CoQ10 is one of the few fertility supplements with a mechanism that makes real sense, and the reason is simple: egg and sperm quality are energy problems, and CoQ10 is an energy molecule.
The practical answer most people want: 200 to 600 mg daily, taken with food, for at least three months before trying to conceive or beginning IVF. Three months is not a marketing number — it is how long an egg takes to mature from a resting follicle, and how long a sperm cell takes to be made.
Below is what the evidence supports, what it does not, and how Ayurveda would frame the same problem through ojas and shukra dhatu.
What CoQ10 Is and Why Fertility Depends on It
Coenzyme Q10 sits inside mitochondria, the structures that produce cellular energy, shuttling electrons through the chain that generates ATP. It is also a fat-soluble antioxidant protecting cell membranes and DNA from oxidative damage.
Two facts make it relevant to fertility.
The egg is the most mitochondria-rich cell in the body. Chromosome separation during maturation and the first days of embryo development are enormously energy-expensive. When that energy supply falters, chromosomes segregate incorrectly — the mechanism behind the rise in aneuploidy with maternal age.
The body's own CoQ10 production declines with age, and mitochondrial function in oocytes declines alongside it. That is the specific gap supplementation aims at.
For sperm, the argument is different but equally coherent: sperm are motile cells that need continuous energy, and seminal CoQ10 concentrations correlate with motility. Sperm are also highly vulnerable to oxidative DNA damage, which antioxidants counter.
Does CoQ10 Help You Get Pregnant?
Honestly stated: the evidence is encouraging on intermediate markers and thinner on the outcome that matters.

Reviews note improved clinical pregnancy rates in several trials, while live-birth data remains limited — the trials are small and heterogeneous.
For men, the picture is stronger on semen parameters. Multiple trials have found improvements in sperm concentration, motility and morphology over three to six months, with reduced oxidative stress markers. Again, evidence that this translates into more live births is weaker than the parameter data.
Most likely to help: women over 35, diminished ovarian reserve, poor response in previous IVF cycles, poor embryo quality, men with low motility or high DNA fragmentation, and anyone on statins, which deplete CoQ10 directly.
Least likely to help: a blocked fallopian tube, a structural uterine problem, untreated thyroid disease, severe endometriosis, or azoospermia. CoQ10 improves cellular energy. It does not fix plumbing, and no supplement should delay a proper fertility assessment.

Ubiquinol or Ubiquinone?
Ubiquinone is the oxidised form — cheaper, more stable, and the form used in most published trials. Your body converts it to ubiquinol.
Ubiquinol is the reduced, active form: better absorbed, more expensive, and generally preferred over the age of 40 where conversion may be less efficient.
Either is reasonable. If cost is not the constraint and you are over 40, ubiquinol is the better bet. If it is, a well-absorbed ubiquinone softgel taken with fat beats an expensive ubiquinol taken badly.
How to Take It
- With a meal containing fat. CoQ10 is fat-soluble, and absorption without fat is poor. This is the most common mistake.
- Split the dose across two or three portions rather than one large one.
- Take it in the morning and at midday. Some people find CoQ10 mildly stimulating and sleep worse if they take it late.
- Softgels in oil absorb better than dry powder capsules.
How Quickly Does CoQ10 Work for Fertility?
The biology sets the timetable, and it is not negotiable.

The Ayurvedic View: Fertility as the Last Tissue to Be Fed
Ayurveda would have no argument with CoQ10 conceptually. It would simply place it in a wider frame.
In Ayurvedic physiology, nutrition passes through seven tissues in sequence — plasma, blood, muscle, fat, bone, marrow and nerve, and finally shukra dhatu, the reproductive tissue. Fertility gets whatever is left after every other tissue has taken its share.

Seen this way, CoQ10 is a rasayana — a rejuvenative that restores tissue quality. Ayurveda simply insists that a rasayana works properly only when what feeds it is working.

Is CoQ10 Risky? Side Effects and Interactions
CoQ10 is among the better-tolerated supplements. Side effects are mild and uncommon: nausea, stomach upset, loss of appetite, headache, rash, and insomnia if taken late. Splitting the dose and taking it with food resolves most of them.
Interactions that genuinely matter:
- Warfarin. CoQ10 is structurally similar to vitamin K and may reduce warfarin's effect. Do not combine without INR monitoring.
- Blood pressure medication. CoQ10 can lower blood pressure modestly, so the combination can overshoot.
- Diabetes medication. It may affect blood glucose; monitor.
- Chemotherapy. Antioxidants during cancer treatment must be discussed with the oncologist first.
- Statins. Not a risk — statins lower your CoQ10, which is an argument for supplementing, not against.
