How to treat acne and lose weight for a 25-year-old female with sensitive skin? - #56312

Anusha

25 year old female -Kapha Pitta Prakriti. Main concerns: 1. Wants to clear acne 2. Wants to reduce about 7 pounds Acne background: She used to have significant acne in her high school and college years. In the last 2 years, she started using trenitron cream and that seemed to settle her acne mostly. But, in recent months, she took a pause from the creeam and also introduced some new skin care products. Now her skin is breakinng out again. The acne is reddish and covers her central cheek on both sides as well as some on her chin. Small red boils, and some with a white pus head. She also has acne scars. Her skin seems quite sensitive to environment and products. she sometimes tries products for skin glow or products meant to help with acne. Weight gain -She has a tendency to gain weight easily unless she is very careful with diet and works out above moderate intensity level. She will be moving to a diet which will be barley/ quinoa/ milllet/ vegetable/ less rice/ mung dhal / buttermilk based. Her digestion and BM is ok, but there is some white coating on tongue. She used to be lethargic in her younger years but not any more. She used to have food regurgitation in younger years ( immediatley after eating) but not any more. Her stress levels and sleep is ok. She is on iron supplement for low iron absorption. A few months ago she had some throat burn but that has gone away now.

From an Ayurvedic perspective, this presentation reflects a Kapha–Pitta imbalance with: Kapha: acne (whiteheads, recurrence, weight gain tendency, tongue coating) Pitta: redness, inflamed papules, sensitivity, scarring Root pattern: mild ama (metabolic toxins) + skin barrier reactivity due to product overload + irregular skin routine The goal is not suppression, but cleansing + cooling + lightening Kapha without aggravating Pitta. Ayurvedic Plan for Acne + Healthy Weight Reduction 1. Internal Detox & Skin Purification (Core Therapy) Morning (on empty stomach) Warm water with: 1 tsp Triphala powder (or Triphala tablet if sensitive) OR plain warm water if bowels are already strong Purpose: clears ama, improves skin clarity, reduces recurrence Key Herbal Support (daily for 6–8 weeks) Manjistha – blood purification, acne control Neem – anti-inflammatory, antibacterial Guduchi (Giloy) – Pitta balancing, skin healing Best form: capsule or decoction under supervision This trio is classic for acne + sensitive inflamed skin 2. Diet Plan (Kapha-Pitta Balancing + Weight Loss) Your planned diet is good — refine it slightly: Best foods Barley (excellent for Kapha reduction) Moong dal (light protein, skin-friendly) Quinoa (moderate quantity) Steamed vegetables (lauki, tori, pumpkin, carrots) Buttermilk (spiced lightly with roasted cumin) Reduce / avoid Excess rice (Kapha-increasing) Dairy (especially curd at night → acne trigger) Sugar, bakery foods, fried snacks Whey protein / heavy supplements Very spicy, oily foods (Pitta flare) Eating timing rule Biggest meal at midday (Pitta time = best digestion) Light dinner before 7:30 pm 3. Skin Care (Very Important in your case) Her acne is also product-reactive, so “less is more”. Morning Gentle wash: neem water or mild herbal cleanser Light gel-based moisturizer (non-comedogenic only) Night Cleanse gently Apply: Aloe vera gel + 1 drop neem oil (if tolerated) Stop for now Multiple “glow” products Random acne actives layering Harsh scrubs / chemical peels at home Skin needs calm restoration, not stimulation 4. Weight Loss (Kapha reduction without weakening body) Best Ayurvedic strategy 30–45 min brisk walking daily (slightly sweaty is ideal) 3–4 days/week light strength training or yoga flow Best Yoga Surya Namaskar (gentle but consistent) Bhujangasana, Trikonasana, Ardha Matsyendrasana 5. Simple Herbal Metabolism Support Trikatu (ginger + black pepper + long pepper) → improves digestion, reduces Kapha, supports fat metabolism (small dose only, avoid if acidity worsens) Ginger tea (morning or mid-day) 6. Key Ayurvedic Insight (Very Important) This is not just “acne + weight gain”. It is: “Rasa–Rakta dhatu imbalance with Kapha accumulation and Pitta inflammation.” So treatment must be: Clean blood (Manjistha, Neem) Calm heat (Guduchi) Reduce heaviness (Barley, movement) Stabilize skin barrier (minimal skincare)

