My right kidney is boarder line small with mild hydronephrosis.The parenchyma is normal.The left is small, moderate hydronephrosis,normal parenchyma

The condition you describe is mostly due to the chronic obstructive or inflammatory condition this is pointing towards the mutra Vag srotasa vikar Which is due to vata and Kapha imbalance lead to obstruction So we will focus on supportive care to reduce inflammation improving renal drainage and support your tissue but you have to clearly monitor and ensure regular kidney function test monitor blood pressure regularly And after this if you feel your problem does not subside please visit to your nearest nephrologist or Urologist for further investigation 1) Punarnavadi Kwath- 15 ml with 45 ml lukewarm water twice daily 30 minutes before food 2) Chandraprabha Vati- two tablet twice daily after food 3) Gokshuradi Guggulu- two tablet twice daily after food 4) Varanadi Kwath- 15 ml with 45 ml lukewarm water twice daily 30 minutes before food 5) Purnarnavasava- 20 ml with 20 ml water twice daily after food Increase your water intake to at least 2-2.5 litte Per day coconut water will be highly beneficial Avoid high protein diet heavy processor food high sodium snacks

First and most important step: You need urgent evaluation by a urologist. Tests like repeat ultrasound / CT and urine test are required to find the cause (stone, blockage, narrowing, etc.). If obstruction is present, it may need a procedure. Medicines alone cannot remove blockage. Do not delay this. Now what you should do immediately: Increase water intake gradually. Start with small amounts and reach around 2–2.5 liters daily unless your doctor has restricted fluids. Do not hold urine. Pass urine regularly. Avoid painkillers unless prescribed, as they can damage kidneys. Diet should be light and supportive. Take lauki, tori, coconut water if suitable. Avoid excess salt, spicy, fried food. Medicines (supportive along with medical care): Chandraprabha vati 1 tablet twice daily after food. Gokshuradi guggulu 2 tablets twice daily after food. Punarnava mandoor 1 tablet twice daily after food. Varunadi kwath 15 ml with equal water twice daily after food. These help improve urine flow, reduce swelling, and support kidney function, but they are not a substitute for treating the root cause. Because you also have fatigue, it may indicate ongoing stress on kidneys, so monitoring is important. If you notice reduced urine, increased pain, fever, or more color change in urine, go for immediate medical care.

When combined with “small” kidney size, it suggests a chronic process that needs careful management to preserve the remaining normal parenchyma (functional tissue). ​In Ayurveda, this is often viewed as Vrukka Shotha (renal swelling) caused by an imbalance in Vata and Kapha obstructing the Mutravaha Srotas (urinary channels). ​Ayurvedic Perspective & Management ​The goal is to clear the obstruction (Srotovishodhana), reduce swelling (Shothahara), and provide strength to the renal tissue (Rasayana). ​Right Kidney (Mild): Focus is on maintaining flow and preventing further pressure. ​Left Kidney (Moderate/Small): Focus is on aggressive drainage and supporting the diminished kidney structure. ​1. Punarnavadi Kwath (Tablet or Liquid) This is the primary choice for reducing kidney swelling and improving drainage. It acts as a potent diuretic and anti-inflammatory. ​Dose: 15 ml liquid mixed with 45 ml lukewarm water, or 2 tablets. Take twice daily, 30 minutes before food. ​2. Chandraprabha Vati A versatile formulation that clears urinary obstructions, balances Vata, and helps manage the “borderline small” aspect by improving overall kidney health. ​Dose: 2 tablets twice daily after food with lukewarm water. ​3. Varunadi Kwath Specifically targeted at clearing obstructions (like small stones or sludge) that often cause hydronephrosis. It helps “scour” the channels. ​Dose: 15 ml liquid mixed with 45 ml lukewarm water twice daily before food (can be taken alongside Punarnavadi Kwath). ​4.
