I was diagnosed as autoimmune inflammatory arthritis with Hlab27 positive and pain and stiffness at sacroilitis taking timely treatments and managing my pain & stiffness through Ayurveda also kept the inflammation under control so recently decided to improve some physical activity so started playing badminton after a long gap after finishing the activity while drinking water I noticed my neck range became limited and it was stiff and painful for 2 days then I visited a physio to get some relief there and doctor did x-ray of a cervical he told your stiffness is caused by the damage to the cartilage and osteophytes are forming and the bones are fusing together so he advised enough with your Ayurveda treatments will start the biologics and arrest these progression and stop the damage. I strongly believing in Ayurveda treatments and medicines than the immuno suppressants drugs looking for an answer from an ayurvedic doctor regarding this situation how to arrest this silent progression of the disease can we do it through Ayurveda treatments or not and can we improve the cartilage between the bones and increase the range of the neck by following Ayurveda or should I need to go for that immunosupressants allopathy medicine thank you for answering my question as it is more valuable to me kindly appreciated.

Autoimmune inflammatory arthritis with Hlab27 - positive involving sacroiliac joints and spine is understood in Ayurveda as a chronic vata dominant disorder with deep seated ama and immune dysregulation affecting asthi and majja dhatu This type of condition does not behave like a simple mechanical neck problem and it does not progress in a linear or predictable way Periods of stability are often followed by flare ups especially when physical strain exposure to cold irregular routine or sudden unaccustomed activity is introduced

In your case the increase in physical activity after a long gap especially a sport like badminton which involves sudden neck rotation shoulder strain and spinal loading has acted as a trigger for vata aggravation The acute neck stiffness and reduced range of movement appearing immediately after the activity indicates muscular spasm ligamentous strain and inflammatory response rather than sudden structural fusion Radiological findings such as cartilage thinning early osteophyte formation or reduced disc space reflect a chronic process that develops over years and not within days X ray changes must always be correlated with symptoms clinical findings and inflammatory markers before making conclusions about irreversible fusion.

From the Ayurvedic view the primary pathology is not bone rubbing bone ,but dryness,inflammation, loss of lubrication ,obstruction of channels and hyper reactive immune response.

When vata is aggravated in the presence of ama it leads to stiffness pain reduced mobility and cracking sensations particularly in the spine and neck.

Ayurveda approaches this condition with three parallel goals First to reduce ama and inflammatory load Second to pacify vata and stabilise immune activity Third to nourish and protect asthi and majja dhatu and preserve joint function.

Arresting progression does not mean reversing X ray findings It means reducing disease activity preventing further damage preserving mobility and improving quality of life over long term when treatment is sustained disciplined and individualised progression slows significantly and flares reduce in frequency and intensity.

Regarding cartilage and disc health Ayurveda works through improving tissue nutrition circulation and lubrication Sneha chikitsa rasayana therapy and vata shamana measures help maintain remaining cartilage and prevent rapid degeneration Regeneration in Ayurveda is gradual and depends on age agni digestive capacity disease chronicity and consistency of treatment

Basti therapy plays a central role in spinal inflammatory disorders Classical basti using medicated oils and decoctions helps regulate vata at its root site colon and influences the entire musculoskeletal and nervous system Patients with spondyloarthropathies often show better pain control flexibility and functional improvement after proper basti courses

Internal medicines based on your current dosha status-

Mahayogaraja guggulu one tablet twice daily after food with warm water Kaishora guggulu one tablet twice daily after food Dashamoola kwatha fifteen to twenty millilitres once or twice daily diluted with warm water

Guduchi satva 500 mg twice daily before food Ashwagandha churna half teaspoon twice daily with warm milk Shallaki or Boswellia one capsule twice daily after food

These medicines reduce inflammation modulate immune response strengthen muscles support joints and stabilise vata

External therapies like

Regular abhyanga with warm mahanarayana taila or sahacharadi taila helps reduce stiffness improve circulation and protect joints

Local neck therapies like greeva basti when done provide excellent relief and improve range of motion over time.

Avoid sudden high impact activities jerky movements competitive sports and unprepared strain Cold exposure ,irregular meals,fasting ,excessive travel and stress ,aggravate vata. Daily routine should be stable predictable and supportive of digestion

Diet should be warm freshly cooked mildly spiced and easy to digest Avoid curd at night ,fermented foods ,refined sugar ,excess sour,deep fried foods and cold drinks. Include ghee cooked vegetables rice soups mung dal and adequate protein.

Yogasanas such as bhujangasana marjariasana shashankasana and setu bandhasana done slowly are beneficial. All movements should be within pain free range. Pranayama like anuloma viloma and bhramari help regulate nervous system and immune response

Coming to the question of immunosuppressive biologics From an Ayurvedic view suppression of immune response without correcting agni ama and vata imbalance does not address root pathology. Ayurveda also recognises the need for integration in certain advanced or aggressive cases. The decision to start biologics should be based on objective markers continuous disease activity neurological compromise or failure to respond to comprehensive conservative management

If your inflammation markers are controlled pain is manageable, and function is improving ,then there is space to continue with structured Ayurvedic care under monitoring.

Your condition requires long term planning. Consistency ,awareness and correct therapies determine outcomes far more than fear based decisions.
