I am 41 years of age. Recently 2 Months back diagnosed with Type 2 diabetes with HbA1c 13.3%, and FBS 306, high BP with almost 140/90+ Pulse Rate:100-110+. I have visited Endocrinologist and She prescribed 2 Insulin for 3 times a day with Linaray M & diabetic neuropathy (Nerve CD3,Gabaprix NT) & BP medicines (Bylenta 40 M & Met XL) & Vitamin D3 weekly once. Then I tested HbA1c 15-20 days previously and saw a drop of 2.8% in HbA1c from 13.3% to 10.5% and FBS from 306 to 90-110. Then Doctor has reduced from 3 times to currently 1 time and Linaray M medicines twice with other medicines. MY c-peptide 0.07 currently and Now the neuropathy pain on testicle is disappeared and also frequent urination as well. But currently burning sensation in feet is there with little pain and so much irritation there from ankle to feet for which I cant sleep properly with AC ON as there is so much cold sensitivity in my feet and ankle. Along with allopathic medicines and with night time insulin, I have started ayurvedic Madhugrit /madhunashini vati after lunch and chandrapava vati . But I observed that night time frequent urination increased, is it due to madhunashini or chandapava vati. Please suggest me some safe ayuvedic medicines which I can take for diabetic to regenerate dead beta cells and for the burning sensation on feet and ankle and also to reduce diabetic neuropathy safely can be taken along with allopathic medicines. Also currently I am using patanjali mahanarayan oil on feet. Please suggest some better as well Note: Currently taking Linaray M after breakfast & dinner, insulin Ryzodeg at before dinner/Insulin taking. Current sugar readings FBS:90-120 & Before dinner sometiime 110-150 & sometimes 160-200. As sometime Before Dinner/Insulin taking is like 160-200 & doctor did not prescibe any diabetic insulin and medicines after lunch thats why I am taking Madhugrit/Madhunashini vati after lunch. My requests: 1. So suggest me safe ayurvedic medicines for diabetes for post lunch to lower sugar post lunch & snacks & High BP with high pulse rate which can be taken safely with the currently taking allopathic medicines and insulin. 2. Also suggest me safe ayuvedic medicines for regenerating dead beta cells which is not possible in allopathic and I think in Ayurveda medicines and churnas are available? 3. Also suggest ayurvedic medicines for diabetic neuropathy like for burning & cold sensitivity on feet and ankle And feeling little burning sensation on testicle connector (for only 1-2 mins) after passing urine. 4. Also from last 2 years due to High sugar I am having ED & PE problems sometime , so suggest some medicines for the same

Your blood sugar has improved significantly, which is encouraging. In Ayurveda, your condition resembles Madhumeha with Vata-Pitta aggravation, affecting nerves, circulation, sleep, and sexual health. Since your C-peptide is very low, insulin remains very important, and Ayurvedic medicines should be used only as supportive therapy along with your endocrinologist’s treatment. 1. Safe Ayurvedic Support for Diabetes, BP & High Pulse For post-lunch sugar spikes and metabolic support, these are commonly used supportive options: Gudmar (Gymnema), Vijaysar, Jamun seed, Methi based formulations – help improve glucose metabolism naturally. Madhunashini Vati can help, but because you are already on insulin and Linaray M, use cautiously and monitor sugars regularly to avoid hypoglycemia. Chandraprabha Vati may increase urination in some people due to its urinary-cleansing action, especially at night. For mild BP and high pulse support: Sarpagandha Ghana Vati Arjunarishta in small doses after meals may help heart strength, circulation, and pulse balance. 