Hormonal acne during the luteal phase is commonly related to Pitta-Rakta imbalance with mild hormonal fluctuation and toxin accumulation (Ama). Since Manjishtadi Kwath is already helping your pigmentation, your blood purification is improving, but acne may need additional support for a few more cycles. You may continue: Manjishtadi Kwath – 15–20 ml with equal water after meals twice daily Avipattikar Churna – 1 tsp at bedtime or 3–4 times weekly for gut cleansing and reducing Pitta For luteal phase acne, you can adding: Kaishore Guggulu – 1–2 tablets twice daily after food Helpful for inflamed, hormonal, painful, or recurring acne If breakouts are oily and frequent: Neem Capsules or Neem-based formulations may help reduce skin inflammation and bacterial tendency External care: Apply aloe vera gel or a mild neem-based face pack 2–3 times weekly Avoid harsh scrubs and over-cleansing Diet & lifestyle: Reduce sugar, fried food, excess dairy, chocolate, and late-night meals during the premenstrual phase Drink warm water and maintain regular bowel movement Sleep on time and manage stress levels Yes, you can achieve clearer skin. Hormonal acne usually improves gradually over 2–4 cycles with consistent Ayurvedic treatment, gut correction, and hormonal balance support. The improvement in pigmentation already indicates that your body is responding positively.

••It is wonderful to hear that you are already seeing a reduction in pigmentation with Mahamanjisthadi Kwath. Manjistha is an exceptional Rakta Prasadaka (blood purifier and skin rejuvenator), so its success with pigmentation makes perfect sense. Analysing Your Current Regimen ••Mahamanjisthadi Kwath: Doing a great job on Varnya (complexion) and reducing pigmentation. It cleanses the blood (Rakta Shodhana), but it might not be strong enough on its own to tackle acute, deep-seated inflammatory acne driven by cyclic hormonal shifts. ••Avipattikar Churna / Avipathi Choornam (3x a week): This is an excellent choice. It ensures regular Mridu Virechana (mild purgation), which is the prime treatment to clear accumulated Pitta from the body. Keep doing this, as it prevents Pitta from overflowing into the sweat and sebaceous glands. Should You Add Kaishore Guggulu? ••Yes, adding Kaishore Guggulu is a highly logical and effective next step. While Manjistha purifies the blood, Kaishore Guggulu acts deeper. It is specifically formulated for conditions where Pitta and Vata/Kapha are blocked in the Rakta (blood) and Mamsa (tissue) dhatus. Why it works for Luteal Phase Acne: ••Anti-inflammatory & Anti-microbial: The Guggulu base, combined with Triphala and Guduchi, acts as a natural antibiotic and anti-inflammatory agent, targeting painful, cystic acne before they fully erupt. Hormonal & Metabolic Balance: It helps regulate Agni (tissue metabolism), preventing the excess sebum production triggered by the hormonal shifts of the luteal phase. ••Synergy: It pairs beautifully with Manjisthadi, enhancing its blood-purifying properties while adding a stronger “scraping” (Lekhana) action to clear clogged pores. ••Mahamanjisthadi Kwath 15–20 ml with equal warm water, twice a day (empty stomach / before meals). Continue blood purification and clearing pigmentation. ••Kaishore Guggulu 1 to 2 tablets, twice a day (after meals with warm water). Reduce inflammatory acne, deep-cleanse tissues, and manage luteal flares. ••Avipathi Choornam 1 tsp at bedtime with warm water (3 times a week, as you are doing). Maintain regular elimination and flush out Pitta.

Acne is triggered by an imbalance in the Pitta Dosha (the energy of fire and metabolism) combined with Kapha Dosha(the energy of structure and moisture). When these energies are aggravated by things like stress, poor digestion, or a heavy diet, they create a breeding ground for inflammation, leading to breakouts, redness, and painful cysts. The process starts deep in your digestive tract. When your digestion becomes sluggish, your body creates an accumulation of internal toxins called Ama. Because your body is trying to eliminate these impurities, they eventually make their way into your bloodstream and head straight for your skin. This toxic buildup clogs your pores and mixes with excess facial oil (sebum), which shows up on your face, chest, or back as active acne flare-ups and boils. To clear acne at its root, Ayurveda focuses heavily on internal purification. Herbal remedies like Mahamanjishtadi Kwath and Nimbakadhi Kwath Tablets act as powerful, natural blood purifiers. Packed with bitter, cooling herbs like Neem and Manjishta, they help filter out those circulating toxins from the inside. By cooling down the excess Pitta heat in your blood, these remedies naturally calm active, angry breakouts and give your body the support it needs to repair tissue from within. Finally, what you eat and how you manage stress can either feed the fire or help put it out. To keep acne at bay, it is incredibly helpful to avoid foods that overheat your system—like highly spicy, sour, fermented foods, and deep-fried items. Instead, leaning into light, cooling meals like freshly cooked vegetables and drinking warm water helps flush out lingering toxins. When you pair this clean eating with a calm mind through gentle breathing or meditation, you lower the stress hormones that trigger oil production, giving your skin the perfect environment to clear up and heal.

