Hello
This is a very serious and understandable concern, and Iwill answer you honestly, carefully, and without exaggeration.
DO THIS SYMPTOMS MEAN END STAGE PKD?
The symptoms you listed:
–Severe itching
–Bleeding tendency
–High blood pressure
–Nausea
–Foul / ammonia-like belching
👉 Yes — taken together, these symptoms are commonly seen in advanced (late-stage) chronic kidney disease, including end-stage kidney disease (ESKD) in people with polycystic kidney disease (PKD).
However, symptoms alone cannot confirm the exact stage.
What these symptoms usually indicate
They strongly suggest:
–Severe uremia (toxic waste buildup)
–Very poor kidney filtration
–Possible Stage 4–5 CKD
–Often seen when eGFR is very low (<15)
These symptoms usually appear when kidneys are no longer able to clear toxins adequately.
IS THIS AUTOMATICALLY LAST STAGE?
Not always — but it is very concerning.
To confirm end stage, doctors look at:
–eGFR
–Creatinine & urea
–Potassium
–Hemoglobin
–Imaging (kidney size & cyst burden)
That said:
When uremic symptoms like itching + nausea + bleeding appear together, most patients are already near or at dialysis-level kidney failure.
WHAT IF SOMEONE REFUSES DIALYSIS OR TRANSPLANT??
I’ll answer this clearly but compassionately.
In general (medical averages, not exact predictions):
Without dialysis or transplant, once true end-stage kidney failure develops:
–Life expectancy is often weeks to months
–Occasionally a few months to a year, depending on:
–Remaining kidney function
–Heart health
–Blood pressure control
–Infections
–Diet & fluid intake
Once severe uremic symptoms are persistent, survival without renal replacement therapy is limited.
WHY SYMPTOMS WORSEN WITHOUT DIALYSIS
When kidneys fail:
Toxins build up → itching, nausea, mental changes
Platelet dysfunction → bleeding
Fluid & hormones imbalance → severe BP
Acid buildup → air hunger, foul belching
Eventually → heart rhythm problems, coma
Dialysis does not cure PKD, but it removes toxins that keep a person alive.
IS CONSERVATIVE CARE EVER CHOOSEN?
Yes — especially if:
The person is elderly
Has severe heart disease or cancer
Chooses comfort-focused care
In that case:
Care shifts to palliative / supportive management
Focus is on:
Comfort
Itch control
Nausea relief
Pain & anxiety reduction
This should be done under medical supervision, not alone.
THIS SHOULD BE DONE URGENTLY
They need:
–Immediate nephrologist evaluation
–Blood tests (urgent)
–Honest discussion about:
–Dialysis options (including gentle or home dialysis)
–Conservative care if dialysis is declined
–Symptom control
Yes, those symptoms are very consistent with advanced / possibly end-stage PKD
Without dialysis or transplant:
Survival is usually limited to months, sometimes shorter
Exact life expectancy cannot be predicted without labs
Medical support is essential — even if dialysis is refused