2. For Pancreatic/Beta Cell Support Ayurveda aims more at improving pancreatic function and metabolism rather than guaranteed “beta-cell regeneration.” Supportive herbs: Guduchi (Giloy) Amalaki Turmeric with black pepper Shilajit (purified, physician-guided) Nisha Amalaki These may support insulin sensitivity, inflammation control, and metabolic balance. 3. For Burning Feet, Neuropathy & Cold Sensitivity Your symptoms suggest diabetic neuropathy with Vata aggravation. Helpful Ayurvedic support: Ashwagandha – nerve nourishment, sleep, fatigue. Balarishta / Ashwagandharishta – supports nerves and weakness. Yograj Guggulu or Trayodashang Guggulu – traditionally used for nerve and Vata disorders. Ksheerbala Taila massage on feet at night is often better for burning + cold sensitivity than regular oils. Lukewarm foot soaking before sleep can reduce irritation. Also check: Vitamin B12 Vitamin D Kidney function because deficiencies can worsen neuropathy. 4. For ED & PE (Sexual Weakness) Long-standing uncontrolled diabetes commonly affects nerves and blood circulation causing ED/PE. Ayurvedic supportive options: Ashwagandha Kaunch Beej Shilajit Safed Musli These may improve stamina, nerve strength, stress tolerance, and vitality gradually. Important Lifestyle Advice Early dinner and proper sleep are essential. Avoid sweets, bakery items, sugary drinks, fried foods, smoking, and alcohol. Walk 20–30 minutes daily after meals. Avoid direct cold AC exposure on feet. Continue regular sugar monitoring, especially if combining Ayurvedic medicines with insulin.

Here’s a draft in the style you’ve been using: Your HbA1c has improved from 13.3% to 10.5%, which is encouraging. However, your C-peptide of 0.07 is extremely low, indicating severely reduced insulin-producing capacity. Therefore, please continue insulin and all prescribed medicines under your endocrinologist’s supervision. Do not stop insulin abruptly. Regarding Madhunashini Vati and Chandraprabha Vati, increased urination is more likely related to Chandraprabha Vati because of its mutrala (diuretic) action. For a comprehensive diabetes reversal and complication management approach, I generally advise: Planet Ayurveda: • Dia-Beta Plus – 2 capsules twice daily after meals • Diabetic Care Pack as advised by physician • Ashwagandha Capsules – 1 capsule twice daily Kairali Ayurveda: • Nisakathakadi Kashayam – 15 ml with equal water twice daily before meals • Chandraprabha Gulika – 2 tablets twice daily after meals • Dhanwantharam Thailam or Ksheerabala Thailam for gentle foot massage at night For diabetic neuropathy: • Ashwagandha Capsules • Ksheerabala Thailam foot massage daily • Regular foot care and avoidance of barefoot walking For ED and PE associated with long-standing diabetes: • Ashwagandha Capsules • Musli Pak – 1 tsp twice daily after meals Lifestyle & Diet: • Avoid sugar, sweets, fruit juices, bakery products, white rice and refined flour • Prefer millet rotis, vegetables, salads, sprouts and protein-rich foods • Walk for 30–45 minutes daily after meals • Maintain 7–8 hours of sleep • Reduce stress and screen exposure before bedtime Yoga & Pranayama: • Mandukasana – 5 minutes daily • Ardha Matsyendrasana – 5 minutes daily • Pawanmuktasana – 5 minutes daily • Anulom Vilom – 10 minutes daily • Bhramari Pranayama – 5 minutes daily • Kapalbhati – start slowly for 3–5 minutes if BP remains controlled Monitor: • Fasting and post-meal sugars regularly • HbA1c every 3 months • Kidney function, liver function and lipid profile every 6 months • BP and pulse weekly With strict diet, weight management, exercise, yoga and proper medical supervision, significant improvement in diabetes control and neuropathy symptoms is possible.

