Hello dear, I am very sorry to hear about the intense pain you are experiencing. Dealing with Rheumatoid Arthritis (RA) for five years, especially while managing the side effects of long-term medication, is incredibly taxing. In Ayurveda, severe chronic RA is often viewed as Amavata. This condition is understood as the accumulation of Ama (metabolic toxins) and the aggravation of Vata (the energy of movement), which settle in the joints, causing extreme inflammation and “lock-down” pain. When pain increases despite starting Ayurvedic herbs, it is often because the internal “fire” (Agni) is too weak to process the medicines, or the Ama is deeply seated. Given the severity of your symptoms and your history of gastrointestinal issues, a “deep-cleansing” approach is often recommended over simple oral tablets. The goal for chronic, severe Amavata is to neutralize toxins and then lubricate the joints. Since you are in Ahmedabad, you are in a great location to seek specialized care. Langhana & Deepana: Lightening the body and boosting digestion to clear the “sludge” that causes inflammation. External Therapy: In your state, treatments like Podikkizhi (herbal powder massage) or Dhanyamla Dhara (pouring of fermented liquids) are traditionally used to reduce swelling before any oil-based treatments. Nidana Parivarjana: Strictly avoiding curd, heavy sweets, and cold foods, which “feed” the inflammation. Prescription : 1. Rasnadi Kashayam: Take 15 ml diluted with 45 ml of lukewarm water twice daily, 30 minutes before food. This is a potent anti-inflammatory for joint stiffness. 2. Kshirabala (101) Capsules: 1 capsule twice daily after food. This is a “processed” version of the oil that helps soothe the nervous system and chronic Vata pain. 3. Jirakadyarishtam: 20 ml with an equal amount of water after lunch. This helps specifically with the bloating and nausea you experienced from previous medications. 4. Pinda Thailam: For very gentle external application only. Do not massage vigorously; simply apply and leave it on for 30 minutes before a warm bath. Since your pain is currently unmanageable and increasing, please visit an Ayurvedic hospital for “Panchakarma” (In-patient detox). Oral medicines alone often struggle to penetrate the tissues when the condition is this advanced. A 14 to 21-day course of clinical treatments like Virechana (purgation) or Vasthi (medicated enemas) can often provide a level of relief that tablets cannot achieve on their own.

This is Amavata (Rheumatoid Arthritis) with active ama + high inflammation. In this stage, heavy rasayana or multiple drugs without clearing ama can worsen pain. Short Ayurvedic treatment (focus first on ama pachana): Simhanad Guggulu 2 tablets, twice daily after meals Amruthotharam Kashayam 15 ml + equal warm water, twice daily before meals Rasnadi Kashayam 15 ml + equal warm water, twice daily before meals Mahayograj Guggulu 1 tablet, twice daily after meals Eranda Taila (castor oil) 1–2 teaspoons with warm milk at bedtime (for ama clearance) External Warm Mahanarayana Taila massage + mild steam daily Diet (very important): Strict ama-reducing diet: warm, light food (moong dal, khichdi) Avoid curd, cold food, heavy, fried, non-veg, bakery Take dry ginger + ajwain water

Your condition appears to be a severe form of Amavata (Rheumatoid Arthritis) in Ayurveda, where Ama (toxins from weak digestion) combines with aggravated Vata, leading to deep joint inflammation, swelling, stiffness, and pain. Since symptoms worsened despite medicines, it suggests the inflammation is still active and digestion/metabolism (Agni) remains weak. Important First Step Because your pain is severe and you have swelling, reduced appetite, bloating, and nausea, you should not continue self-medication without supervision. Some formulations may not suit your current digestive state or body constitution. Ayurvedic Approach At this stage, treatment often requires a stepwise approach, not only tablets: Ama reduction & digestion correction Light, warm, easy-to-digest food Avoid curd, cold foods, fried items, bakery foods, excess pulses, and packaged foods Sip warm water throughout the day Pain & inflammation management Gentle castor oil use (if suitable) External therapies like: Warm medicated oil application Patra pinda sweda Mild steam therapies under supervision Panchakarma support In chronic RA, many patients benefit more from supervised therapies such as: Virechana Basti therapy rather than oral medicines alone. Very Important If pain is rapidly worsening despite treatment, please also follow up with your rheumatologist. Severe uncontrolled RA can damage joints permanently and sometimes requires adjustment of modern medicines alongside Ayurveda. Rx 1. Rg forte cap - 1 cap twice a day after meal 2.myaxyl cap - 1 cap twice a day after meal 3. Sahacharadi kwath- 15 ml twice a day after meal 4. Maharasna di kwath tab - 1 tab twice a day after meal

