Doctors' responses

1️⃣ Can you take Pankajakasthuri Breatheasy Syrup with Apixaban? Short answer: ⚠️ Not without your doctor’s approval. Why? Apixaban is a blood thinner (anticoagulant). Many herbal syrups contain ingredients that may: Increase bleeding risk Affect liver enzymes (CYP3A4 / P-gp) Interfere with how Apixaban is metabolized Even natural/herbal medicines can increase bleeding risk when combined with anticoagulants. 2️⃣ What’s the main risk? With Apixaban, the biggest danger is: 🔴 Internal bleeding Nosebleeds Gum bleeding Black stools Vomiting blood Brain bleeding (rare but serious) Some herbal ingredients like: Vasaka (Adhatoda) Ginger Turmeric Licorice Certain essential oils can mildly affect clotting or liver metabolism. Even if the risk is small, because your PE was unprovoked, your clot risk is already high — so we must be cautious. 3️⃣ Since your respiratory health is good (only occasional issues) If it’s just: Mild cold Occasional cough Seasonal irritation 👉 It’s safer to try: Steam inhalation Saline gargles Warm water Plain honey (small amount) Avoiding cold triggers Rather than adding herbal syrups. 4️⃣ Very Important Question Are you on Apixaban lifelong or for fixed duration (like 1–2 years)? Unprovoked PE patients are often kept on long-term anticoagulation. 5️⃣ My Practical Advice ✅ Do NOT start Pankajakasthuri on your own ✅ Ask your physician who prescribed Apixaban ✅ If needed, they may check interaction safety If you want, tell me: What other prescription medicines you are taking Why you want Breatheasy (cough? wheeze? allergy?)

Hello Satheesh Thank you for your question. As you are already on anticoagulant like apixaban after pulmonary embolism It is very important to be careful for adding any medications ABOHR PANKAJAKASTHURI BREATH EASY SYRUP It is basically used for Cough Breathing difficulty Allery And milk respiratory problems SAFETY WITH ANTICOAGULANT Apixaban is a strong anticoagulant And combining this with any herbal medication should be done carefully As Some herbs can increase bleeding risk Some may alter drug action
Example- Ginger may increase blood thining effect Therefore what i suggest is Use a small dose for short duration 5ml daily after food once is safe But also monitor for easy bruising or nose or gum bleeding Better than breatheasy syrup you can switch to some simple home remedies 1.WARM WATER Drink. Warm. Water Improves throat health 2.STEAM INHALATION Daily steam inhalation Clear airways 3.TULSI TEA Boil few tulsi leaves Supports lungs DIET ADVICE INCLUDE warm food Light diet Fruits Vegetables ❌AVOID Excess oily food Cold drinks Smoking Warm regards, Dr Snehal Vidhate

HELLO, You’re taking apixaban 2.5 mg twice daily after an unprovoked pulmonary embolism- that’s important, because apixaban is a blood thinner and can interact with certain herbal products REGARDING PANKAJAKASTHURI BREATHEASY SYRUP This is an ayurvedic poly herb formulation typically containing herbs such as -vasaka -tulsi -ginger -long pepper -turmeric -licorice -other herbal extracts SAFETY WITH APIXABAN There is no strong clinical trial data confirming it is safe with apixaban. However, some ingredients raise caution -ginger-> mild blood thinning effect -turmeric-> may increase bleeding risk -tulsi-> possible mild coagulant effect -licorice-> can affect blood pressure and potassium Because you are on apixaban after an unprovoked PE (higher recurrence risk), even small increases in bleeding risk should be taken seriously RISK SUMMARY Combining herbal formulations that have anti platelet or anticoagulant effects with apixaban may -increase risk of nosebleeds -cause easy brusing -increase GI bleeding risk -rarely increase internal bleeding risk Even though your dose (2.5 mg BID) is a lower maintenance dose, interaction risk still exists MY RECCOMENDATIONS -Do not start pankajakasthuri breathes without informing your prescribing doctor -If your respiratory health is gradually good (AS YOU MENTIONED, only occasional issue), it may be safer to –use steam inhalation –use saline nasal spray –use simple honey + warm water unless diabetic –stay hydrated These are safer with anticoagulants Instead of multi herb syrups, consider -plain steam inhalation with ajwain -warm turmeric milk in very small amounts (but avoid high-dose supplements) AVOID -high dose turmeric capsules -ginger extracts -mixed syrups without physician clearance Because you are on apixaban after unprovoked PE, its safer to avoid pankajakasthuri breathes syrup unless physician explicitly approves it DO FOLLOW HOPE THIS MIGHT BE HELPFUL THANK YOU

Since you are on Apixaban (Eliquis) after an unprovoked pulmonary embolism, you must be very careful with any herbal syrup. Pankajakasturi Breatheasy syrup contains multiple herbal ingredients. Some herbs (especially those affecting circulation, inflammation, or liver metabolism) can theoretically: • Alter blood thinning effect • Increase bleeding tendency • Interfere with drug metabolism in the liver With anticoagulants, even mild interactions can be clinically significant. The risk is not always obvious immediately. Because your pulmonary embolism was unprovoked, long-term anticoagulation is usually prescribed for a strong reason. In such cases, safety comes first. I would not recommend starting the syrup on your own without: Informing your prescribing physician Reviewing the full ingredient list carefully Assessing bleeding risk If your respiratory health is generally good and issues are only occasional, it may be safer to use non-systemic measures (steam inhalation, saline gargles, breathing exercises) instead of internal herbal syrups. Given your history, this requires individualized assessment. I suggest a proper consultation where we can review: • Your full medication list • The exact syrup formulation • Your clotting history and current risk profile Only after that can a safe decision be made.