••It is excellent to see the significant progress you have made in just two months. Dropping your HbA1c from 13.3% to 10.5% and stabilizing your fasting blood sugar (FBS) to 90–110 mg/dL is a major milestone. Your endocrinologist’s decision to reduce your insulin from three times a day to just once (Ryzodeg before dinner) confirms that your body is responding well to treatment. ••However, your C-peptide level of 0.07 ng/mL is critically low. This indicates severe pancreatic beta-cell exhaustion or insufficiency, meaning your pancreas is currently producing very little insulin of its own. Because of this, you must never stop your allopathic medicines or insulin without your endocrinologist’s explicit guidance. 1. Understanding the Frequent Urination (Prameha Vega) ••You noticed an increase in nighttime urination after starting Madhunashini Vati and Chandraprabha Vati. •The Cause: Chandraprabha Vati is a potent classical formulation widely used for urinary tract disorders. It acts as a natural diuretic (Mootrala). Its primary function is to clear toxins (Ama) and excess fluid from the body by increasing urine output and flushing the kidneys. ••The Fix: When taken close to bedtime, it will naturally cause frequent urination at night, disrupting your sleep. Shift the timing of Chandraprabha Vati to morning (after breakfast) and afternoon (after lunch). Avoid taking it in the evening or night. 2. Managing Post-Lunch Sugar & Cardiovascular Parameters Since you do not take an allopathic oral hypoglycemic agent after lunch, your blood sugar rises to 160–200 mg/dL before dinner. To manage this and help stabilize your pulse rate (100\110 bpm) and BP complementary to Bylenta and Met XL: ••For Post-Lunch Sugar: Continue Madhugrit or Madhunashini Vati (1–2 tablets) roughly 30 minutes before or after lunch. You can also introduce Vasant Kusumakar Ras (with gold/swarna). This is a premier, high-potency classical Ayurvedic formulation for long-standing or severe diabetes with complications. It helps stabilize fluctuating blood sugars and provides cellular rejuvenation. ••Dosage: 1 tablet twice a day (e.g., after breakfast and after lunch) crushed and taken with honey or warm water. ••For High Pulse Rate & BP (Rakta Vata): •Arjunarishta / Arjuna Churna: Arjuna (Terminalia arjuna) is a proven cardioprotective herb that helps regulate heart rate (pulse) and strengthens cardiac muscles without interacting negatively with beta-blockers like Met XL. •Dosage: 15–20 ml of Arjunarishta mixed with an equal quantity of water, twice daily after meals. 3. The Truth About Beta-Cell Regeneration It is a common belief that Ayurveda can completely regenerate dead beta cells. To be clinically precise and honest with you: completely dead or fibrosed beta cells cannot be brought back to life by any system of medicine. ••However, what Ayurveda can do exceptionally well is rejuvenate dormant, exhausted, or partially damaged beta cells and improve peripheral insulin sensitivity (reducing insulin resistance). This is achieved through Rasayana (rejuvenation) therapy. ••Guduchi (Giloy) & Amalaki (Amla): This combination, known classically as Nisha-Amalaki, is a potent antioxidant that protects the remaining pancreatic beta cells from oxidative stress and glucotoxicity. •Shilajit: Regular use of purified (Shodhana) Shilajit acts as a powerful Rasayana that improves glucose uptake at the cellular level and provides deep tissue vitality. 4. Diabetic Neuropathy, Cold Sensitivity & Urinary Sensation ••Your burning sensation, severe irritation from the ankle to the feet, and hypersensitivity to the AC are classic signs of Vata-Pitta vitiation in the nerves (Dhatu Kshaya Janya Vata Prakopa), compounded by your low C-peptide. External Management (Foot Care) •Patanjali Mahanarayan Oil is excellent for joints, but for diabetic neuropathic burning and cold irritation, a cooling yet nerve-soothing oil is preferred. ••Ksheerabala Thailam: Switch to or alternate with Ksheerabala Thailam. It is processed with cow’s milk and Bala (Sida cordifolia), which directly nourishes degenerated