It sounds like you are going through an incredibly difficult time. Managing severe Rheumatoid Arthritis (RA)—referred to in Ayurveda as Amavata—requires a very delicate balance, especially when the transition from long-term conventional medicine to Ayurvedic treatment coincides with a flare-up and a loss of appetite. Given the severity of your pain and the reduction in your appetite, the primary focus in Ayurveda is typically on improving digestion (Agni) and clearing metabolic toxins (Ama) before heavy medications are introduced. Ayurvedic Perspective on Your Symptoms In Amavata, if the digestive fire is weak, even “good” medicines can sometimes contribute to further “Ama” (toxin) buildup, which may explain why you feel the pain is increasing and your appetite has dropped. I have prescribed medications for your condition take them consistently , it can take some time because ayurveda works slowly as it acts on roots cause of your problem . The “Dos” • Warmth is Key: Drink only warm water. Use warm compresses (Dry heat like a sandbag or salt bag is often preferred over steam for RA). • Light Diet: Since your appetite is low, stick to Mudra Yusha (Moong dal soup) or ginger-infused thin rice gruel. This helps rekindle your digestive fire. • Ginger and Garlic: Incorporate processed ginger (Shunti) and garlic into your diet, as they are potent anti-inflammatories in the Ayurvedic tradition. • Rest: Ensure your joints are supported and rested during high-inflammation phases. The “Don’ts” • Avoid Heavy Foods: Curd (yogurt), sweets, heavy grains, and fried foods are known to increase “Ama” and joint stiffness. • Avoid Cold: Stay away from cold water, AC drafts, and refrigerated foods. • No Vigorous Massage: In the acute “Ama” stage (severe swelling), heavy oil massages can sometimes block channels further. Only gentle application of medicated oils is recommended. • Daytime Sleep: Sleeping during the day can slow down metabolism and increase stiffness.

Your condition suggests active Aamavata (Rheumatoid Arthritis) with high Ama and weak digestion, which is why pain is worsening and appetite is low. At this stage, multiple heavy medicines may aggravate symptoms. First focus on Ama pachan (improving digestion): Take light, warm food (khichdi, soups) Use ajwain–jeera–dry ginger water in small sips You may take mild Hingwashtak or Ajmodadi churna before meals Avoid heavy, oily, cold, and fermented foods. Do not do oil massage now; use dry heat for pain. Once digestion improves, medicines can be restarted gradually in a planned way. You need stage-wise treatment, not direct long-term medication. Supervised Ayurvedic care is strongly advised.

Dear patient, We understand your pain. Right now your body has high Ama (toxins) and weak digestion, so strong(specially guggulu) or multiple medicines and oil application can increase swelling and pain. We will first reduce Ama, then treat joints. 💊 Medicines Guggulutiktaka Kashayam – 15 ml + warm water, twice daily before food Kineaz tablet – 1 tablet twice daily after food Ajmodadi Churna – before meals Eranda Taila/ Castor oil capsule – 1 at bedtime with warm water 🌿 External Kolakulathadi Churnam – apply with buttermilk or lukewarm water, once daily 🍽 Diet Avoid: rice, curd, cold/heavy food, excess walking Take: warm, light meals + warm water 👉 First step is Ama reduction, then pain will gradually improve. Stay stressfree n happy Dr SHubhi, Ayurveda Consultant

