Managing Persistent Rectal Discomfort and Bowel Disruption - #48487

Aaron Meyler

I am sorry for the rather crude and embarrassing question, I am regular paying client here but I do have a quick question. After long standing bowel disruption from catheter and antibiotics I was just getting myself right when I got hit my a nasty uti in late November, put on more antibiotics and since my bowel issues have got worse I have been able to. Control the constipation but now I have developed a feeling on incomplete evacuation, but the hardest thing is a lingering rectal awareness, it's not sore but it's uncomfortable it's doesn't really feel like stool is sitting there but it's a non stop sensation that drives me mad I don't know if my use of magnesium oxide is driving my rectum or colon out or something , I have some. Small success taking ayhaberista with warm water it seems to help the discomfort but I am wondering if I should add kumarasava aswell to try and keep it settled through the day. I have reduced my. Magnesium oxide by half from 1000 mg to 500 mg nightly but plan to reduce it further to around 375mg but first I need to get this sensation irritation sorted out any suggestions would be warmly appreciated Thank you for all of valued advice so far and I plan to. Come back for further paid consults soon. Kind regards Aaron

Doctors' responses

I UNDERSTAND WHAT YOU ARE EXPERIENCING AND PLEASE DO NOT FEEL EMBARRASSED THIS TYPE OF RECTAL AWARENESS AND FEELING OF INCOMPLETE EVACUATION IS VERY COMMON AFTER PROLONGED ANTIBIOTIC USE CATHETER RELATED BOWEL DISTURBANCE AND STRESS EVEN WHEN STOOLS ARE PASSING DAILY

FROM AN AYURVEDIC VIEW THIS CONDITION OCCURS DUE TO APANA VATA DUSHTI WITH LOCAL PITTA IRRITATION IN THE RECTUM AND LOWER COLON ANTIBIOTICS DISTUR THE GUT FLORA DRY THE INTESTINES AND MAKE THE RECTAL NERVES OVER SENSITIVE SO EVEN WITHOUT STOOL PRESENT THE RECTUM CONTINUES TO SEND FALSE SIGNALS OF FULLNESS THIS IS WHY YOU FEEL CONSTANT DISCOMFORT WITHOUT TRUE PAIN

MAGNESIUM OXIDE IN HIGHER DOSES CAN DEFINITELY AGGRAVATE THIS CONDITION BECAUSE IT PULLS WATER INTO THE BOWEL AND OVERSTIMULATES RECTAL SENSATION YOUR DECISION TO REDUCE FROM ONE THOUSAND MG TO FIVE HUNDRED MG IS CORRECT AND EVENTUALLY IT SHOULD BE BROUGHT DOWN FURTHER BUT FIRST THE RECTAL IRRITATION MUST SETTLE

THE FACT THAT ABHAYA ARISTHA IS GIVING YOU RELIEF IS VERY IMPORTANT IT MEANS YOUR APANA VATA IS RESPONDING WELL AND THE BOWEL IS REGULATING NATURALLY

I DO NOT RECOMMEND ADDING KUMARASAVA AT THIS STAGE BECAUSE IT HAS USHNA GUNA AND MAY INCREASE RECTAL IRRITATION IN A SENSITIVE GUT

CONTINUE ABHAYA ARISTHA FIFTEEN ML WITH EQUAL WARM WATER AT NIGHT AFTER FOOD

ADD KAMDUDHA RAS ONE TABLET TWICE DAILY AFTER FOOD TO SOOTHE RECTAL PITTA AND NERVE IRRITATION

ADD TRIPHALA CHURNA HALF TEASPOON AT BEDTIME WITH LUKEWARM WATER THIS WILL HELP COMPLETE EVACUATION WITHOUT FORCING THE RECTUM

IF RECTAL AWARENESS IS VERY DISTURBING ADD JATIPHALADI CHURNA ONE FOURTH TEASPOON AT NIGHT FOR A SHORT PERIOD IT CALMS RECTAL NERVE HYPERSENSITIVITY AND CONTROLS FALSE URGE

DIET SHOULD BE SOFT WARM AND OILY AVOID RAW FOODS FERMENTED ITEMS COFFEE AND SPICY FOODS DO NOT STRAIN DO NOT SIT LONG ON THE TOILET

INVESTIGATIONS ADVISED INCLUDE STOOL ROUTINE AND MICROSCOPY TO RULE OUT POST ANTIBIOTIC DYSBIOSIS PROCTITIS OR EXCESS MUCUS SERUM VITAMIN B12 AND D LEVELS BECAUSE DEFICIENCY CAN WORSEN NERVE SENSITIVITY AND IF SYMPTOMS PERSIST A PROCTOSCOPY MAY BE NEEDED TO RULE OUT LOCAL IRRITATION OR PILES

THIS CONDITION IS REVERSIBLE WITH PROPER VATA PITTA BALANCING AND DOES NOT INDICATE ANY SERIOUS DISEASE PLEASE CONTINUE GRADUAL REDUCTION OF MAGNESIUM ONLY AFTER SYMPTOMS STABILISE AND YOU SHOULD FEEL STEADY IMPROVEMENT OVER THE NEXT FEW WEEKS