Thank you, can you clarify the following. 1. What high quality brands do you recommend for Giloy, Manjistha, and Neem. How long to take these and what dosage and time? 2. Is Giloy ok if the person may have some thyroid antibodies (although her TSH is normal) 3. Can Triphala guggulu be used to achieve detox as well as reduce fat- what dose and timing 4. What about medohar vati or medohar guggul - are those too strong? 5. Is radish water or honey + lemon water advisable 6. When is Kadirarishta chosen instead of Manjista? , or how does Kaishore guggulu fit in . I received some recommendations for these medicines and I am confused 7. All recommendations must suit her gut as she has a tendency to have heartburn

1. Good-quality brands and typical dosage Some reputable Ayurvedic manufacturers include: Himalaya Wellness Baidyanath Dabur Arya Vaidya Sala Kottakkal Kerala Ayurveda Sri Sri Tattva Giloy (Guduchi) Typical dose: Powder: 2–3 g once or twice daily Tablet: 500 mg–1 g twice daily Best time: After meals if prone to acidity Avoid taking on an empty stomach if sensitive Duration: Usually 6–12 weeks, then reassess Manjistha Typical dose: Powder: 2–3 g twice daily Capsule/tablet equivalent as per label Best time: After meals Duration: 2–3 months commonly used for skin and inflammatory conditions Neem For someone with heartburn: Start very cautiously. 250–500 mg capsule once daily after food. Neem is cooling but can sometimes aggravate sensitive digestion if overused. 2. Is Giloy safe if thyroid antibodies are present but TSH is normal? This is the area where I would be most cautious. There have been reports suggesting that Giloy may stimulate immune activity, and avoiding it in people with known autoimmune thyroid conditions such as Hashimoto’s Thyroiditis. If: Thyroid antibodies are elevated, Hashimoto’s is suspected, Or autoimmune disease has not been ruled out, then I would avoid long-term Giloy use until thyroid status is clearly understood. If TSH is normal but antibodies are unknown, Manjistha is often the safer first choice for blood purification and skin support. 3. Can Triphala Guggulu help with detox and fat reduction? Yes, traditionally it is used for: Ama (metabolic toxins) Sluggish metabolism Mild obesity Constipation Inflammatory conditions Typical dose 1–2 tablets twice daily after meals For sensitive stomachs Start: 1 tablet after dinner for 1 week Then increase if tolerated Important Because it contains Guggulu, some people experience: Acidity Gastric irritation Heat sensation In a person with heartburn, proceed slowly. 4. Medohar Vati / Medohar Guggulu – too strong? For someone with: Chronic acidity Heartburn tendency Sensitive digestion I generally consider Medohar formulations stronger than necessary initially. Many contain: Guggulu Trikatu (ginger, black pepper, long pepper) Other heating herbs These can aggravate: Burning sensation Acid reflux Gastritis I would usually prefer: Correct bowel movements first. Improve digestion gently. Add Manjistha if needed. Consider weight-management medicines later. 5. Radish water or honey + lemon water? Radish water Can help: Water retention Sluggish liver-bile function Kapha accumulation But may cause: Gas Reflux Stomach irritation If she already has heartburn, use cautiously. Honey + lemon water For acidity-prone individuals: Strong lemon water on an empty stomach is often not ideal. A better option: Warm water alone Or warm water with ½ teaspoon honey Avoid concentrated lemon if it triggers symptoms. 6. Manjistha vs Khadirarishta vs Kaishore Guggulu This is where many people get confused. Manjistha Best when: Skin disorders Acne Pigmentation Inflammation General blood purification Most gentle among these options. Khadirarishta Traditionally chosen when: Chronic skin disorders Itching Eczema Recurrent rashes Fungal tendency However, it is an Arishta (fermented preparation), which may sometimes worsen acidity in sensitive individuals. Kaishore Guggulu Usually chosen when there is: Inflammation Joint pain Elevated uric acid Deep-seated toxins with Pitta involvement Recurrent boils or inflammatory skin conditions Contains Guggulu, therefore can be more heating than plain Manjistha. If heartburn is a concern My order of preference would usually be: Manjistha Low-dose Neem (if tolerated) Kaishore Guggulu only if there is significant inflammation Khadirarishta only if skin symptoms strongly indicate it and digestion is stable 7. A heartburn-friendly Ayurvedic approach For someone with: Acidity/heartburn tendency Mild weight concerns Need for detoxification Possible inflammatory or skin issues A gentler approach is often: Morning Warm water Light walk After breakfast Manjistha After lunch Fennel tea or coriander-fennel-cumin water After dinner Triphala (or low-dose Triphala Guggulu if constipation and weight issues are significant) Avoid initially High-dose Neem Strong lemon water Aggressive Medohar formulations Multiple Guggulu products together