Severe pain even after starting Ayurvedic medicines usually means the condition is still in an active inflammatory phase (ama stage). In this stage, strong guggulu-based medicines sometimes do not give relief immediately and can even feel heavy if digestion is weak. Right now, the priority is to reduce inflammation and improve digestion first, rather than continuing multiple medicines together. Keep your diet very simple for some time — warm, light food like moong dal, rice, vegetable soups. Avoid curd, cold food, heavy, fried, and processed items. Do not skip meals even if appetite is low. Avoid oil massage directly on very inflamed joints. Instead, use mild warm fomentation (heat) for relief. At home, you can take warm water with a pinch of dry ginger through the day to reduce ama and inflammation. For now, simplify medicines: Simhanad Guggulu – 1 tablet twice daily after food Maharasnadi kwath – 15 ml with equal warm water twice daily Castor oil – 1 teaspoon with warm water at bedtime (2–3 times a week) Stop taking too many combinations together for now. Since pain is very severe and long-standing, you may need supervised Panchakarma (like Virechana or Basti) for better control — medicines alone may not be enough in this stage. Also, do not abruptly stop your previous allopathic medicines without guidance, as RA needs continuous control. With the right staged approach, pain can be reduced, but it will take consistent management. Regards, Dr Raghuveer (Ayurvedacharya)

Hello, This is not a mild imbalance anymore. After 5 years of Rheumatoid arthritis with severe pain + swelling + poor appetite, this has progressed to Ayurvedic view -Amavata (advanced stage) -deep ama (toxins)+aggravated vata in joints -now entering ‘saama’-> nirama transition failure (treatment not working because Ama still present) That’s why current medicines (even good ones like sinhanad guggulu) are not giving relief- your Agni is weak + Ama still high First Rule (very important) Do Not take heavy joint medicines blindly right now Because -your appetite is low -ama is still present -> This will increase pain instead of reducing it (exactly what you’re experiencing) Step by step management 1) Ama removal + agni correction (Top priority - 7 to 14 days) -Amruthotharam kashayam= 20ml + equal water twice daily before meals =best for ama + inflammation +feverish joint pain -Vaiswanara churna= 1/2 tsp before meals with warm water =improves digestion, reduces bloating -Hinguvachadi gulika= 1 tab before meals twice daily =relieves gas, abdominal discomfort Goal= Bring back hunger + reduce ama Don’t expect joint pain relief yet- this is preparation phase 2) Once Appetite improves (very important shift) -Simhanada guggulu= 2 tabs twice daily after meals =classic for Amavata (clears ama + joint inflammation) -Yogaraj guggulu= 2 tabs twice daily after meals =deep vata control, chroinc stiffness -Rasnasaptaka kashayam= 20ml with warm water before meals =strong anti inflammatory for joints For severe pain (immediate relief support) -Dhanwantaram tailam= warm oil local application twice daily =reduces stiffness + improves mobility If pain unbearable-> mild steam after oil at home Diet this will decide your result -warm, light food -moong dal, rice, soup -garlic + dry ginger in cooking -jeera-ajwain water Strictly avoid -curd big trigger in RA -cold food, fridge items -wheat heavy, bakery items -non veg (especially during flare) -night eating Most Important part Your case now needs panchakarma Best therapy -Virechana (detox for pitta + ama) -Followed by Basti (main treatment for vata/RA) Medicines alone WON’T be enough now This is why you’re not improving When to act urgently -severe swelling + unbearable pain -no appetite for many days -continue fatigue Don’t delay panchakarma in these cases You are stuck because -ama not cleared -agni weak -direct RA medicines started too early Correct way= digest->detox->then treat joints Do follow Hope this might be helpful Thank you

