My father has been suffering from sciatica for the past year and is unable to perform physical activities properly. He also has diabetes. Please suggest an appropriate solution. Doctor’s Assessment: Lumbar muscle spasm → Tight lower back muscles → Loss of normal lumbar curve L5 over S1 anterolisthesis (Grade I–II) → Mild forward slipping of L5 vertebra → Can cause back pain and leg pain Degenerative (wear & tear) changes → Small bone spurs (osteophytes) → Endplate changes at multiple levels Disc bulges → Present at L3–L4 → Present at L5–S1 → Mild indentation on spinal sac Nerve compression → At L3–L4 level → At L5–S1 level → Compression of exiting nerve roots → Can cause sciatica symptoms Disc tear at L5–S1 (annular tear) → Tear in outer disc layer → Possible disc extrusion → Major contributor to pain

this condition is mainly seen as Vata aggravation in the lumbar region (Gridhrasi – Sciatica) along with Avarana (nerve obstruction) due to disc degeneration. With diabetes, we must choose mild, safe, and anti-inflammatory Vata-pacifying care. Ayurvedic Approach 1. Pain & Nerve Relief Daily warm oil massage (Abhyanga) with Mahanarayana oil or Dhanwantharam oil on lower back and legs Followed by hot fomentation ( warm towel heat) → Reduces stiffness, improves nerve flow 2. Internal Support (Vata + Nerve healing) Ashwagandha + Guggulu formulations Dashmool Kwath for inflammation and muscle relaxation Yograj Guggulu is often used in sciatica (only under supervision, especially with diabetes) 3. Physical Activity (Very important) Gentle walking 15–20 minutes twice daily Simple physiotherapy stretches (hamstring stretch, pelvic tilt) Avoid long sitting, bending, and lifting weights 4. Diabetes-friendly care Keep diet light: barley, green vegetables, bitter gourd, turmeric Avoid excess sugar, fried foods, and cold foods (they worsen Vata + nerves) 5. Start Trayodashanga Guggulu - 1-0-1 after food. Start with pure cow ghee nasya, 2 drops in each nostril at night before sleeping start applying pinda tailam oil massage on legs, in upward direction, for back pain, apply sahacharadi tailam on back, everyday night and sleep… control diabetes with your current medication… in ayurveda you can start taking Amrutamehahari churna-1tsp-0-1sp before food no rice or curd in night sleep keeping pillow under the knees… no extra exertion
