Guidance on Combining Ayurvedic Medications for Avascular Necrosis - #46604

I recently sought medical advise through this platform for treatment of Avascular necrosis of the femoral head. The responses were overwhelming and i am grateful to all the doctors/specialists that took their time to respond. In that regard, i was able to come up with a list from the different prescribed medications which varied from doctor to doctor. Apparently, each doctor suggested slightly different regiments, but i picked the main/common ones, which i have since gone ahead to purchase. Now im not sure of what to take or what not take and how best to combine them as well as how to take them, with or without food etc etc. Here below is the list of what i was able to buy: 1. Dashmoola Kwath - 400ml 2. Abha Guggulu- tabs 3. Kaishore Guggulu- tabs 4. Lakshadi Guggulu- tabs 5. Panchatikta Ghruta Guggulu- tabs 6. Manjishthadi Kashay- 400ml 7. Mahanarayan Kashay- 400ml 8. Haritaki Vati- tabs My request, in light of the above meds, is may you kindly prescribe/recommend the best combination to tackle Avascular Necrosis of the femoral head stage 2 to 3. I would be grateful for your assistance in this regard. Brighton.

I’m going to sort out the meds you already bought so everything works well together, keeps you safe, and actually helps with your Avascular Necrosis (AVN) of the femoral head – Stage 2–3

YOUR CONCERN AND AYURVEDIC UNDERSTANDING

Basically, AVN of the femoral head means:

–Asthi-Majja Gata Vata: Issues with bone marrow because of ‘Vata’ imbalance. –Srotorodha: Blocked tiny blood vessels. –Asthi kshaya: Bone wearing away. – Inflammation and pain. –Poor blood flow to the femoral head.

TREATMENT GOAL – Get your blood moving better. –Clear out toxins and blockages. –Feed your bones and marrow. –Calm the pain and swelling. –Slow down or stop the problem from getting worse.

First Big Rule – Don’t Take Everything At Once

You’ve got some really good medicines, but many of them do similar things. If you take them all together, you’ll: * Overload your digestion. * Not absorb them as well. * Have a higher chance of side effects.

INTERNAL MEDICATION

✅ MAIN MEDICINES (What I suggest)

1. Lakshadi Guggulu – For bone heal 2 tablets, twice a day After food Warm water or milk Why it helps: * Helps bones rebuild * Makes the femoral head stronger * Super important for AVN

2. Panchatikta Ghrita Guggulu – Deep bone care 1 tsp twice a day After food Warm water Why it helps: * Gets deep into bone tissue * Clears blockages in bone channels * Slows down the breakdown This + Lakshadi Guggulu = The core of your AVN treatment.

3. Manjishthadi Kashayam – For circulation and healing 15 ml with 15 ml warm water Morning and evening before food Why it helps: * Boosts blood flow to the femoral head * Reduces swelling * Helps tissues fix themselves

4. Triphala churna i – For digestion and cleansing 1 tsp at bedtime Warm water Why it helps: * Stops constipation * Helps your body process the guggulu medicines better * Reduces toxins

Medicines to Skip or Hold Off On

❌ Dashmoola Kwath: This is more for pain. It’s better for sudden, bad pain, not really for long-term AVN. ❌ Kaishore Guggulu: This is a strong cleanser. You don’t really need it unless you have gout or major swelling. ❌ Abha Guggulu: Mainly for mending broken bones. AVN needs something that feeds the bones and marrow more deeply. ❌ mahanarayan Kashayam:Good for nerve pain and stiffness. We can add this later if stiffness becomes a big issue.

We can bring these in later, but not all at once.

YOUR DAILY PLAN (Easy to Follow)

Morning (Before anything else) Manjishthadi Kashayam – 15 ml with 15 ml warm water

🍽 After Breakfast Lakshadi Guggulu – 2 tablets Panchatikta ghrita guggulu 1 tsp

Evening (Before dinner) * Manjishthadi Kashayam – 15 ml with 15 ml warm water

🍽 After Dinner * Lakshadi Guggulu – 2 tablets * Panchatikta Ghrita Guggulu – 1 TSP

Bedtime Triphala churna Vati – 1 tsp

How Long Will This Take?

AVN isn’t a quick fix. Stopping too soon means you won’t get the full benefit.

❌ AVOID * Drink alcohol (super important!) * Take steroids (unless it’s truly life-saving) * Put too much weight on your hip * Jump, run, or squat * Expose your hip to cold

✅ INCLUDE

* Walk with partial weight if your doctor says so * Do gentle physical therapy * Massage your hip with warm oil * Get enough sleep

DIET PLAN

✅INCLUDE * Warm, freshly cooked food * A little ghee * Moringa leaves * Black sesame * Milk with turmeric (if you’re okay with dairy)

❌ Avoid:

What to Really Expect

Ayurveda can slow AVN down, ease pain, and help bone health in Stage 2–3. But remember: * Reversing damage takes time. * Being consistent is more important than how many medicines you take. * Panchakarma (Basti) later on often gives the best results.