Hello, I understand your pain and frustration, especially after trying multiple treatments without proper relief. But I want to be very honest with you—starting or changing treatment without fully understanding your gut condition and root cause is like walking on a double-edged sword. Sometimes, the same medicines we take to control a disease can worsen underlying issues (especially digestion, metabolism, and Ama in your case), which can actually increase pain instead of reducing it. So before we move further with treatment, it’s very important to assess your internal condition properly. I’m sharing some important questions below—please answer them carefully and honestly. This will help me identify the root cause and guide you in the right direction. Please answer the following: 1-How is your appetite? (Normal / Low / Variable) 2-Do you feel heaviness, bloating, or gas after meals? 3-What is your bowel pattern? (Constipation / Loose stools / Alternating / Sticky stools) 4-Do you feel stiffness more in the morning that improves with movement? 5’Is there swelling and warmth in joints, or only pain? 6-Do your symptoms worsen after certain foods like curd, cold items, heavy food, or tea? 7-How is your sleep quality? (Disturbed / Refreshing / Pain-related awakening) 8-Do you feel fatigue, body heaviness, or lethargy throughout the day? 9-Have you noticed coating on your tongue (especially white thick coating)? 10-Are you currently still taking any allopathic medicines along with Ayurvedic ones?

Hello. I completely understand how unbearable this flare-up is, and we need to correct your protocol immediately. Here is exactly why the Ayurvedic medicines you started 1 month ago are increasing your pain instead of reducing it: Your appetite is significantly reduced, and you have a history of severe bloating and nausea. In Ayurveda, Rheumatoid Arthritis is called Amavata-a disease caused by Ama (undigested toxic sludge) being pushed into the joints by Vata. When your digestive fire (Agni) is this weak, heavy tablet formulations like Singhnad Guggulu and Vatarin cannot be digested. Instead of acting as medicine, they are sitting in your gut, fermenting into more toxins, and physically adding to the swelling and extreme pain in your joints. We must stop “feeding” the disease and first reignite your digestive fire. The Detox & Rescue Protocol (Immediate Action Plan) We must temporarily stop the heavy tablets, clear the toxic blockage in your gut, and use purely external dry heat to physically melt the swelling in your joints. 1. Medication Adjustments STOP Temporarily: Halt the Singhnad Guggulu, Vatarin, and Punarnawadi immediately. Your body simply cannot process them right now. CONTINUE Amrutottaram Kashayam: This is the only medicine you should keep. It is a mild, excellent digestive and anti-inflammatory liquid. Dosage: 15 ml mixed with 15 ml of warm water. Time: Twice daily, strictly on an empty stomach (30 minutes before meals). START Sanjivani Vati: Dosage: 1 tablet twice daily. Time: 30 minutes after meals with warm water. 2. Crucial External Therapy (Valuka Sweda) Strict Rule: Because your joints are actively inflamed and swollen, do not apply any herbal oils or massages. Oil will instantly trap the toxins and double your pain. Dry Sand Fomentation: You must use dry heat. Take a cup of clean, coarse sand (or rock salt), heat it in a dry pan, and tie it tightly inside a thick cotton cloth to make a bolus (Potli). Press this hot, dry bolus against your swollen joints for 10-15 minutes twice a day. This dry heat physically cuts through the heavy Ama and provides immediate, soothing relief to the severe joint stiffness. 3. The Langhana (Lightening) Diet For the next 5 to 7 days, you must eat like someone recovering from a severe fever. No heavy grains, no dairy, no raw salads, and no sweets. Sip on hot water boiled with fresh ginger throughout the day. Eat only warm, soupy foods like Mung Dal Peya (very thin green gram soup) or plain Khichdi. You must give your digestive system zero mechanical workload so it can focus entirely on burning away the joint inflammation. Regards Dr Gursimran Jeet Singh MD Panchakarma

Let me be very clear taling NSAIDS and analgesic for this much time will affect your body. So ayurveda can treat this but you have to take panchakarma therapy. Mainly vaitran basti. This i told for your reference 1000s of research has been published on this therapy for RA and the results shows itself. In a year 2 to 3 sittings with medicines.
