How to manage chronic pain and tightness in knees for a 75-year-old with Pitta/Vata constitution? - #52785

Client_b804a8

I am 75 years old. I have a Pitta/Vatta constitution with Pitta Dominant. I am taking oxycodone 20 mg daily for chronic pain due to arthritis and a "mystery condition" that I have had for over 17 years. the doctors were calling it Fibromyalgia for a long time but I had a diagnosis of Epstein Barr Syndrome about three years ago. The symptoms vary widely in intensity but seem to get worse in the summer season. I do not drink alcohol but have in the past. Lately, my knees have become very tight and painful and my legs feel uncomfortable if I sit still for a period of time. They are better when I am walking and doing things but still do not feel as strong and nimble as they should.

How long have you been experiencing the knee tightness and pain?:

- More than 6 months

How would you rate the severity of your knee pain?:

- Moderate — affects daily activities

When do your knee symptoms seem to worsen?:

- No specific pattern

Have you noticed any specific activities that trigger your knee discomfort?:

- Sitting for long periods

How is your overall energy level during the day?:

- Moderate — some fatigue

How would you describe your sleep quality?:

- Light or interrupted

Have you tried any treatments for your arthritis or knee pain before?:

- Prescribed medication or therapy

Namaste I can understand your situation Your body type (Prakriti) can be considered as combination of pitta and vata And your symptoms can be considered as sandhigata vata with pitta causing inflammation especially which increases in summer ▪️Ayurvedic Plan of treatment Internal medication ➖Yograj guggulu 1 tablet twice daily after food ➖Dashmoolaristha 20 ml twice daily after food ➖ GT capsule 2 at bed time This will help to reduce your pain and also reduce dependency on painkillers And please do not stop the oxycodone suddenly we can gradually taper it ▪️External treatment Daily do gentle oil massage on mahanarayana taila After this do hot water fomentation ▪️Panchakarma If feasible go for panchakarma -Janu basti - oil retenting on your knees -Matra basti -Enema therapy for preventing further degeneration ▪️Diet plan -have warm easily digestable food -have soaked almonds daily. -1 glass of milk daily -Avoid -Spicy Too oily food items -excess tea or coffee ▪️Lifestyle tips -Avoid long standing works -Do light walking ▪️for tightness and restless ness This clearly indicates vata aggrevation Do night massage And also take warm. Milk. With a pinch of nutmeg This helps is relaxation See you condition is chronic with multiple issues like arthritis also anf fibromyalgia so. Result will be gradual With proper treatment and external care you will definately feel the relief 😊 Wishing you a good health 😊 Warm Regards Dr Snehal Vidhate

How long you are taking oxycodone? Long term use can cause fatigue, sleep quality. Slight knee strengthening exercises can help you especially thighs. Avoid long sitting. Gentle walking can be done in btw a gap of 40 minutes. While sitting you can do leg stretching and ankle bending exercises. Warm oil compression with oils like kottamchukkadi tailam + Maha narayana tailam. Keep a consistent sleep time. Go to bed before 10pm. 1. Rasnerandadi kashaya tablet 2-0-2 before food. 2. Ksheerabala avarti cap 1-0-1 after 1 hour of food intake. 3. Kottamchukkadi + Mahanarayana tailam for external application. Do pichu with the same tailam Follow up after 2 weeks. Take care, Dr. Shaniba

At your age and with the long history you have shared your condition reflects a deeper imbalance rather than separate symptoms your body has been dealing with long standing vata aggravation over a pitta dominant base and this combination naturally leads to fluctuating pain stiffness weakness and sensitivity which becomes more noticeable in summer because heat further dries the tissues and disturbs vata The earlier understanding as Fibromyalgia and later association with Epstein Barr Virus Infection shows that your system has gone through chronic fatigue immune disturbance and gradual depletion of strength what we call dhatu kshaya and ojas depletion in Ayurveda over time this starts affecting joints muscles and nerves leading to tightness reduced strength and that uncomfortable feeling in the legs especially when sitting still Your knee tightness and relief with movement clearly indicates vata involvement in the joints where dryness and lack of proper nourishment is present and the lingering pitta component adds a layer of inflammation which is why the symptoms vary and sometimes flare You are taking Oxycodone which is helping in controlling pain but does not address the root so our approach has to be gentle supportive and safe without disturbing your current treatment You can start with Yograj Guggulu one tablet twice daily after food with warm water to help reduce vata in the joints and improve mobility Along with this Maharasnadi Kashayam fifteen ml mixed with equal warm water twice daily after food will help reduce stiffness and deeper inflammation For strength and nourishment Ashwagandhadi Lehyam one teaspoon at bedtime with warm milk or warm water will gradually support muscle strength energy and the nervous system Externally daily application of warm Mahanarayana Taila on knees legs and lower back followed by mild warmth will help reduce tightness and improve circulation this should be done gently without pressure Since your symptoms increase in summer it is important to keep pitta calm avoid excess heat spicy sour and fermented foods keep the body well hydrated and include simple nourishing meals like rice moong dal ghee cooked vegetables and soaked almonds in small quantity Avoid sitting continuously for long periods try to move gently every thirty to forty minutes as this will help circulation and reduce that restless uncomfortable feeling in the legs

