The spleen has been enlarged since childhood, and the portal vein is dilated, but the liver is normal. The spleen size is currently 18 cm, and immunity is weak, causing constant fatigue. Please help with a cure for this condition

Enlarged spleen, or splenomegaly, requires careful attention, especially with the dilation of the portal vein. In Ayurveda, this condition might link to imbalances in Pitta and Vata dosha. A holistic approach focusing on their pacification while supporting liver function and immunity can be considered. First, let’s address diet. Shun spicy, acidic, and oily foods as they aggravate Pitta. Incorporate bitter greens like kale and methi (fenugreek) which help detoxify. Foods rich in iron and vitamins like beetroot and pomegranate may support energy levels and immunity — ideally, consume them lightly cooked for better digestion.

Herbal therapy can be of assistance. Guduchi (Tinospora cordifolia), renowned in Ayurveda for boosting immunity, can be consumed in powder form, about 1 teaspoon mixed with warm water daily. Use Triphala at bedtime to regulate bowel movements and aid detoxification. However, be sure to consult with your primary healthcare provider before starting any herbal regimen, especially if other medications are used concurrently.

Lifestyle also plays a pivotal role. Engage in gentle yoga or pranayama like Anulom Vilom to enhance circulation and energy without straining the body. Avoid air-conditioned environments excessively, as cold can exacerbate Vata issues.

Consider regular Abhyanga (oil massage) with warm sesame oil, as this can calm Vata and enhance blood circulation — once or twice weekly in the morning before showers can suffice. From a Siddha-Ayurvedic perspective, this condition may be linked to imbalances in the doshas, likely involving kapha and pitta, considering the spleen and blood-related issues. The primary goal would be to support the spleen’s function and strengthen your immunity while keeping the digestive fire, or agni, strong.

Start by incorporating the herb Punarnava (Boerhavia diffusa) which is known to support spleen health and reduce inflammation. A decoction can be prepared by boiling a teaspoon of Punarnava root powder in two cups of water until it reduces to one cup. Consume this once daily.

Additionally, a blend of Triphala, which balances all three doshas and supports digestion and elimination, should be taken. Mix half a teaspoon of Triphala powder with warm water and take it at night before bed. This will help support the body’s natural detoxification processes and improve digestive agni.

Weak immunity may benefit from Chyawanprash, traditionally known to boost resilience. Taking a teaspoon of Chyawanprash on an empty stomach in the morning could help build stamina and immunity over time.

It’s also crucial to manage stress and rest adequately since they greatly affect immunity. Practices like gentle yoga and pranayama can be integrated to enhance energy circulation and reduce fatigue.

However, given the complexity of your condition, continue consulting with a medical professional regularly to monitor your spleen’s size and function. An integrative approach blending modern and traditional treatments often yields the best results while ensuring safety.