In stage 2 to early stage 3 AVN, in Ayurveda the goal is to reduce vata rakta obstruction, improve micro circulation to the femoral head, nourish bone tissue and prevent further collapse.Taking too many medicines together can disturb digestion, weaken agni and reduce the effectiveness of even good medicines.

I would advise you to restrict to only three core medicines for .These three are sufficient, classical, complementary to each other, and safe for long term use

The best three medicines for your condition are Lakshadi Guggulu, Manjishthadi Kashayam, and Dashmoola Kwath.

Lakshadi Guggulu is the backbone medicine for avascular necrosis when viewed through the Ayurvedic lens. This formulation directly acts on asthi dhatu and majja dhatu. Laksha is known for its role in bone healing, strengthening and regeneration, while guggulu improves circulation, reduces inflammation and removes srotorodha, especially in deeper tissues. In AVN, the blood supply to the femoral head is compromised. Lakshadi Guggulu helps improve micro circulation while simultaneously supporting bone integrity. This medicine is particularly useful in stage 2 and early stage 3 AVN where the aim is preservation of the joint. It should be taken after food, because guggulu formulations can irritate the stomach. Two tablets twice daily after breakfast and dinner with lukewarm water

The second important medicine is Manjishthadi Kashayam. This is crucial because AVN is not only a bone disorder, it is also a rakta and circulation disorder. Manjishtha is one of the best herbs in Ayurveda for rakta shodhana, improving blood quality and micro vascular flow. In many AVN patients, there is clotting tendency, inflammation, or sluggish circulation that modern reports may not clearly show. Manjishthadi Kashayam works gently but deeply to clear stagnation, reduce inflammatory processes and support tissue oxygenation. This kashayam should ideally be taken on an empty stomach, morning and evening, around 15 ml mixed with an equal quantity of warm water. If your digestion is sensitive, you may start with once daily in the morning for the first week and then increase to twice daily. This medicine complements Lakshadi Guggulu beautifully because it prepares the tissue environment for healing.

The third medicine is Dashmoola Kwath. Dashmoola is important in because vata is a central factor in AVN progression. It pacifies vata, reduces deep seated inflammation, relieves pain and stiffness around the hip joint and supports neuromuscular balance. It also helps counter stress related vata aggravation, which otherwise keeps disturbing circulation and healing. Dashmoola Kwath is best taken in the evening, around one hour before dinner or at least two hours after meals. Take 15 ml mixed with equal warm water.

Instead, it may burden digestion, increase dryness and aggravate vata . Mahanarayan Kashayam is useful in neuromuscular pain and paralysis like conditions and can be added later if required. Haritaki Vati should not be taken unless constipation is present

include warm, nourishing, slightly unctuous foods. Avoid dry, cold, raw foods, excessive salads, packaged foods and intermittent fasting. These aggravate vata and slow bone repair. Milk, ghee well cooked vegetables, soaked almonds, sesame seeds and warm meals to be taken . Avoid alcohol completely, as it directly worsens avascular changes. Excess caffeine and late nights should be avoided. Avoid high impact exercises, running, jumping or heavy weight bearing

In stage 2 to 3 AVN, you should assess progress over months, not weeks. Pain reduction, improved mobility, better sleep and reduced stiffness are early signs that the medicines are working. Radiological improvement takes longer. Do not keep changing medicines frequently. Consistency is far more important than quantity.

After 8 to 12 weeks on this regimen, a reassessment can be done and only then should any additional medicines be considered, based on response and digestion. For now, keeping the treatment simple, focused and disciplined will give you the best possible outcome.

I will advise you a modified protocol which works best in the management of AVN. Start with- 1. Panchatikt Ghrit Guggulu 2-0-2 after food 2. Gandha Taila Capsule 2-0-2 after food 3. Mahamanjisthadi Kashayam 15ml-0-15ml with 15ml warm water before food 4. Regenerative Bhasma Mix: Mix Mukta Shukti Bhasma (500mg) + Guduchi Satva (500mg) + Yashad Bhasma (125mg). Take this mix twice daily with Honey or Ghee.

If Possible Go for Panchatikt Ksheer basti that is medicated Enema therapy given in Panchakarma. Find yourself a nearby Panchakarma centre for this. Plan this for 15 days.

External Therapy Murivenna Oil: Apply gently on the hip and groin area. Do not massage vigorously.

Dietary Advice Avoid: Sour foods (Curd, Pickles), Alcohol, and Excessive Walking/Weight bearing. Include: Milk, Ghee, Meat Soup (Mutton soup), and Black Gram (Urad Dal) to nourish the wasting tissues.
