••As an Ayurvedic practitioner, I look at this through the lens of Prameha Upadrava (complications of chronic diabetes) affecting the Vrikka (kidneys), specifically damaging the Mutra-vaha Srotas (the channels responsible for urine formation and filtration). At an eGFR of 26, he is in Stage 4 Chronic Kidney Disease (CKD). While Ayurvedic principles can be beautifully tailored to support kidney function, maintain his current strength, and help delay the progression toward dialysis, it must be done with extreme precision. ••Because you are overseas and cannot access specific Sri Lankan herbs, we will focus entirely on dietary modifications (Ahara) and lifestyle adjustments (Vihara) using globally available ingredients to manage blood sugar, control urea/creatinine buildup, and protect his remaining nephrons (filtering units). ••1. Dietary Management (Ahara) When diabetes complicates kidney function, the diet requires a delicate balance: it must keep blood sugar stable without overloading the kidneys with excess protein, potassium, or phosphorus. Grains to Favor ••Old Rice / Basmati Rice: Well-cooked, warm Basmati rice is easy to digest (Agni-deepana) and puts minimal stress on the digestive tract. ••Barley (Yava): This is perhaps the most highly recommended grain in Ayurveda for kidney and diabetic health. Barley is naturally Kaphahara (reduces fluid retention) and acts as a mild, safe diuretic (Mutrala) to help clear toxins without straining the filtration system. You can give him well-cooked barley porridge or use barley flour. ••Avoid: Whole wheat, ragi (finger millet), and bajra, as they are exceptionally high in potassium and phosphorus, which the kidneys are currently struggling to filter. Vegetables & Cooking Styles ••The Gourd Family: Vegetables like bottle gourd (lauki), ridge gourd (turai), pointed gourd (parwal), and ash gourd (pethai) are cooling (Sheeta Virya) and soothing to the urinary tract. They help balance Pitta and Vata in the kidneys. ••Leaching Technique: Always peel, chop, and boil vegetables in a large amount of water, then discard the water before final cooking. This significantly reduces the potassium content, making it safe for his stage of CKD. ••Avoid: Raw vegetables, salads, spinach, tomatoes, potatoes, and sweet potatoes. Protein Regulation ••Moong Dal (Green Gram): Split yellow moong dal is the only lentil recommended at this stage. It should be cooked thin (like a soup) with a pinch of turmeric and cumin. ••Strictly Restrict: Heavy proteins like red meat, seafood, paneer, and heavy lentils. High protein intake increases blood urea nitrogen (BUN), forcing the kidneys to work much harder. Spices to Include ••Turmeric & Coriander Powder: Excellent anti-inflammatory agents that help protect micro-vessels. ••Cumin & Fennel: Support digestion without generating excess internal heat (Pitta). ••Strictly Avoid: Excess salt (keep sodium strictly limited to manage blood pressure and fluid retention) and heavy hot spices like red chili powder. 2. Vital Fluid & Electrolyte Management At an eGFR of 26, the kidneys lose their ability to manage fluids properly. ••Fluid Intake: Do not force him to drink excess water thinking it “flushes the kidneys.” Water intake should match his urine output plus a small baseline (usually around 1 to 1.5 liters total per day, but this must be cross-verified with his nephrologist). ••Avoid: Coconut water, citrus fruit juices, and banana smoothies. While incredibly healthy for a normal individual, their exceptionally high potassium content can be dangerous for a diabetic kidney patient and can severely affect heart rhythm. 3. Lifestyle & Mindset (Vihara) ••Gentle Movement: Strenuous exercise increases muscle breakdown, which spikes creatinine levels. Stick to calm, gentle breathing exercises (Pranayama like Anulom Vilom) and short, gentle walks. ••Rest: Ensure he gets sound sleep. In Ayurveda, proper rest supports Ojas (vital energy) and tissue healing.
