Hello Lillian. I’m sorry to hear your husband is struggling with fatigue; Polycythemia Vera (PV) is taxing enough on its own, and Jakafi (Ruxolitinib), while effective, can definitely add a layer of exhaustion. ​In Ayurveda, Polycythemia is often viewed as a Rakta-Pitta imbalance an excess of “heat” and “blood energy” that leads to congestion and toxicity. Jakafi’s side effects are seen as an exhaustion of Ojas (vitality) and an aggravation of Vata (nervous system energy) due to the drug’s drying nature. ​The goal is to cool the blood (Pitta-Shamana), support the liver, and gently build energy without increasing the “thickness” of the blood. Prescription : 1. ​Drakshadi Kwath: This is a premier formula for exhaustion and heat-related blood disorders. It improves strength and relieves fatigue. ​Dose: 15 ml mixed with 45 ml lukewarm water, twice daily before food. 2. ​Amrutarishtam: A fermented tonic that detoxifies the blood and supports the immune system (Ojas) while fighting the “hidden heat” that causes tiredness. ​Dose: 20 ml with an equal amount of water, twice daily after food. 3. ​Chavanaprash: Specifically the Kerala Ayurveda formulation, which is rich in Vitamin C (Amla) to boost energy levels naturally. ​Dose: 1 teaspoon every morning on an empty stomach, followed by a sip of warm milk or water. ​ ​Ensure he drinks water processed with Usheera (Vetiver) or Chandana (Sandalwood) to cool the system internally. ​ Fresh pomegranate is a “Hridya” (heart-tonic) in Ayurveda that builds healthy blood cells without aggravating Pitta. ​ Spicy, sour, and fermented foods (like vinegar or yogurt), as these increase blood heat and can worsen Jakafi-related fatigue.

Polycythemia (often treated with Jakafi) can cause fatigue due to both the disease process and drug effect. Ayurvedic management should be supportive only and must not replace ongoing hematology care Ayurvedic support for fatigue 1. Aushadhi (medicines) Ashwagandha Churna – 3 g, twice daily with warm milk Guduchi Satva – 500 mg, twice daily with water Amalaki Churna – 3 g, once daily (morning) Brahma Rasayana – 1 teaspoon, morning on empty stomach 2. If weakness is significant: Chyawanprash – 1 teaspoon daily (avoid if sugar uncontrolled) 3. Diet Light but nourishing food: moong dal, rice, ghee, pomegranate, cooked vegetables Adequate hydration Avoid alcohol, smoking, very heavy/oily foods 4. Lifestyle Gentle yoga, pranayama (especially Anulom Vilom) Proper sleep, avoid overexertion

Fatigue with Jakafi (for polycythemia) is quite common, mainly due to bone marrow suppression and metabolic load on the body. It should be managed as supportive care, not by stopping or replacing the medicine. First priority: Do not stop Jakafi without hematologist advice. What can help: Keep routine balanced Short, frequent meals instead of heavy meals Adequate hydration Light daily movement (even 10–15 min walking) to improve circulation Diet support: Include iron-supportive and nourishing foods (beetroot, pomegranate, dates in small quantity if allowed) Protein intake should be adequate (dal, lentils, light proteins) At home: Fresh pomegranate juice (small quantity) Soaked raisins (5–7) daily Warm water through the day Ashwagandha tablet – 1 tablet at bedtime Drakshasava – 10 ml with equal water after meals Chyawanprash – 1 teaspoon in morning These help improve energy, strength, and recovery, without interfering with ongoing treatment. Important: Since he also has shortness of breath, regular follow-up and blood monitoring (Hb, counts) is essential. If fatigue worsens significantly, dose adjustment by doctor may be needed.