nerves and reduces burning sensations. ••Application: Warm the oil slightly and apply it gently from the knees down to the soles of your feet at bedtime. Do not rub vigorously. Internal Nerve Rejuvenation ••Ekangveer Ras or Maha Vatavidhwansan Ras: These are classical herbo-mineral preparations aimed at correcting nervous system disorders (Vata Vyadhi). They work beautifully alongside Gabaprix NT. •Dosage: 1 tablet twice a day with warm water. ••Ashwagandha Churna / Extract: Ashwagandha is neuroprotective, promotes nerve growth factor, and helps reduce the stress and sleep disturbances caused by foot irritation. ••Testicular Connector Sensation The brief 1–2 minute burning sensation after urination near the testicular area suggests transient nerve irritation or mild urinary acidity. Moving your Chandraprabha Vati to daytime will help clear this. Additionally, drink Gokshuradi Guggulu (1 tablet twice daily) to protect the genitourinary tract. 5. Erectile Dysfunction (ED) & Premature Ejaculation (PE) High blood sugar damages both the micro-blood vessels (endothelial dysfunction) and the autonomic nerves supplying the reproductive organs. This restricts blood flow and reduces sensitivity, leading to ED and PE. Because your body is currently weak (Dhatu Kshaya) and blood sugars are still stabilizing, strong, heating aphrodisiacs must be avoided. Instead, we use nutritive, safe Vajikarana herbs: ••Kapikachhu (Mucuna pruriens): It is a natural source of L-dopa, which improves dopamine levels, helps manage PE, and improves motor/nerve coordination. ••Safed Musli & Shatavari: These are cooling vitalizers that restore tissue strength (Ojas) without elevating your blood pressure or pulse rate. ••Recommended formulation: Tentex Royal (Himalaya) or a combination of Ashwagandha and Kapikachhu powder (3 grams each) with lukewarm milk at bedtime. Note: Introduce reproductive vitalizers only after your fasting and post-meal sugars stay consistently under control for at least another month… ••Monitor Regularly: Ayurvedic herbs like Madhunashini and Vasant Kusumakar Ras are potent. When combined with Linaray M and Ryzodeg, there is a risk of hypoglycemia (low blood sugar). If you feel dizzy, sweaty, or shaky, check your sugar immediately and consume glucose. ••Liver and Kidney Function: Because you are combining multiple systems of medicine, get a Liver Function Test (LFT) and Kidney Function Test (KFT/Serum Creatinine) done every 3 months to ensure your organs are processing the medications safely.

For diabetic neuropathy, Katakakhadira Kashayam along with Ashwagandharishta may be taken together with allopathic medicines. Similarly, for high blood pressure, Sarpgandha Vati and Arjunarishta are commonly used alongside allopathic treatment. These combinations are generally considered safe in many patients when taken in appropriate doses with regular monitoring of blood sugar and blood pressure.

Your sugar control is improving well, which is a very positive sign. Since you are already on insulin and multiple medicines, Ayurveda should be added carefully and gently to avoid sudden sugar drops. Frequent urination may increase with Chandraprabha vati because it has mild diuretic action. You can reduce or stop it temporarily and observe. For neuropathy, burning feet, weakness, and sugar support: Madhunashini vati – continue 1 tablet after lunch only Ashwagandha capsule – 1 at bedtime Balarishta – 15 ml with equal water after lunch and dinner Ksheerbala oil or Dhanvantaram oil – gentle massage on feet at night Cap. Neuron Plus / Brahmi based nerve support – once daily For BP and pulse: Arjunarishta – 15 ml with equal water after food twice daily For ED/PE: Shilajit capsule – 1 at bedtime after sugar becomes more stable Kaunch beej churna – ½ teaspoon at night with warm milk At present, focus more on stabilizing diabetes and nerves first. Ayurveda may support pancreatic function and metabolism, but “beta cell regeneration” cannot be guaranteed. Continue regular sugar monitoring and do not reduce insulin on your own. Regards, Dr Raghuveer (Ayurvedacharya)
