ULTRASONOGRAPHY OF PELVIS Real time. B mode, gray scale sonography of the pelvic organs was performed. Additional transvaginal study was also performed. UTERUS: The uterus is retroverted and retroflexed. It measures 7.2 x 4.1 x 4.1 cms. in the longitudinal, transverse and anteroposterior dimensions, respectively. This is within normal limits. The uterine myometrium shows heterogenous echotexture. Small hypoechoic deposits are seen at the posterior surface of uterus, likely to be endometriotic deposits The endometrial echo is in the midline. It measures 6.1 mm. OVARIES: The ovaries on the either side appear adherent to posterior surface of uterus. Right ovary is bulky and shows an endometriotic cyst within. The cyst measures 24 x 21 x 20 mm (5 ce in vol.) The right ovary measures 44 x 31 x 23 mm (17 cc in vol) and the left ovary measures 33 x 22 x 12 mm (4 ce in vol). A well defined heteroechoic lesion is seen in right adnexa closely abutting the uterus and right ovary. It measures 2.7 x 2.4 x 2.2 cm in size. Differential diagnosis includes a subserosal fibroid and an endometriotic cyst. Minimal free fluid is seen in the cul-de-sac IMPRESSION: Small hypoechoic deposits are seen at the posterior surface of uterus, likely to be endometriotic deposits. Endometriotic cyst in right ovary. A well defined heteroechoic lesion in right adnexa closely abutting the uterus and right ovary. Differential diagnosis includes a subserosal fibroid and an endometriotic cyst. Ahove findings are concerning for deep pelvic endometriosis.
