During pregnancy, recurrent pus discharge after an ischiorectal abscess can happen when a fistula tract remains internally. From an Ayurvedic perspective, this resembles Bhagandara, where deeper inflammation and infection continue beneath partially healed tissue. Since you are 24 weeks pregnant, treatment must remain gentle and safe. Conservative care and close follow-up with your surgeon are important until delivery unless symptoms worsen. Supportive Ayurvedic Continue keeping the area clean and dry. Take warm sitz baths 2–3 times daily, especially after bowel movements. You may use plain warm water or mild antiseptic solution only if advised by your doctor. Avoid prolonged sitting; use a soft cushion or donut pillow for support. Prevent constipation by taking adequate water, fiber, cooked vegetables, fruits, and light meals. Avoid very spicy, oily, and heavy foods which may aggravate inflammation. Rest adequately and avoid strain or lifting heavy weight. Traditionally, Ayurveda considers therapies like Ksharasutra effective for fistula management, but this is generally evaluated after delivery unless urgently needed. Please seek prompt medical review immediately if you develop: Fever or chills Increased swelling or redness Severe pain Foul smell Heavy pus discharge Difficulty passing stool or urine Because you are pregnant, do not start any internal Ayurvedic medicines or herbal formulations without direct supervision from a qualified doctor and your obstetrician. Proper wound care, nutrition, and monitoring are most important right now.

Your symptoms fit fistula-in-ano after ischiorectal abscess. Because you are 24 weeks pregnant, only safe, mild conservative care should be used until delivery. Ayurvedic management (pregnancy-safe, conservative) Triphala Churna: 3 g at night with warm water (keeps stool soft) Gandhak Rasayan: 250 mg twice daily after food Shatavari Churna: 3 g with milk twice daily (supports healing, safe in pregnancy) 2. Local treatment Sitz bath with Triphala decoction: 10–15 minutes, 2 times daily After cleaning, apply Jatyadi Taila locally with sterile gauze, 1–2 times daily Keep area dry and clean always 3. Diet and routine High-fiber diet, plenty of warm water Avoid constipation, spicy and fried food Do not sit long; use soft cushion Gentle walking daily Dr RC BAMS MS

Because you are pregnant, the primary goal is conservative care to manage symptoms until after delivery. Internal medications must be kept to a minimum---- 1.Shatavari capsules 1 cap twice daily with warm milk after meals 2.Gandhak rasayan 2 tab twice daily with water after meals 3.Triphala churna 1 tsp at bedtime with warm water Local Wound Care -Sitz Baths: Take a warm sitz bath for 10–15 minutes twice daily. You can use a Triphala decoction (boiled and strained Triphala water) -Application: After cleaning and drying the area, apply Jatyadi Taila using sterile gauze. -Sitting Support: Use a donut pillow or soft cushion to relieve direct pressure on the site when sitting. Important Precautions -Diet: Maintain a high-fiber diet with plenty of cooked vegetables and fruits to prevent constipation, which can worsen fistula pain. Avoid spicy and oily foods. -Surgery: Definitive Ayurvedic surgical treatments like Ksharasutra are usually deferred until after delivery. -Red Flags: Seek immediate medical attention from your surgeon or obstetrician if you experience fever, chills, severe pain, or foul-smelling discharge.

AS you are pregnant major part of te treatment would be based on diet and external applications. Your diet goals right now are: ✔ Soft, easy stools no straining ✔ Reduce inflammation ✔ Support pregnancy nutrition ✔ Avoid foods that worsen discharge/irritation Full day diet plan: Early Morning (6–7 am) 1 glass lukewarm warm water 4–5 soaked raisins + 1 soaked fig (anjeer) Breakfast (8–9 am) Choose one: -Vegetable oats porridge + 1 tsp ghee -Whole wheat vegetable poha -Moong dal chilla with soft chutney Add: 1 fruit (apple / pear) Mid-Morning (11 am) 1 glass buttermilk (with roasted jeera + pinch rock salt) or coconut water Lunch (1–2 pm) 1–2 phulka with ghee 1 bowl dal (moong preferred) 1 bowl vegetable (lauki, tori, pumpkin, carrot, spinach) 1 small bowl rice (optional) Salad (cucumber + beetroot — small quantity if no gas) Evening Snack (4–5 pm) Roasted chana or handful peanuts or fruit + handful nuts Dinner (7:30–8:30) Light meal (important!) Vegetable khichdi + ghee or dal + soft chapati + cooked veggies Must: -2.5–3 liters water -1–2 tsp ghee daily -Eat at regular times (gut rhythm matters) Avoid: Spicy, oily, fried food Red meat Excess tea/coffee Bakery items (maida, biscuits) Cheese-heavy or junk food Very dry foods (chips, namkeen) Constipating foods (raw banana, excess potatoes) Medications: External Jatyasi tailam Make it lukewarm, soak a sterile cotton swab in it and keep it in the ana region at night. Sitz bath. Lukewarm water mixed with tumeric, sit in it for atleast 20 minutes daily twice a day. Internal Isabgol (phyllium husk) Soak half tsp in one glass of water for half an hour and take it at night. No other internal medicines can be used in pregnancy. You should visit your surgeon and gynacologist for antibiotics and NSAIDS in pain is too much.

Hello, Andrew, this is a clear case of fistula in ano during pregnancy, and your surgeon’s advice (conservative until delivery) is correct and standard. The reopening + continuous pus means the tract is still active, not fully healed First priority (very important) Since you are 24 weeks pregnant, No aggressive procedures now unless emergency (abscess, severe infection) -Continue conservative management till delivery -Watch for worsening signs (fever, severe pain, swelling) Ayurvedic view -Bhagandar= fistula -caused by= pitta + kapha (pus formation), vata (tract formation + pain) Goal now -control infection -reduce discharge -support healing -keep area clean Internal support 1) Gandhak rasayana= 1 tab twice daily after meals =safe antimicrobial + wound healing support 2) Stool soft + healing very important -Triphala churna= 1 tsp at night with warm water =prevents strain ->key for fistula Local care most important in your case 1) Sitz bath= 2-3 times daily -warm water + turmeric + pinch of salt =reduces infection + pain 2) Dressing support -Apply jatyadi taila on sterile gauze =promotes healing, reduces pus Keep area= clean, dry, well ventilated Sitting and daily comfort -use soft cushion/donut pillow -avoid sitting > 30-40 min continuously -lie on side when possible Diet eat -high fiber vegetables, oats -warm fluids -ghee small amount-> healing Avoid -spicy, fried food -constipation triggers -excess sugar What to expect -discharge may continue during pregnancy (common) -pain and irritated can be controlled -definitive cure like ksharasutra surgery -> after delivery Seek immediate care if -fever develops -sudden swelling abscess reformation -severe pain increase -foul smell or bleeding Current condition= active fistula (controlled, not dangerous yet) Goal now -control infection -stay comfortable -prevent worsening Definitive treatment= after delivery Do follow Hope this might be helpful Thank you
