Short answer: your report is essentially normal — the AST:ALT ratio of 1.21 is not a problem in your case. Why your report is okay: AST (23) and ALT (19) are both well within normal limits Ratio slightly >1 is only meaningful when values are elevated Your GGT, bilirubin, ALP, proteins → all normal 👉 This means no active liver damage right now Important point (this matters more): You mentioned daily alcohol intake 👉 In such cases, even a normal report today does NOT mean future safety A ratio >1 can be seen in alcohol-related patterns, but: ✔ Yours is mild and enzymes are normal → no current damage ❗ But continued alcohol can gradually change this What you should do: 1. Alcohol (most important step): Reduce gradually → ideally stop or keep occasional (This alone will protect your liver more than any medicine) 2. Diet: Avoid fried, processed, excess sugar Add: Fruits, vegetables, हल्का भोजन Adequate hydration 3. Ayurvedic liver support (optional): Bhumi amla juice/powder – morning Kalmegh – for liver detox support Milk thistle (if already considering, can be used as support) 4. Uric acid (borderline high): Reduce red meat, alcohol, sugary drinks Increase water intake 5. Follow-up: Repeat LFT after 3–4 months (especially if you reduce alcohol) When to worry: If future tests show: AST/ALT rising GGT increasing Symptoms like fatigue, heaviness, jaundice Bottom line: ✔ Your current liver function is healthy ✔ AST:ALT ratio alone is not concerning here ❗ Alcohol is the only real risk factor going forward If you control that → you likely won’t face liver issues. Hello, It is reassuring to see that your absolute values for Total Protein, Albumin, and Globulin are within the normal range. In Ayurveda, a slight elevation in the AST:ALT ratio is often viewed as a Pitta-related imbalance in the Yakrit (liver), suggesting that while there is no structural damage, the metabolic “fire” or Ranjaka Pitta needs cooling and stabilization. In Ayurveda, the liver is the seat of Pitta. A slightly altered ratio suggests an accumulation of “Amavisha” (metabolic toxins) or excessive heat affecting the liver cells. The goal is to perform Yakrit Uttejana (liver stimulation) and Pitta Shamana (pacifying the heat) to restore balance. Prescription : 1. Patolakaturohyadi Kwath This is a classical Kerala Ayurvedic decoction specifically used for Pitta-related liver disorders and skin/blood purification. Dose: 15 ml of the liquid mixed with 45 ml of lukewarm water, twice daily on an empty stomach (morning and evening). 2. Avipathi Churnam Since a high ratio can sometimes be linked to heat and acidity, this powder helps in mild purgation (Virechana) to flush out excess Pitta. Dose: 1 teaspoon with warm water at bedtime, twice a week only. Excessively spicy, sour, or fermented foods (pickles, curd, vinegar). Incorporat Cooling foods like Bottle Gourd (Lauki), Pomegranate, and Coconut water.