For a Kapha-Pitta prakriti with acne, mild ama (white tongue coating), and weight gain tendency: Take Kaishora Guggulu – 2 tablets twice daily after food. Gandhaka Rasayana – 250 mg twice daily after food. Triphala Churna – 3–5 g at bedtime with warm water. Arogyavardhini Vati – 1 tablet twice daily after food for 6–8 weeks. External: Apply a paste of Lodhra + Neem powder with rose water for 15 minutes, 3 times weekly. Avoid frequent changes of skincare products. Diet: Continue barley, millet, quinoa, mung dal, vegetables, and buttermilk. Avoid excess sugar, fried foods, bakery items, cheese, curd at night, and whey protein. Weight reduction: Warm water with 1 tsp honey in the morning. Brisk walking or exercise 45 minutes daily. Monitor iron levels and continue iron supplementation as advised by her physician. If acne is severe, persistent, or causing significant scarring, dermatology evaluation is also recommended.

••This is a classic clinical presentation of a Kapha-Pitta Prakriti individual dealing with a Pitta-dominant Vikriti (imbalance) manifesting in the skin (Rakta and Mededa Dhatu dusti), alongside a underlying Kapha-dominant metabolic tendency toward weight gain and Manda Agni (sluggish metabolism, indicated by the white coating on the tongue). ••From an Ayurvedic perspective, her acne can be classified as Mukhadushika (Yauvanpidika), driven primarily by Pitta (inflammation, redness) and Kapha (pus formation, sebum), which has been aggravated by the sudden cessation of topical retinoids and potential chemical irritation from new skincare products. ••Clinical Analysis (Samprapti Ghataka) ••Dosha: Pitta (Bhrajaka Pitta in skin, Pachaka Pitta in GI tract) is acutely aggravated, causing the red, painful boils. Kapha is chronically dominant, causing the pus heads and the tendency toward easy weight gain. ••Dushya (Affected Tissues): Rasa (plasma), Rakta (blood), and Meda (fat tissue). ••Agni (Digestive Fire): She has a state of Manda-Agni mixed with Tikshna-Agni (Visham/Tikshna tendency previously seen as regurgitation/throat burn, but currently manifesting as a white coated tongue indicating Ama—undigested toxic waste). ••Srotas (Channels): Raktavaha and Medovaha Srotas are mildly obstructed (Sanga). ••Dietary Management (Ahara) Her proposed shift to a diet centered around barley (Yava), quinoa, millets, vegetables, mung dal, and buttermilk is excellent and aligns perfectly with a Kapha-Pitta pacifying regime. Modifications & Specific Recommendations: ••Barley (Yava): This is the gold standard for her. It is Ruksha (dry) and Lekhana (scraping), which directly addresses both weight reduction and dries up excess sebum/pus in acne. ••Buttermilk (Takra): Ensure the buttermilk is fresh, churned, low-fat, and not sour. Sour buttermilk will aggravate Pitta and worsen acne. Infuse it with roasted cumin powder (Jeera) and a pinch of dry ginger (Sunthi) to digest the Ama on her tongue. ••Vegetables: Focus on bitter (Tikta) and astringent (Kashaya) vegetables which purify the blood and calm Pitta. Pointed gourd (Parwal), bitter gourd (Karela), bottle gourd (Lauki), zucchini, and leafy greens are ideal. Avoid nightshades (tomatoes, brinjals) and excess potatoes. ••The Iron Supplement Connection: Iron supplements can sometimes cause low-grade Agni Mandya (constipation or sluggish digestion). She should take her iron supplement with a little warm water or a squeeze of lemon to aid absorption without dampening her digestive fire. 3. Lifestyle & Metabolic Regulation (Vihara) Exercise for Weight Loss: Since she notes she needs above-moderate intensity to lose weight, she should engage in vigorous exercise (Vyayama) up to her half-capacity (Ardhashakti—until she sweats from the forehead and armpits). Early morning is the best time (Kapha kala). ••Skin Care - “Less is More”: Her skin is currently Sukumara (highly sensitive) and inflamed. Strictly stop all active chemical products meant for “skin glow” or harsh anti-acne acids. Wash the face with plain water or a mild, cooling herbal wash like Mung dal flour mixed with a pinch of Neem powder. Avoid rubbing or scrubbing, which will worsen acne scars and spread the infection. 4. Suggested Ayurvedic Herbal Protocol (Chikitsa) ••Note: These are classical formulations targeted at purifying the blood, scraping fat, and clearing the skin from within. Internal Medications (Abhyantara Chikitsa) To Clear Ama and Correct Agni (First 2-4 weeks): •Panchakola Choorna or Trikatu Choorna: Just a tiny pinch (250mg) with warm water before meals to clear the white coating on the tongue and boost metabolism. •Blood Purifiers & Anti-inflammatory (For Acne & Scars): ••Kaishore Guggulu: 2 tablets twice a day after meals. It is specific for Pitta-Kapha disorders, clears blood impurities, and helps heal deep-seated skin tissue. ••Mahamanjisthadi Kwath or Khadirarishta: 15-20ml with an equal quantity of lukewarm water, twice a day after meals. Manjistha and Khadiara are the ultimate Raktashodhaka (blood purifiers) in Ayurveda. For Weight Loss (Medohar): ••Triphala Guggulu or Navaka Guggulu: 2 tablets twice a day. This will assist the barley-based diet in scraping (Lekhana) excess Meda (fat) and toning the digestive tract. External Applications (Lepa) Because her skin is sensitive, external applications must be soothing, not heating. ••Lepa for Active Acne: Mix Rakta Chandana (Red Sandalwood) powder, Lodhra powder, and a pinch of Neem powder with pure rose water. Apply as a thin paste over the affected central cheeks and chin. Wash off gently with lukewarm water before it completely dries out and cracks. ••For Acne Scars (Post-Inflammatory Phase): Once the active white pus heads subside, Kumkumadi Tailam (just 2-3 drops) can be massaged very gently at night, only if her skin does not feel excessively oily. Alternatively, a paste of Yashtimadhu (Licorice) and milk/rose water works wonders for hyperpigmentation without clogging pores.

