Based on your symptoms, including a feeling that the knee may “dislocate,” persistent swelling and stiffness, and difficulty walking for more than 6 months, there may be an underlying structural issue such as a meniscal injury, ligament weakness, cartilage damage, or degenerative changes in the knee joint. ---------Since your orthopedic doctor has advised an MRI scan, it is important to proceed with the investigation to identify the exact cause. Ayurveda may help reduce pain, swelling, stiffness, and improve joint function, but if there is a significant structural problem, further orthopedic management may be required. ------Recommendations —Investigations -Proceed with the MRI scan as advised. -Review the MRI findings with an orthopedic specialist. ---------Activity Modification -Avoid running, jumping, squatting, and sudden twisting movements. -Limit activities that worsen the instability. -Consider using a knee brace or support if needed. ---------Physiotherapy & Exercise -Gentle strengthening exercises under guidance may help improve knee stability. -Avoid exercises that increase pain or swelling. ---------Ayurvedic Management 1) Yogaraja Guggulu – 2 tablets twice daily after food. 2) Maharasnadi Kashayam – 15 ml with equal quantity of warm water twice daily before food. 3) Mahanarayana Taila – Local application over the affected knee followed by gentle warm fomentation once or twice daily. 4) Shallaki mr 1 tab twice daily after meal ---------Diet & Lifestyle Prefer warm, freshly prepared foods. Avoid cold foods and beverages. Maintain adequate hydration and proper sleep.

••I completely understand the deep sense of instability and discomfort you are experiencing. That sensation of a “vacuum” or emptiness in the joint, combined with the feeling that the knee might give way or dislocate, tells us a very specific story from both modern and Ayurvedic perspectives. Let’s break down exactly what is happening in your knee and how Ayurveda can help manage, strengthen, and potentially resolve this condition. •The Modern vs. Ayurvedic Perspective When an allopathic doctor mentions an MRI and keyhole surgery (arthroscopy) for a knee that feels like it’s going to dislocate, they are usually looking for a structural issue. ••Modern View: The “vacuum” feeling and instability typically point to a ligament tear (like the ACL or meniscus) or severe cartilage wear. Without these stabilizers, the femur and tibia don’t sit snugly together, causing that empty, unstable sensation. ••Ayurvedic View: In Ayurveda, this is a classic presentation of Sandhigata Vata (Vata localized in the joints) with an element of Majja-Kshaya (depletion of the bone marrow/intra-articular fluids) or ligament laxity (Snayu Shithilata). •Vata is dry, light, and empty. When Vata increases in the joint, it dries up the Shleshaka Kapha (the synovial fluid that acts as a cushion/lubricant). •The loss of this fluid creates a literal and metaphorical “emptiness” (Akasha/space element increase), which matches your description of feeling a vacuum. ••Will Ayurveda Help You? Yes, Ayurveda can be highly effective here, but with a realistic, clinical caveat: It depends on the grade of the structural damage. ••For Partial Tears, Degeneration, or Laxity: Ayurvedic therapies excel. We focus on deeply nourishing the joint, rebuilding the synovial fluid, and strengthening the surrounding Snayu (ligaments) and Kandara (tendons) to stabilize the knee naturally. For a Complete Ligament Tear: If the MRI reveals a complete, high-grade tear where the tissue is entirely severed, mechanical stability might ultimately require that keyhole surgery. However, even if surgery becomes necessary, Ayurvedic pre- and post-operative care can drastically speed up healing and prevent future joint degeneration. ••Line of Treatment: Rebuilding the Cushion To counter the dry, unstable Vata in your knee, the treatment protocol must be intensely nourishing (Brimhana) and lubricating (Snehana). 1. External Therapies (Bahya Chikitsa) ••Janu Basti: This is the gold-standard Ayurvedic therapy for your exact complaint. A ring of black gram dough is built around your knee, and warm, medicated Ayurvedic oils are pooled inside it for 30–40 minutes. This deeply penetrates the joint capsule, subdues Vata, and helps restore the “cushion” to eliminate that vacuum feeling. ••Janu Upanaha: Applying a warm, medicated herbal poultice over the joint overnight to strengthen the ligaments and reduce structural instability. ••Abhyanga: Gentle downward oil massage using Mahanarayana Taila, Ksheerabala Taila, or Murivenna (excellent for ligament injuries). 2. Internal Medications (Abhyantara Chikitsa) We use herbs that are Vata-hara (Vata-destroying) and Asthi-Snayu Poshaka (nourishing to bones and ligaments): ••Lakshadi Guggulu & Kishore Guggulu: To promote tissue repair and strengthen connective tissue. ••Gandha Thailam: A specific classical oil taken orally (capsule/drops) renowned for its ability to heal bone, ligament, and joint injuries. ••Ksheerabala 101 Avartana: Medicated milk-infused drops to calm severe Vata-induced degeneration. ••Immediate Do’s and Don’ts: •Do Wear a Knee Support: Use a hinged knee cap or brace while walking to mechanically support the joint and prevent accidental twisting. ••Do Rest: Restricting heavy walking was a smart move. Avoid squatting, kneeling, or cross-legged sitting for now. ••Don’t Massively Massage a Loose Joint: Do not apply heavy pressure or hard massage to the knee, as it can further destabilize a torn ligament. Only gentle, warm oil application is recommended.

Thank you for sharing your symptoms. Based on your description—feeling that the knee may “dislocate,” a sensation of vacuum or instability, constant swelling and stiffness, worsening with walking, reduced mobility, and a history of minor injury—this appears to be a condition that requires proper evaluation. The MRI recommended by your orthopedic doctor is important because it can identify ligament, meniscus, cartilage, or other structural damage that may be contributing to your symptoms. Ayurvedic Understanding In Ayurveda, such symptoms are commonly associated with Vata aggravation in the knee joint (Janu Sandhi), often accompanied by joint degeneration, weakness of supporting tissues, and obstruction caused by inflammation (Ama). This can lead to instability, pain, stiffness, swelling, and difficulty walking. Can Ayurveda Help? Yes, Ayurveda may help improve symptoms, support joint strength, reduce inflammation, and enhance mobility, especially when combined with an accurate diagnosis. However, if there is a significant ligament tear, meniscal injury, or mechanical instability, Ayurvedic treatment should complement—not replace—appropriate orthopedic evaluation. Ayurvedic Recommendations 1. External Therapies Gentle warm Mahanarayana Taila or Ksheerabala Taila massage around the knee. Follow with mild warm fomentation (heat therapy) if swelling is not acute. Traditional therapies such as Janu Basti, Patra Pinda Sweda, and Abhyanga… 2. Diet and Lifestyle Prefer warm, freshly cooked foods. Include nourishing foods such as: Cooked vegetables Mung dal Cow’s ghee in moderation Sesame seeds Almonds (if suitable) Avoid: Cold foods and drinks Excessively dry or processed foods Frequent fasting Excessive walking, running, or stair climbing 3. Gentle Exercise Avoid high-impact activities. Practice gentle knee-strengthening exercises and yoga only after professional assessment. Do not force movements that create instability or a slipping sensation. 4. Common Ayurvedic Herbs Depending on your constitution and diagnosis Shallaki (Boswellia serrata) Guggulu preparations Ashwagandha Rasna Nirgundi These herbs are traditionally used to support joint health, reduce inflammation, and improve mobility.