Your presentation suggests a combination of degenerative joint changes (likely Osteoarthritis), overlapping with a chronic pain syndrome such as Fibromyalgia and post-viral fatigue features related to Epstein-Barr Virus Infection. From an Ayurvedic perspective, this is a Vata predominance over joints (Sandhigata Vata) with associated Pitta aggravation, which explains: pain and stiffness (Vata) worsening in summer and sensitivity (Pitta) fatigue and variability of symptoms Your observation that movement improves symptoms but sitting worsens them is very characteristic of Vata involvement. 1. Core principle of management At your age and condition, the goal is not aggressive correction but: reducing stiffness and pain improving joint lubrication and mobility calming Vata without aggravating Pitta 2. Daily routine corrections (most important) Avoid prolonged sitting Do not sit continuously for long periods Every 30–40 minutes, gently move or walk Even small joint movements while sitting help Gentle, regular movement Walking is excellent for you — continue it Add mild knee rotation and stretching exercises Avoid overexertion 3. External therapies (very beneficial) Oil application (Abhyanga) Use lukewarm medicated oils daily or at least 4–5 times/week: Mahanarayana taila Ksheerabala taila (better if burning sensation present) Method: Apply gently over knees and legs Follow with mild fomentation (warm towel or steam) This helps: reduce stiffness improve circulation nourish joints 4. Internal Ayurvedic support Under supervision, the following are useful: Yogaraj Guggulu (for Vata in joints) Rasnaerandadi kwath (for stiffness and mobility) 5. Diet guidelines (very important for Pitta-Vata balance) Prefer: warm, freshly cooked meals ghee in small quantity (joint lubrication) easily digestible foods Avoid: very spicy, sour, and fried foods excessive heat exposure (important in your case) dry and cold foods (increase Vata) Hydration should be steady but not excessive at once. 6. Sleep and nervous system care Your light and interrupted sleep is contributing to pain sensitivity. Helpful measures: warm milk with a pinch of nutmeg at night fixed sleep timing reduce screen/light exposure before sleep 7. Important note on current medication You are taking Oxycodone regularly. Do not stop or change this suddenly Any reduction should be supervised by your physician Ayurvedic therapies can be used alongside, but coordination is important

Hello, In a 75-year-old with a Pitta/Vata constitution, chronic knee pain and tightness typically represent Sandhigata Vata (osteoarthritis) with an underlying inflammatory (Pitta) component. At this age, Vata is naturally dominant, leading to the depletion of Sleshaka Kapha (the synovial fluid that lubricates the joints), while the Pitta element can manifest as burning sensations or redness in the joint. The goal is to provide Snehana (oleation) to lubricate the joint and Shothahara (anti-inflammatory) support to reduce stiffness without overheating the system. Vata: Responsible for the “tightness,” cracking sounds (crepitus), and the degenerative “wear and tear.” Pitta: Responsible for any warmth, sharp pain, or sensitivity in the joint. Treatment Logic: You need a “cooling-nourishing” approach. We avoid overly “hot” Vata-capsules (like those with heavy Guggulu/Pepper) and instead use herbs that soothe the nerves and rebuild the cartilage. Prescribed Medicines (Kerala Ayurveda) 1. Ksheerabala 101 Avarti (Capsule/Drops) This is the gold standard for Vata-Pitta joint issues. It is Bala (Sida cordifolia) processed in cow’s milk and sesame oil 101 times. It is highly nourishing to the nerves and bones while being cooling to the system. Dose: 1 capsule twice daily before food, or if using drops, 10 drops with warm milk twice daily. 2. Pinda Thailam (External Application) Unlike many Ayurvedic oils that are heating, Pinda Thailam is specifically formulated for Pitta-Vata conditions. It contains Manjistha and Sariva, which cool the joint while the oil base provides the necessary lubrication for the tightness. Usage: Apply lukewarm oil gently over the knees. Do not use vigorous pressure. Dose: Apply twice daily, leaving it on for at least 30 minutes before washing with lukewarm water. 3. Sudarsana Tablet (Internal) If there is a significant Pitta/inflammatory flare-up (burning or throbbing), this helps clear the “Amavisha” (metabolic toxins) and cools the internal heat that aggravates the joint. Dose: 1 tablet twice daily after food with warm water. 4. Dhanwantharam Kwath (Tablet or Liquid) This is a comprehensive formula for all Vata disorders. It acts as a uterine and nervous system tonic, but in geriatrics, it is excellent for strengthening the musculoskeletal system. Dose: 2 tablets (or 15ml liquid with 45ml lukewarm water) twice daily on an empty stomach (usually 6:00 AM and 6:00 PM).

At your age with long-standing pain and a Pitta–Vata constitution, this knee tightness and discomfort is mainly due to joint dryness, degeneration, and nerve sensitivity. It can be managed by lubrication, gentle movement, and calming the system. Apply warm Mahanarayan oil on both knees daily and do gentle massage, followed by mild heat. This helps reduce stiffness and improves movement. Avoid sitting in one position for long time. Keep changing position and do slow walking regularly, as you already notice it gives relief. Gentle stretching is helpful, but avoid strain. Your diet should be warm, soft, and slightly unctuous. Include 1–2 teaspoons of ghee daily. Take cooked foods like soups, khichdi, and well-cooked vegetables. Avoid very spicy, sour, and dry foods as they increase both pain and inflammation. Take warm water and maintain good hydration. Avoid cold exposure, especially to joints. For sleep and nerve calming, maintain a fixed sleep routine and do light oil massage on feet at night. Medicines: Yograj guggulu 2 tablets twice daily after food. Mahayograj guggulu 1 tablet twice daily after food. Ashwagandha churna 3 grams at night with warm milk. Dashmoolarishta 15 ml with equal water twice daily after food. Continue your current pain medication as advised by your doctor, do not stop it suddenly. This approach helps in reducing stiffness, improving joint lubrication, and supporting strength gradually. For proper personalised plan considering your full history and medicines, you can consult me.
