If all medical tests are normal, erections are normal, and premature ejaculation is mainly related to stress, performance anxiety, and previous negative experiences, then treatment should focus more on calming the mind and improving ejaculatory control rather than using strong aphrodisiac herbs. Take Brahmi Ghrita 1 teaspoon with warm milk at bedtime Helps reduce anxiety, mental overactivity, and stress. Brahmi Vati (plain) 1 tablet twice daily after food. Ashwagandha Churna 3 g at bedtime with warm milk for 2–3 months. If it causes heat or discomfort, reduce to 2 g or stop. Jatamansi Churna 2 g at bedtime with warm water. Useful when anxiety is the dominant factor. Lifestyle: Practice slow deep breathing (10 minutes daily). Avoid excessive focus on performance during intercourse. Regular walking, yoga, and adequate sleep. Reduce caffeine, alcohol, and late-night screen exposure. For ejaculatory control: Pelvic floor (Kegel) exercises: 10–15 contractions, 3 times daily. Stop-start technique during intercourse can gradually improve control.

The fact that all medical tests are normal, erections are satisfactory, and Dapoxetine gives clear improvement indicates that the ejaculation control mechanism is intact but is being overridden by anxiety, anticipation, and fear of failure during intimacy. After experiencing PE repeatedly, many men develop a cycle where they start monitoring their performance excessively. This increases sympathetic nervous system activity (“fight or flight” response), leading to rapid ejaculation despite normal sexual desire and erection quality. Since aphrodisiac herbs have caused heat and discomfort without benefit, I would avoid strong Vajikarana medicines initially and focus on calming the nervous system and improving ejaculatory control. From an Ayurvedic perspective, this resembles Vata aggravation with Manasika Chinta (anxiety), Bhaya (fear), and Rajasic mental overactivity, rather than Shukra Dhatu deficiency. Therefore, treatment should be directed toward the mind–nervous system axis. Recommended Ayurvedic approach: 1.) Brahmi Vati (Suvarna Yukta if available) – 1 tablet twice daily after meals for 8–12 weeks. 2) Ashwagandha Capsule (standardized extract) – 500 mg at bedtime with warm milk or water for 2–3 months. If Ashwagandha causes heat or discomfort, discontinue and use Jatamansi Churna 2–3 g at bedtime instead. 3)Saraswatarishta – 15 ml with equal water after dinner for 2 months. These medicines are aimed at reducing performance anxiety, improving nervous system resilience, and helping restore confidence rather than acting as aphrodisiacs. A very useful home practice is pelvic floor (Kegel) exercises: 10 contractions held for 5 seconds each, 3 times daily. This has helped many men improve ejaculatory control over time. During intimacy, focus on slow breathing and avoid mentally “checking” whether ejaculation is approaching. Diet should be simple and calming: milk, soaked almonds, dates (in moderation), ghee, moong dal, seasonal fruits, and adequate hydration. Reduce excessive tea/coffee, alcohol, smoking, energy drinks, and late-night screen exposure, as these can worsen anxiety and nervous system sensitivity. If available, Shirodhara or Takradhara can be beneficial when anxiety is severe and persistent. Panchakarma is generally more useful for the anxiety component than for PE itself. Since symptoms have persisted for more than a year and Dapoxetine helps, I would also strongly consider working with a sexual health counselor or therapist experienced in performance anxiety.
