used to rinse scalp once or twice a week, which helps in flaking and infection control. For oiling, use a mild medicated oil like Neelibhringadi taila, but apply gently and not on very inflamed or oozing areas. Use a mild herbal or baby shampoo, 2–3 times a week, avoid overwashing. Ayurvedic medicines with dose and timing: Arogyavardhini Vati – 1 tablet twice daily after food Mahamanjisthadi Kwath – 15 ml with equal water, twice daily Bhringraj Churna – 1 teaspoon once daily These are traditionally used to support skin healing, reduce inflammation, and promote hair regrowth gradually. You may start noticing reduction in itching and flaking within 2–3 weeks, but hair regrowth in bald patches can take 6–8 weeks or more depending on response. If bald patches are increasing rapidly or completely smooth, it is better to get evaluated once to rule out alopecia areata, as it may need more focused treatment. Consistency is very important in this condition.
