understand your concern. After 20 years of type 2 diabetes, neuropathy (burning, pain, tingling, numbness) usually develops because long-term elevated blood sugar gradually damages the nerves and their blood supply. From both modern and Ayurvedic perspectives, simply adding medicines without improving metabolism, circulation, and diet often gives only partial relief. In your case, the most likely contributing factors are: Long-standing diabetes causing nerve degeneration and impaired microcirculation. Fluctuating blood sugar levels (“moderately controlled” diabetes can still worsen neuropathy over time). Possible nutritional deficiencies, especially vitamin B12 (common in patients taking diabetes medicines for years). Chronic metabolic inflammation and oxidative stress, which Ayurveda correlates with Madhumeha affecting the nerve tissues (Majja Dhatu) and Vata aggravation. From an Ayurvedic clinical viewpoint, treatment should focus on controlling blood sugar, nourishing damaged nerves, and improving circulation simultaneously. I commonly advise the following combination 1. Nishamalaki Churna 3–5 grams twice daily before meals with lukewarm water Helps improve glucose metabolism and reduce diabetic complications. 2. Chandraprabha Vati 2 tablets twice daily after meals Supports urinary and metabolic function in long-standing diabetes. 3. Ashwagandha Capsules/Churna 500 mg capsule twice daily or 3 grams powder at bedtime Helps nerve nourishment, strength, sleep quality, and recovery. 4. Mahayograj Guggulu 1–2 tablets twice daily after meals Often useful when neuropathy is associated with pain, stiffness, or burning sensations. For external therapy: Daily gentle massage of feet and legs with warm Mahanarayana Taila or Ksheerabala Taila for 10–15 minutes before bath. This may help circulation, reduce Vata aggravation, and improve comfort. Panchakarma can be very beneficial when neuropathy has been present for years: Abhyanga (medicated oil massage) Basti therapy (medicated enemas), which is considered one of the most effective Ayurvedic treatments for chronic Vata-related nerve disorders Patra Pinda Sweda or other localized therapies if pain is significant Diet is equally important: Eat whole grains in moderation, vegetables, bitter gourd, fenugreek, Indian gooseberry (amla), lentils, and adequate protein. Avoid sugar, sweet beverages, refined flour, excessive rice, processed foods, and frequent snacking. Walk 30–45 minutes daily after meals when possible. I would also strongly recommend checking: HbA1c Vitamin B12 Kidney function tests Foot sensation examination Please keep realistic expectations: neuropathy present for more than 2 years usually improves gradually rather than disappearing completely. With consistent blood sugar control, appropriate Ayurvedic support, diet correction, and Panchakarma, many patients notice reduction in burning and pain over 2–4 months, while nerve recovery may take 6–12 months or longer.

Fronisjm an Ayurvedic perspective, long-standing type 2 diabetes (Madhumeha) with neuropathy requires a combination of blood sugar control, nerve nourishment, diet, exercise, and lifestyle correction. No Ayurvedic or naturopathic treatment has been proven to reverse diabetic neuropathy completely, but some patients experience improvement in symptoms. Take • Nishamalaki Churna 3 g twice daily before meals with warm water. • Gudmar (Meshashringi) Churna 2–3 g twice daily before meals. • Ashwagandha Churna 3 g at bedtime with warm milk (if blood sugar is well controlled). • Balarishta 15–20 ml with equal water twice daily after meals for neuropathic weakness. For neuropathy: • Ksheerabala Taila or Mahanarayana Taila massage to feet and legs daily. • Gentle walking, stretching, and foot care. Diet: • Avoid sugar, sweetened beverages, refined flour, and processed foods. • Emphasize vegetables, legumes, whole grains, nuts, and adequate protein. • Maintain healthy body weight and regular physical activity.

••In Ayurveda, a long-standing history of diabetes involves Dhatu Kshaya (the depletion and degradation of deeper bodily tissues) alongside severe Vata-Pitta imbalance. The burning sensations (Daha), numbness (Suptata), and tingling in your extremities occur because high systemic metabolic impurities (Ama) have blocked the micro-channels (Srotas), depriving your nerves of nourishment. ••When Western pharmaceuticals fail to provide relief for neuropathy, it is often because they mask nerve pain rather than restoring vascular circulation and tissue vitality. Below is the multi-layered, evidence-based Ayurvedic treatment protocol engineered to reverse this pathology. 1. Evidence-Based Abhyantara Chikitsa (Internal Herbal Therapies) Clinical research validates that specific classical herbo-mineral formulations can significantly lower HbA1c while simultaneously arresting nerve degradation. For Glycemic Control & Metabolic Correction: ••Chandraprabha Vati: A premier formulation that cleanses the Mootravaha Srotas (urinary tract and kidneys) and regulates insulin sensitivity. ••Nisha Amalaki (Equal parts Turmeric and Amla): Extensively researched for its ability to prevent diabetic complications. Curcuma longa reduces systemic inflammation, while Emblica officinalis acts as a potent antioxidant protecting the vascular endothelium (blood vessel linings). For Nerve Regeneration & Neuroprotection (Neuropathy): ••Ekangveer Ras / Mahavat Vidhwansan Ras: Highly potent formulations used specifically to treat severe neurological pain and restore motor-sensory pathways. ••Ksheerabala 101 Avarti: A specialized preparation of Sida cordifolia processed 101 times in milk and sesame oil. Taking 5–10 drops internally with warm milk helps rebuild the lipid myelin sheath of degraded nerves. 2. Bahirparimarjana Chikitsa (External & Clinical Procedures) To relieve peripheral neuropathy immediately, localized circulation must be forced back into the ischemic (blood-deprived) nerve endings 1 Udwarthana (Dry Herbal Scrub) Phase 1: Clear Obstructions Before applying any heavy oils, the body must be scrubbed with dry, warm herbal powders like Triphala Churna or Kottamchukkadi Churna. This breaks down the stagnant Kapha and Meda (fat tissue) blocking the peripheral blood vessels. 2 Dashamoola Ksheera Dhara (Medicated Milk Pouring) Phase 2: Soothe Burning (Daha) For severe burning sensations, standard hot oil massage can aggravate Pitta. Instead, a continuous, warm stream of milk decoction infused with Dashamoola (ten roots) is poured over the legs and feet. This instantly pacifies Vata-Pitta and relieves neuropathic pain. 3 Matra Basti (Medicated Enema) Phase 3: Systemic Vata Root Eradication Because the root seat of Vata is the colon, systemic neuropathy cannot be cured by local application alone. A series of low-volume enemas using Sahacharadi Taila or Ksheerabala Taila directly pacifies the systemic Vata responsible for nerve degeneration. ••3. Dinacharya & Pathyapathya (Dietary Rules & Lifestyle) No formulation will work if the metabolic fire (Agni) is consistently choked by incompatible foods. Ahara (What to Incorporate vs. What to Avoid) The Power of Barley (Yava): Swap wheat and rice entirely for Barley (Yava). Classical texts identify Barley as the premier grain for Prameha because it clears excess fluid and fat tissue without spiking blood glucose. Bitter & Astringent Vegetables: Increase your intake of Bitter Gourd (Karavellaka), Fenugreek leaves (Methi), and Pointed Gourd (Patola). Strictly Avoid (Apathya): Yogurt (Dadhi), freshly harvested grains (Navanna), jaggery, and sleeping during the day (Diva-Svapna), as these instantly trigger Kapha-Meda accumulation and worsen nerve blockages.
