••In Ayurveda, tinnitus is known as Karnanada (sound in the ear) or Karnakshweda (ringing/whistling in the ear). It is primarily classified as a Vataja Vyadhi (a disorder caused by the vitiation of Vata Dosha), where aggravated Vata enters the sound-carrying channels (shabda-vaha srotas) of the ear, leading to false sound perceptions. ••Sthanika Chikitsa (Localized Therapies) Local interventions are highly effective for ear disorders as they directly target the site of Vata vitiation. ••Karna Purana (Ear Drops): Bilva Taila or Apamarga Kshara Taila: These are the classical choices for Karnanada. •Method: Instill 3–5 drops of lukewarm oil into the affected ear(s) once daily (preferably in the evening) and remain in a side-lying position for 10–15 minutes. Contraindication: Do not perform if there is active discharge, infection, or tympanic membrane perforation. ••Karna Pratisarana & Shiro-Abhyanga: Regularly massage the external ear, the area behind the pinna, and the scalp with warm Ksheerabala Taila or Maha Narayana Taila. This helps lower systemic and localized Vata. ••Nasya Karma (Nasal Therapy): The nose is considered the gateway to the head (Nasa hi shirso dwaram). ••Instill 2 drops of Anu Taila or Shadbindu Taila (or warm Goghrita / cow’s ghee) into each nostril in the morning on an empty stomach. This strengthens the sensory organs above the clavicle (Urdhvajatrugata Roga). 2. Abhyantara Chikitsa (Internal Formulations) The internal management focuses on Vata-Shamana (calming Vata), Rasayana (rejuvenation), and Balya (strengthening the nerves). Herbo-Mineral Formulations (Rasa Aushadhi & Vati) ••Saraswatarishta (with Gold / Gold-infused): 15–20 ml with an equal quantity of lukewarm water, twice daily after meals. It acts as an excellent Majja Dhatu Rasayana and calms the nervous system. ••Ashwagandharishta: 15 ml twice daily can be combined with Saraswatarishta to enhance Vata-pacifying and tissue-nourishing properties. ••Ekangaveer Ras or Sameerpannag Ras: 125 mg twice daily, traditionally used for severe Vata disorders affecting the nervous pathways. ••Laxmivilas Ras (Nardiya): 1 Tab (250 mg) twice daily with honey or warm water, beneficial for chronic upper respiratory and ENT-related chronic conditions. Single Herb Rejuvenators (Ghanavati / Churna) ••Ashwagandha Churna / Ghanavati: 1 tablet or 3g powder with warm milk or water at bedtime. ••Brahmi Churna / Shankhapushpi: Excellent Medhya herbs to improve microcirculation to the brain and auditory cortex, mitigating the psychological distress or perception of the sound.
