Pelvic hypertonia with rectal pressure and false urgency is mainly Apana Vata dysfunction with pelvic floor spasm. A more precise Ayurvedic protocol is as follows. Internal medicines Ashwagandha churna 3 g at night with warm milk for neuromuscular relaxation Hingvastak churna 1 g after lunch and dinner with warm water to correct gas, spasm and downward bowel coordination Yogaraj Guggulu 1 tablet twice daily after food for chronic Vata-pain and pelvic tightness If constipation or incomplete evacuation is present Isabgol 5 g at bedtime with warm water or milk Avipattikar churna 3–5 g at night if associated acidity or burning sensation Local therapy Perianal and lower abdominal massage with Mahanarayana taila or Dhanwantharam taila once daily for 10–15 minutes Warm sitz bath with Dashamoola kwatha decoction for 10–15 minutes daily to relax pelvic floor muscles Basti therapy (most important in this condition) Anuvasana basti with plain sesame oil 60 ml once daily for 7 days If symptoms are severe, Yogabasti schedule under supervision (Matra basti alternated with Niruha basti using Dashamoola decoction) for 8–10 days Lifestyle correction Avoid long sitting, straining, constipation, cold food, excessive caffeine Practice gentle deep breathing and pelvic floor relaxation rather than tightening exercises
