Hello dear, I am very sorry to hear about the intense pain you are experiencing. Dealing with an anal fistula is incredibly difficult, especially when the discomfort is acute. While surgery is the standard definitive treatment in modern medicine (to prevent recurrent infections or abscesses), Ayurveda offers a specialized perspective and alternative procedures that are often sought to avoid conventional surgery. In Ayurveda, a fistula-in-ano is known as Bhagandara. It is primarily viewed as an imbalance of the Pitta (inflammation/heat) and Kapha (blockage/pus formation) doshas. The goal is to drain the tract, reduce inflammation, and heal the wound from the inside out. Prescription : For a painful, active fistula, the following internal medicines from Ayurveda are typically used to manage infection and pain: 1. Guggulutikhthaka Gritham Dose: 10–15 ml mixed with a half glass of warm milk or water. Twice daily on an empty stomach. 2. Triphala Guggulu Dose: 2 tablets (must be crushed/swallowed, standard dose). Twice daily after food. 3. Varanadi Kashayam Dose: 15 ml diluted with 45 ml of lukewarm water. Twice daily, 30 minutes before food. 4. Jatyadi Tailam (External) Apply locally to the area or soak a small sterile gauze piece and place it at the opening. This aids in soothing pain and healing the tract. Immediate Pain Management Sitz bath is non-negotiable. Sit in a tub of warm water with a pinch of salt or Triphala Churna for 15 minutes, 3 times a day. It relaxes the sphincter and cleanses the area. Avoid spicy food, caffeine, and alcohol immediately. Constipation will make the pain unbearable; ensure soft stools.

Hello, In Ayurveda, fistula-in-ano is often correlated with Bhagandara. It is understood as a tract/tunnel near the anal region that may be associated with -Inflammation (often linked with aggravated pitta) -swelling/congestion (can involve kapha) -pain, spasm, dryness, strain (can involve vata) Treatment goals -reduce acute pain and inflammation -support drainage if infection is present (but not forcefully at home) -keep stool soft and easy to pass (very important) -support wound hygiene -reduce recurrence risk -decide early if surgical care is needed Important truth about without surgery -simple fistulas rarely heal completely with medicines alone -Ayurveda may help support inflammation, bowel habits, local care, and sometimes chronic management -In Ayurveda itself, ksharasutra(medicated thread therapy) is a classic recognised treatment for fistula, many surgeon use it instead of conventional surgery in selected cases. It is still a procedure, but different from standard surgery Internally start with 1) For bowel softening/reduce straining -Triphala tablets= 2 tabs with lukewarm water at bedtime =support bowel regularity, reduce constipation/straining (important because straining worsens pain) 2) For inflammation support -Kaishor guggulu= 2 tabs twice daily after meals =support inflammatory balance, support tissue healing External supportive care 1) Warm sitz bath very important -sit in comfortably warm water 10-15 minutes, 3-4 times daily -especially after bowel movement can help -ease pain -relax spasm -improve comfort -support cleanliness 4) Local wound hygiene -wash gently with lukewarm water -pat dry, do not rub -avoid harsh antiseptics Because you have severe pain + swelling + redness, be cautious about -applying random oils/pastes directly over a possibly infected area -strong laxatives causing diarrhea (can irritate the area) -self starting an medication -delaying evaluation if ppain is worsening Diet very important focus on soft, easy stools -warm water -cooked vegetables -moong dal -rice gruel -papaya -pomegranate -soaked black raisins -fiber from food Reduce or avoid -very spicy food -deep fried foods -alcohol -heavy red meat -very dry foods -constipating foods -excess chilli (may worsen burning) Lifestyle -gentle walking only if comfortable -rest -hydration Avoid -straining during stool -long sitting -heavy lifting -cycling -prolonged walking if it worsens pain Yoga/pranayam during acute pain -avoid strong yoga postures -avoid pressure on pelvic floor -do anulom vilom= gentle breathing for relaxation -diaphragmatic breathing= can reduce guarding/spasm Avoid currently -strong core work -deep squats -aggressive stretches -forceful pranayam like kapalbhati during acute pain 3 days of severe pain + swelling/redness can be an abscess emergency. Do follow Hope this might be helpful Thank you
