Moderate fluid in pouch of Douglas and endometrial inflammation/endometriosis can occur due to pelvic infection, ovulation, cyst, or endometriosis. Proper gynec evaluation and ultrasound follow-up are important.
Take
Ashokarishta
15–20 ml with equal water twice daily after food
Kanchanar Guggulu
2 tablets twice daily after food
Chandraprabha Vati
1 tablet twice daily
If pain is present
Dashmool kwath 15 ml twice daily before food
Diet: Avoid very spicy, junk, cold, and fermented foods. Take warm light food and good hydration.

Hello maam , free fluid can be managed very well with medications only . And all other symptoms like fever and abdominal pain may be due to the PID . Many of the times metabolic disturbance is the cause for this ,so proper investigation like diabetic profile , thyroid levels and lipid profile is needed . You should workup on this . And endometrial issues you have not mentioned what kind of issues , but most of the issues are manageable with ayurvedic treatment.

Moderate fluid in the pouch of Douglas with endometrial changes usually indicates pelvic inflammation, hormonal imbalance, infection, or conditions like endometriosis/PID. Since you also have fever and abdominal pain, proper gynecological evaluation is important and should not be ignored.
From an Ayurvedic perspective, this condition is commonly related to aggravated Vata-Pitta dosha, pelvic inflammation, and disturbed Artava Dhatu
Diet & Lifestyle
Avoid very oily, spicy, junk, cold, and fermented foods.
Reduce excess sugar and bakery items to help with inflammation and weight gain.
Take adequate rest and avoid stress.
Gentle walking and yoga like Baddha Konasana and Supta Vajrasana may support pelvic circulation.
Advice
Because you have fever with pelvic pain, please get a pelvic ultrasound and gynecologist consultation if not already done, as infections or endometriosis sometimes require timely medical treatment along with Ayurveda.