Hello, Your pattern sounds very typical of hormonal luteal phase acne -acne flares before periods -pigmentation slowly improving -ongoing inflammatory breakouts -sensitive/reactive skin The good sign is -Manjisthadi is already helping pigmentation That means inflammation and skin healing are responding But hormonal acne usually improves more slowly than pigmentation Will you ever get clear skin? Yes- many women with lutealphase acne improve significantly but -hormonal acne often needs 3-6 months of consistent treatment -stress/sleep/ hormonal balance matter greatly -skin barrier care is important So don’t judge results after only 1 month Ayurvedic view This pattern commonly reflects -pitta + rakta aggravation -hormonal inflammatory response -sometimes mild kapha blockage -stress related flare tendency About avipattikar churna using it 3 times/week is reasonable if -yyou have acidity/heat tendency -constipation -inflammatory flares It helps reduce -excess pitta -gut health -inflammatory burden But avoid overuse if stools become loose or weakness develops Should you add Kaishor guggulu? Possibly helpful if -acne is inflammatory -painful red pimples occur -body heat/pitta signs are strong -breakouts persist despite Manjisthadi Kaishor guggulu is traditonally used for -inflammatory skin condition -acne -rakta/pitta imbalance But -it can sometimes aggravate -acidity, dryness and sensitive digestion So if you already have -reflux -gastritis -loose stools -strong heat sensitive Then use cautiously Stronger medicine is not always better Since -pigmentation improved -acne is moderate and not severe You ma benefit more from -consistency -hormonal balance -gut support -sleep/stress correction rather than stacking many medicines quickly Very Important for Luteal acne sleep and stress Hormonal acne is strongly worsened by -stress -late sleeping -cortisol fluctuation Even healthy eating cannot fully compensate for poor nervous system balance Diet reduce during luteal phase -sugar -whey protein -excessive dairy -fried foods -ultra processed snacks Helpful foods -warm cooked meals -vegetables -flax/chia seeds -adequate protein -hydration Skin care very important avoid over irritating the skin better -gentle cleanser -non comedogenic moisturiser -sunscreen consistently Over exfoliation can worsen pigmentation + inflammation Ayurvedic support 1) Neem capsules= 1 cap twice daily after meals for 21 days =best for inflammatory acne 2) Amla= 1 tsp churna with warm water in morning emoty stomach =supports pitta balance, collagen, skin healing Hormonal evaluation worth considering if acne perists Especially if -chin/jawline acne -irregular periods -facial hair -weight gain then do -androgen profile -insulin resistance evaluation -pcos reassessment Even if cycles seem normal now Most hormonal acne improves gradually with consistent internal + external care, but it usually takes pateince rather than rapid medicine changes Do follow Hope this might be helpful Thank you

Yes, clear skin is possible. Hormonal acne usually takes 2–4 months to improve. You can add Kaishore Guggulu 2 tablets twice daily after food with warm water. Continue Manjishtadi Kwath. Use Avipattikar Churna only if acidity or constipation is present. Avoid sugar, fried food, excess dairy, and late nights during luteal phase.

Yes, hormonal acne during the luteal phase can take a little longer to settle, so improvement in pigmentation first is actually a positive sign. Clear skin is possible, but consistency and hormone balance are important. Avoid excess sugar, dairy, fried foods, and late nights during the premenstrual phase. Keep digestion regular and avoid constipation. Continue Manjishtadi as advised Continue Avipattikar in moderate dose Kaishore Guggulu – 1 tablet twice daily after food can be added if acne is inflamed/red You can also use plain aloe vera gel or sandalwood-based gentle application externally. Usually hormonal acne improves gradually over 2–3 cycles with proper management. Regards, Dr Raghuveer (Ayurvedacharya)
