my mother is around 60 years suffering from multiple myeloma last 18 years. she has long stopped chemotherapyfrom 2017. shefirst did naturopathy and then ayurvedic treatment from then . but somehow from last few months she has developed pain in her knee joints. as checked by doctor now even knee surgery is also not possible. also the pain around knee is severe. as medicine we give various ayurvedic supplements like ashwagandha satavari, shilajit, pushpadhanwa ras and vitamins.butnow pain in her joints and overall body has increased. can you suggest what can we do

I understand your concern caring for her for so many years and now this increase in joint and body pain can be very difficult especially with a long history of multiple myeloma and stopping chemotherapy since many years In this condition the pain is usually not just simple knee problem it can be due to bone weakness long term disease effect and increase of vata in the body so treatment has to be gentle and supportive rather than aggressive First give her maximum rest avoid strain on knees use support while walking and keep the joints warm you can apply warm mahanarayana thailam or ksheerabala thailam over the knees and painful areas twice daily followed by mild heat this helps in reducing stiffness and gives some comfort Internally it is better to keep medicines simple and focused rather than many supplements together you can continue ashwagandha in small dose like half teaspoon at night with warm milk if she is tolerating Along with this start yograj guggulu one tablet twice daily after food with warm water for pain and stiffness and dashamoola kwath fifteen ml twice daily after food with equal quantity of warm water for reducing pain and inflammation If available gandha taila capsule one twice daily after food can be added for bone support Avoid giving too many ras medicines together for long duration as it may not suit her system now especially if digestion is weak Diet should be light warm and easy to digest give soft foods like rice with moong dal vegetable soups little ghee avoid cold dry and raw foods as they increase pain keep giving warm water in small quantities If pain is very severe you can consider gentle therapies like mild oil application regularly but avoid strong panchakarma Also it is important to check her calcium vitamin d and recent reports because in long standing multiple myeloma bone pain can increase due to weakness of bones so this should be monitored The aim now is to reduce pain improve comfort and maintain her strength slowly if you can share more details or reports I can guide you further based on her condition

Given her history, we must balance Vata without being too heavy on the digestive system. Medicine :​ 1. ​Pinda Thailam (External): Apply gently over the knee joints twice daily. This is specifically indicated for pain associated with inflammatory conditions and bone weakness. Do not massage vigorously; use light circular strokes. 2. ​Ksheerabala (101) Capsules: Take 1 capsule twice daily before food. hello, ii would like to see her full reports including RA reports if you have done one. aside from knee joints what other joints does she have pain? is there any swelling, pain worsening in morning, stiffness? how is her appetite and what all medicies are she taking now? and whats her current diet like? also for a slight pain releif topical use mix 1 big spoon f the powder with 1 glass warm water and cook it over a tove till a paste consistancy. apply it iover the knees and other joints. add in a 1/2 a teaspoon of salt if there is swelling. wrap the knees with a kotton cloth after applying for atleast 6 hours. better to do that in the night and keep it wrapped over night, the results are better if you can cover the joint with arka patra or erand patra before wrapping. the other time apply the second lepa over the joints in a thick layer. internal medicines can only be advised after her reports are reviewed and hiostory is taken properly. continue these local treatments for 14 days and there will be good relief.

First priority should be re-evaluation, especially: • CBC, calcium, vitamin D • Kidney function • Bone scan / X-ray knee if not done recently • Myeloma follow-up (to rule out activity) For symptom relief and support: Reduce load on knees. Avoid walking long distances or standing for long time. Use a walker or support if needed. Warm fomentation can help reduce stiffness. Gentle passive movements only, no forceful exercises.
