A 20.9 mm gallbladder stone is considered a large stone. In Ayurveda/Naturopathy, treatment may help reduce symptoms, improve digestion, and sometimes support gradual reduction of small stones, but a stone of this size usually has limited chances of fully dissolving with medicine alone. If you are having repeated pain, vomiting, fever, jaundice, or infection, then surgery is often the safer option. ----From an Ayurveda point of view, this condition is related to Pitta–Kapha imbalance, Ama accumulation, and impaired Yakrit–Pittashaya function. -------rx- 1. Aarogyavardhini Vati 1 tablet twice daily after food with lukewarm water 2. Varunadi Kashayam 15 ml + 15 ml warm water twice daily before food 3. Tab Cristone (Baidyanath/Bang) 1 tablet twice daily after food 4. Syp Neeri 2 tsp twice daily after food with lukewarm water —Pathya (Diet Advice) Avoid oily, fried, spicy, bakery foods Avoid paneer, cheese, red meat, alcohol Take light warm food, moong khichdi, lauki, tori Drink adequate warm water Daily walking 30 minutes ----Advice Monitor pain, vomiting, fever, jaundice Repeat USG after 2–3 months If severe pain or blockage symptoms occur, surgical opinion is necessary

This treatment plan is a structured Ayurvedic approach focused on managing your gallbladder stone, which measures 20.90 mm. In Ayurveda, gallbladder stones are often linked to an imbalance in the digestive fire and internal clearing systems. Because a stone of this size is quite large, the primary goal of these medicines is to support bile flow, reduce inflammation in the gallbladder wall, and try to break down or soften the stone over a 5 to 6-month period. However, as noted, if the stone does not dissolve or if symptoms worsen, surgery remains the necessary next step. The medications prescribed work together in a few distinct ways. Formulas like Varun Shigru Kwath is traditionally used as lithotriptics, meaning they are selected for their potential to gradually break down or weaken hard mineral deposits. Meanwhile, Arogyavardhini Vati and Tab. Liposem Plus focus heavily on supporting your liver and regulating cholesterol metabolism, which is highly relevant since most gallbladder stones are made of hardened cholesterol and stagnant bile. Another major part of this line of treatment is reducing swelling and flushing the system. Remedies like Punarnavaasav, Gokshur Guggulu, and the Shwet Parpati mixture act as natural anti-inflammatory agents and diuretics (medicines that increase urine and fluid clearance). By reducing swelling in the surrounding tissues and promoting a smooth flow of fluids, they help ensure that the bile ducts do not become easily irritated or obstructed by the stone. The natural juices and fresh leaves added to your routine provide deep purification. Karela (bitter gourd) or Amla (Indian gooseberry) juice helps stimulate healthy liver enzymes and bile secretion, while fresh Kalanchoe pinnata (Parnabeeja) leaves are traditionally valued in folklore medicine for their specific action on internal stones. Taking these on an empty stomach allows their active components to be absorbed effectively by the digestive system. It is absolutely vital to monitor how your body responds month by month. While you are advised to take these for 5 to 6 months, you must do a follow-up check after the first month. If you experience severe pain in the upper right side of your abdomen, fever, nausea, or yellowish discoloration of the eyes (jaundice) at any point, do not wait for the month to end. These are signs that the stone may be blocking a bile duct, meaning you should skip the herbal approach and consult a surgeon immediately.

Can a 20.90 mm Stone Be Dissolved with Medicine? ••To be completely honest and direct, it is highly improbable and clinically unsafe to attempt to fully dissolve or flush a 20.90 mm stone using alternative medicines alone. ••The Size Limitation: While smaller stones (under 5–7 mm) or biliary sludge sometimes respond well to Lekhana (scraping) and Bhedana (breaking) Ayurvedic therapies, a stone larger than 2 cm is highly structured and dense. ••The Risk of Obstruction: If a 20.90 mm stone begins to break or shift under the influence of strong cholagogue medicines (which force the gallbladder to contract), a piece can easily get lodged in the neck of the gallbladder or the Common Bile Duct (CBD). This can cause life-threatening complications such as acute cholecystitis, obstructive jaundice, or biliary pancreatitis. ••What is Better: Surgery or Medicine? To decide the best path forward, your current clinical symptoms must be evaluated. In modern gastrointestinal surgery and integrated Ayurveda, the protocol depends on how the stone behaves: •Case A: If You Have Active Symptoms (Emergency/Surgical) •If you are experiencing: •Sudden, severe pain in the upper right •abdomen radiating to the back or shoulder (Biliary Colic)
•Nausea, vomiting, or persistent bloating after fatty meals •Fever with chills, or yellowing of the eyes/skin (Jaundice) ••Verdict: Surgery (Laparoscopic Cholecystectomy) is absolutely required and is the safest option. Delaying surgery in the presence of symptoms can lead to gallbladder perforation or severe internal infections. Case B: If the Stone is Completely Asymptomatic (Silent) •If this stone was discovered accidentally during a routine ultrasound and you currently have zero pain or digestive distress: •You may temporarily delay surgery under strict medical supervision. •In this window, you can use conservative treatment (Ayurveda/Naturoveda) not with the expectation of dissolving the stone, but to prevent the stone from growing, reduce inflammation, and improve bile flow. 3. Ayurvedic Line of Management (Conservative Protocol) If you are asymptomatic and managing the condition conservatively under professional guidance, classical Ayurveda approaches Pittashmari by balancing the Pitta-Kapha doshas and strengthening the liver (Yakrut):
••Deepana & Pachana (Improving Digestion): Using formulations like Arogyavardhini Vati (which contains 50% Kutki, a potent liver stimulant) or Shankha Vati to maintain Agni and prevent further bile stagnation.
••Bile Regulating Decocotions: Varanadi Kashayam or Patolakaturohinyadi Kashayam are traditionally utilized to cleanse the biliary tree and improve the solubility of bile.
••Dietary Strictness: •Completely eliminate heavy, saturated fats, deeply fried foods, and refined sugars that strain bile production.
•Incorporate bitter and light foods like bitter gourd (Karavellaka), pointed gourd (Patola), and horse gram (Kulattha), which naturally balance Kapha and Pitta.
