My daughter had an accident, she has grade 2 tibia fibula fracture with ORIF - rod insertion in both legs. She is recovering but is stiff at times and still walking and moving is restricted, it’s been 10 weeks from surgery. We started hadjod and lakshada Guggul after first six weeks, we want to know when can she have oil therapy to relax muscles as her left leg fracture was open wound fracture and right was closed

Hello, I am so sorry to hear about your daughter’s accident. Managing a bilateral grade 2 fracture with hardware (ORIF) is a marathon, not a sprint. At 10 weeks, she is in a critical transitional phase where the bone is knitting, but the soft tissues muscles, tendons, and fascia are often “frozen” from disuse and the trauma of surgery. ​When to Start Oil Therapy (Abhyanga) ​For the right leg (closed fracture), oil therapy can usually begin once the surgical incision is fully scarred over and there is no internal heat or redness. ​For the left leg (open wound fracture), you must be much more cautious. Even if the skin looks closed, the deep tissue trauma was higher. ​Wait until the surgeon confirms the bone callous is stable and there is no risk of osteomyelitis (deep bone infection). ​Avoid the hardware sites: If the rods or screws are palpable under the skin, avoid applying heavy pressure directly over them. ​ The Vata Stage ​In Ayurveda, a fracture is a Bhagna. After 6–8 weeks, the “healing” phase transitions from stabilization to “Vata-pacification.” Stiffness is caused by excess Vata (cold/dry energy) in the bone and muscle. Oil therapy serves to “melt” this stiffness and improve blood flow (Srotas) to speed up bone mineralization. Ayurved medicine : ​For a 10-week post-op recovery, focus on oils that nourish the bone while reducing nerve stiffness. ​1. External Oil (The Method) ​Ksheerabala Thailam: This is the gold standard for post-fracture stiffness. It is processed with milk and Sida cordifolia, making it highly nourishing for nerves and muscles. ​How to use: Do not massage vigorously. Use the Anuloma stroke (light, downward strokes). Warm the oil slightly. Apply for 20 minutes before a lukewarm bath. For the left leg (open fracture), only apply it to the surrounding muscles (calf/thigh) and avoid the direct scar area if it remains sensitive. ​2. Internal Medicine ​Gandha Thailam Capsules: This is the specific “bone-joining” oil of the Kerala tradition. ​Dose: 1 capsule twice daily after food with warm milk or water. ​Kshirabala (101) Avartti: This is a potentiated version of the oil for internal use to calm the nervous system and reduce the “stiffness” sensation. ​Dose: 5 to 10 drops in a cup of warm milk at bedtime.

At 10 weeks post-surgery, oil therapy can be started gradually, but with caution since one side had an open fracture. First priority is: Ensure wound is completely healed (no discharge, no infection, no tenderness) before applying oil directly on that area. For now, you can start gentle oil application around the muscles (not directly on scar initially). Use warm oil (like Mahanarayan oil or Bala Ashwagandha oil) and do very light massage, no deep pressure. Start with: 5–10 minutes gentle application once daily, followed by mild warm fomentation. Avoid strong massage over operated area. Continue physiotherapy regularly, as that is most important for mobility recovery. At home: Keep limb slightly elevated when resting. Gentle movement exercises as advised should be continued. Lakshadi Guggulu – continue as advised Hadjod (Cissus) – continue These support bone healing along with oil therapy. After a few weeks, once comfort improves, you can slowly increase duration and pressure (only if no pain). If swelling, pain, or redness increases, stop and reassess. At 10 weeks after ORIF, be careful with oil therapy. Open fracture leg (left) Start oil massage only after complete wound healing (no wound, no discharge, no redness). If fully healed now, you can start very gentle massage. Closed fracture leg (right) Can start oil massage now. How to do (both legs) Use Mahanarayana taila or Dhanwantharam taila Gentle massage only (no pressure) – 10–15 min daily Follow with mild hot water fomentation (warm towel) Continue medicines Hadjod (Cissus quadrangularis) – as taking Lakshadi guggul 1 tab twice daily after food Do physiotherapy regularly (very important for stiffness) Avoid strong massage or forceful movements

You must wait until the skin and deep tissues at the site of the open wound have completely epithelialized (closed without any scabs or discharge). Even then, the area directly over the ORIF hardware should be handled very gently. Guidelines for Application: *Avoid “Massage”: At 10 weeks, do not “massage” or apply pressure. Instead, perform “Anuloma Abhyanga”—a very gentle downward stroking of the oil. *Avoid the Incision: Stay away from the surgical scar itself until the surgeon confirms the internal hardware is stable and the skin is fully matured. *Temperature: The oil should be lukewarm, as heat helps improve blood circulation to the bone. Recommended Ayurvedic Oils— For stiffness and muscle relaxation post-surgery, these oils are most effective: Kottamchukkadi Thailam Mahanarayan Taila Murivenna Dietary Support: Ensure she is getting enough Ghee and Calcium-rich foods. You can add Ashwagandha Churna (1/2 tsp twice daily with milk) to her routine to help with her low energy and fatigue.