Thanks. She also has tendency for congestion in nasal area in Kapha season. Will Triphala guggulu / Trikatu also reset that tendency. Is it safer than Medohar vati or Medohar guggulu I receved recommendations to use Giloy tablets for her to cool the body. Is it safe to take if she may have a higher thyroid antibody count ( although TSH is ok) . Is Guduchi Sattwa better - and what brand and dose and timing to adopt? Or neem tablets are ok ? I am also confused about how to cool the pitta without aggravating the kapha ( mucus and weight gain)

For your thyroid condition, you may take Kanchanar Guggulu; this would be the best option for you. To address cold-related issues, you can use Trikatu, which will prove beneficial. To pacify Pitta, you may use Chandanasava; this will not aggravate your cough while simultaneously reducing your Pitta levels. Take Kanchanar Guggulu in a dosage of 1-0-1 (one tablet in the morning and one in the evening) with plain water after meals. Prepare a decoction (Kwath) using Trikatu powder before consuming it. Finally, take 15 ml of Chandanasava twice daily (BD)—mixed with an equal amount of water—after meals.

Your acne appears to be related to a combination of Pitta and Kapha aggravation. Pitta causes the redness, inflammation, and pus-filled pimples, while Kapha contributes to blockage of the skin pores and recurrent breakouts. The white coating on your tongue suggests a mild accumulation of -----Ama (metabolic toxins) due to incomplete digestion and metabolism. Your tendency to gain weight easily also indicates a Kapha predominance. —Frequent changes in skincare products may have irritated your sensitive skin and disturbed its natural balance. The goal of treatment is not only to dry the pimples but also to improve digestion and metabolism, reduce excess Kapha and Pitta, purify the blood, and support healthy skin from within. With proper diet, regular exercise, and consistent treatment, both acne and weight management should gradually improve. ----Internal Medicines 1) Mahamanjishthadi Kwath 15 ml + 15 ml warm water twice daily before food. 2) Gandhak Rasayan 2 tablets twice daily after food. 3) Triphala Guggulu 2 tablets twice daily after food. 4) Khadirarishta 15 ml + equal water twice daily after meals. ----Diet Advice Prefer barley, millet, quinoa, mung dal, cooked vegetables, and buttermilk. Drink warm water throughout the day. Avoid fried foods, bakery items, excess sugar, chocolates, cheese, junk food, and cold drinks. Avoid frequent experimentation with cosmetic and skincare products. ----Lifestyle Brisk walking/exercise 45–60 minutes daily. Regular sleep before 10:30 PM. Avoid day sleep. Practice yoga and pranayama regularly. ----Panchakarma Plan -Poorva Karma: 1) Deepana-Pachana as required based on Ama status. -Shodhana: 2) Virechana Karma (preferred for Kapha-Pitta acne and skin disorders). 3) Followed by Raktamokshana (Jalaukavacharana) in selected persistent inflammatory acne cases. 4) Udvartana may be added for Kapha reduction and weight management.