Your reports overall look normal, with only a slightly elevated AST:ALT ratio (1.21) and a note of daily heavy alcohol use. In Ayurveda, this situation is usually linked to Pitta aggravation and early Yakrit (liver) stress. RX- Tab kutki (Picrorhiza kurroa) – 250–500 mg twice daily before meals → Strong liver detox & improves enzyme balance Liv-52 – 1 tsp twice daily after meal. Guduchi (Giloy) juice daily. → Improves immunity + reduces Pitta Tab Bhumyamalaki (Phyllanthus niruri) – 500 mg twice daily after meal. → Excellent for liver repair and enzyme normalization. Avoid Heavy meals. Heavy drinking alcohol Try to reduce drinking alcohol day by day. Lifestyle changes Daily yoga and anulom vilom Daily walking after meal. Drink more and more water to avoid dehydration. 👉 No Medicines will protect your liver if heavy daily alcohol continues. Reducing or stopping alcohol is the most important “medicine” here. The lab reports you provided, shows liver and kidney function parameters within the normal reference ranges. The minor elevation in the AST:ALT ratio (1.21) and Uric Acid (6.70 mg/dL) are seen but are currently near the upper limit of normal rather than clinically pathological. This indicates us a need to focus on to maintain Pitta and Kapha balance to ensure continued liver (Yakrut) health and to prevent the accumulation of metabolic waste (Ama). 👇👇👇👇👇👇👇👇👇👇👇 1) Patolakaturohinyadi Kwath- 15 ml with 45 ml lukewarm water Twice daily, 30 mins before food 2) Livact Tablet- 1 tablet Twice dally, after food 3) Punarnavadi Kwath- 15 ml with 45 ml lukewarm water Twice dally, 30 mins before food 4) Avipathi Churnam- 1 tsp Once daily at bedtime with warm water 👇👇👇👇👇👇👇👇👇👇👇👇 Include vegetables like bottle gourd (Lauki), bitter gourd (Karela), and pointed gourd (Parwal), Increase Intake of mung dal and fiber-rich grains, Avoid: High-protein red meats (if non-vegetarian), excessive salt, refined flour (Malda), and fermented foods (Curd, Idil, Dosa) Drink 2.5-3 liters of water daily, Warm water bolled with Coriander seeds Alcohol and smoking 30 minutes of brisk walking or Yogasana (specifically Mandukasana and Ardha Matsyendrasana) Sleep: Ensure 7-8 hours of sound sleep. Hello, While these laboratory results appear reassuring, it is important to address the underlying impact of regular alcohol consumption. While your body has demonstrated significant resilience thus far, it should be understood not to test the limits of that biological capacity. Chronic alcohol intake creates a systemic burden that these current markers—which remain within normal ranges—do not fully represent. Of particular concern is the depletion of Vitamin B6 within the liver. This nutrient is essential for the proper production and release of liver enzymes; its deficiency can lead to laboratory results that appear normal, masking underlying stress or pathology. Furthermore, alcohol induces specific mitochondrial damage in the liver. Because AST is predominantly derived from these mitochondria, any observed elevation in the AST:ALT ratio serves as a sensitive indicator of this specific type of cellular stress. While the GGT and ALP levels are currently within reference ranges, relying on these luck factors is not a sustainable strategy for long-term health. Please understand that therapeutic interventions, including Ayurvedic hepatoprotective protocols, cannot achieve their intended results if the liver is simultaneously subjected to the toxic load of alcohol. The efficacy of any treatment is directly dependent on the removal of the primary causative factor. If you are serious about restoring your health and liver function, a complete cessation of alcohol is non-negotiable. Please consider these medications only if you are fully committed to this lifestyle change: Patolakaturohinyadi Kashayam: 15 ml, diluted with 60 ml of warm water, to be taken twice daily before food. Heposem Tablet: 1 tablet, twice daily, to be taken after food. This regimen is intended to support the gradual recovery of your digestive and hepatic environment. Please plan for a follow-up consultation in one month to reassess your progress and the clinical response to this protocol.

Alcohol depletes Vitamin B6, which is essential for liver enzyme regulation. -Manage Uric Acid: Your levels are at the upper limit (6.70 mg/dL). Increase water intake to at least 3 liters daily and avoid high-purine foods like red meat and sugary drinks. -Dietary “Cooling”: Incorporate Pitta-pacifying foods like Bottle Gourd (Lauki), Pomegranate, and Coconut Water. Avoid spicy, fermented (pickles, vinegar), and fried foods. -Morning Detox: Start your day with warm water and 5–7 drops of lemon to stimulate Agni (digestive fire).

You have absolutely nothing to worry about. Your liver and kidneys are in excellent, stable health. No medical intervention or specific treatment is required for your liver. It is very common to see an AST:ALT ratio slightly above 1.00 and feel concerned, but this ratio is only clinically significant when your actual AST and ALT numbers are elevated (which usually points to liver inflammation or alcohol-related liver stress). Because your AST (23 U/L) and ALT (19 U/L) are both low and perfectly within the safe, normal range, the ratio of 1.21 is mathematically expected and medically irrelevant. AST is not just found in the liver; it is also present in your muscles and red blood cells. Normal daily activities or mild exercise can cause AST to naturally sit slightly higher than ALT in a perfectly healthy person. Clinical Observations & Preventive Care While your liver and kidney functions are optimal, there is one minor metabolic marker to keep an eye on: Uric Acid (6.70 mg/dL): This is at the higher end of the normal range. In Ayurveda, creeping uric acid indicates a mild, early accumulation of metabolic toxins (Ama) in the blood (Rakta), which can eventually lead to joint stiffness if left unchecked. ✓Lifestyle Recommendations Since your overall lab profile is fantastic, you only need minor dietary maintenance to keep that uric acid from crossing the upper limit: Hydration: Ensure you are drinking plenty of warm water throughout the day. This acts as a natural diuretic to help your perfectly functioning kidneys flush out the excess uric acid. Moderate Purines: Keep an eye on heavy, purine-rich foods like red meat, excessive lentils, or processed foods, ensuring your diet remains balanced and easy to digest. Keep up your current lifestyle, as it is clearly working well for your internal organs. Regards Dr Gursimran Jeet Singh MD Panchakarma