●Hello! It is wonderful that she is taking a proactive, holistic approach to her health. In Ayurveda, skin breakouts and a tendency to gain weight easily are often linked to an imbalance in the Pitta (heat) and Kapha (water/earth) doshas, alongside a buildup of Ama (metabolic toxins). The white coating on her tongue is a classic sign of this residual Ama. Her proposed diet of barley, quinoa, millets, mung dal, and buttermilk is absolutely excellent, as these foods are naturally light, scraping (Lekhana), and cooling, which will simultaneously pacify the skin’s redness and stimulate her metabolism. ●To address the active acne, redness, and scarring, she can directly implement the traditional herbal face mask provided. Mixing Multani mitti with Neem, Manjistha, Mulethi, Sandalwood, Turmeric, and a tiny pinch of Borosilicate ash (Borax Bhasma) with rose water creates a powerful topical treatment. The Neem and Borax work to dry up the active white pus heads and control excess oil, while Manjistha and Sandalwood specifically target the bright red inflammation and help fade old acne scars. Because her skin is sensitive and currently reacting to a pause in her tretinoin cream, she should apply this as a thin layer only 2–3 times a week, washing it off gently with lukewarm water before it gets excessively dry. ●Since true skin health starts from within, she can use the recommended internal Ayurvedic cleansers to clear out the internal heat (Pitta) that is causing the cheek and chin breakouts. Taking 1/4 teaspoon of Triphala powder with fennel water at night (or on an empty stomach in the morning) will gently cleanse the digestive tract and cool the body. Additionally, a daily combination of Triphala and Giloy powder mixed with lukewarm water will help eliminate the Ama (reflected in her white-coated tongue), purify her blood, and boost her immunity, which prevents new breakouts from forming while supporting healthy digestion. ●For her second goal of shedding about 7 pounds and managing her stubborn metabolism, the provided traditional remedies offer excellent support. She can try the radish seed remedy by mixing 3 to 5 grams of radish seed powder with a teaspoon of honey twice a day, followed by lukewarm lemon water, which traditionally helps target stubborn fat. Furthermore, adding small amounts of black and white pepper to her meals, or taking turmeric and black pepper with warm water, acts as a natural metabolic booster to help her body burn fat more efficiently during her high-intensity workouts. ●To give her weight loss goals a structured boost, she can safely introduce the specific herbal supplements mentioned in the protocol. She can take 1 teaspoon of Stholyantak Churna twice daily after meals with warm water (or prepare it as a boiled decoction). Alongside this, she can take 1 capsule of Trim Support and 2 capsules of Garcinia half an hour after her meals with warm water. These herbs work synergistically to suppress emotional food cravings, optimize fat metabolism, and stop the body from easily storing excess carbohydrates as fat. ●Finally, she should strictly pair these remedies with the recommended dietary discipline to see lasting results. She must avoid high-carbohydrate and heavy foods like white rice, potatoes, sweets, fried items, and junk food, while opting for her chosen whole grains like barley and millets. For protein, she should swap out heavy meats for mung beans, nuts, and fish, and stick to low-fat dairy like cottage cheese and plain yogurt or buttermilk. Combined with her regular higher-intensity workouts, drinking plenty of water, and avoiding touching or squeezing her pimples, this holistic routine will help her calm her skin and smoothly shed those 7 pounds.

Her acne appears to be a combination of Pitta-Kapha aggravation with mild Ama accumulation, especially since there is facial redness, pustules, sensitive skin, and a coated tongue. The planned diet is appropriate and should support both skin health and gradual weight reduction. For now, avoid frequent changes in skincare products. Keep the routine simple and gentle. Reduce sugar, excess dairy, fried foods, protein powders, and late-night eating. Kaishore Guggulu – 1 tablet twice daily after food Mahamanjishtadi Kwath – 15 ml with equal water twice daily after food Avipattikar tablet – 1 tablet at bedtime if needed for Pitta balance Kumkumadi tailam – 2-3 drops externally at night for acne marks (avoid active inflamed lesions) For weight management: Warm water through the day 30–45 minutes of brisk walking or exercise at least 5 days/week Barley, millet, quinoa, mung dal, vegetables, and buttermilk are good choices With consistent treatment, improvement in active acne is usually seen within 6–8 weeks, while acne marks may take longer. Regards, Dr Raghuveer (Ayurvedacharya)
