Hello, Mr. Ajit Kumar Maiti, your situation needs careful handling - especially because of age (76 yrs), kidney involvement, long term nerve Pain medicines, and gastric issues FIRST- about Lumbagest capsules Do not start lumabgest on your own right now Reasons -you already have early kidney impairment (albumin-creatinine ratio 35.9-> boderline abnormal) -Many Ayurvedic pain formulation (including lumbagest) may contain –guggulu, resins, or mineral preparations-> can stress kidneys if digestion is weak You also have severe bloating+ gastric disturbance, which increase risk of poor metabolism of such medicines So yes- your concern is valid. it may aggravate kidney function or digestion Your condition (important) -chronic L4-L5 disc issue->now aggravated -Likely nerve compression-> earlier treated with Gabapentin/pragabalin Now -kidney stress mild -severe bloating, gas -weak digestion -age related vata aggravation In Ayurveda, this is vata vyadhi + ama + asthi/majja involvement Internal medications 1) Dashmoolarishtam = 15ml + equal water after meals twice daily =reduces inflammation, nerve pain 2) Gandharvahastadi kashaya tablets= 2 tabs twice daily before meals =best for gas, constipation, vata correction 3) Rasnaerandadi kashaya tablet= 2 tabs twice daily before meals =specifically for disc pain, sciatica -For gastric and bloating very important Without correcting this, no pain relief will sustain -Hinguvachadi gulika= 1 tab twice daily after meals -warm water sipping whole day Avoid= curd at night, fried food, chana, rajma, cabbage External treatment (most effective) Daily at home -dhanwantaram taila =warm oil massage on lower back 15 min -follow with hot water fomentation What to Avoid -self starting Lumagest -painkiller overuse -long sitting -cold exposure to back -dry foods increase vata About Pregabalin stopping -good you stopped (kidney caution) -but nerve pain may rebound When to seek immediate medical help -leg weakness increasing -loss of bladder control -severe night pain not reducing Do not replace pregabalin with lumagest directly First stabilize -digestion -vata -kidney safety Do follow Hope this might be helpful Thank you

Avoid lumbagest We will prioritize high safety herbs for nerve pain and inflammation without putting undue strain on the kidneys. 1) Ksheerabala (101) Capsules-1 Capsule twice daily before food 2) Sahacharadi Kwath Tablet- 2 Tablets-Twice daily, before food 3) Dasamoolarishtam-15 ml liquid with 45 ml water Twice daily, after food 4) gandharv hastadi castor oil- 5 ml once daily at bedtime with warm water avoid heavy proteins like paneer, soy, and urad dal. Stick to light Moong Dal or Peya (thin rice water). sip warm water throughout the day Use Old rice (Sastika Shali), Barley (Yava)well-cooked Oats. Bottle gourd (Lauki), Pointed gourd (Parwal), Ridge gourd (Turai), and Pumpkin. . Use small amounts of Ginger, Cumin (Jeera), Fennel (Saunf), and Turmeric. Avoid Potato, Cauliflower, Cabbage, Broccoli, and Ladies’ finger (Bhindi). These will worsen his L4-L5 pain by increasing “Vata” (gas) in the gut. Avoid curd, especially at night. Fresh Buttermilk (Takra) with a pinch of Cumin is a better alternative during the day. Keep salt intake low to protect the kidneys. Avoid red chili and heavy garam masala which aggravate gastric issues. Avoid Idli, Dosa, or Bakery items (bread/biscuits) you shouldfinish his last meal by 7:00 PM. This prevents nighttime bloating and pressure on the spine. Avoid long-distance walking. Instead, 5-10 minutes of gentle walking inside the house after meals is enough to aid digestion. Use a firm chair with back support. Never sit on the floor or low stools, as this puts maximum pressure on the L4-L5 disc.

Your urine test shows early kidney strain, so you should avoid taking any new medicines –You are taking: Thyronorm → continue as prescribed Urimax-D → continue unless advised otherwise No issue with kidney from these at your current stage. –Your condition (long-standing disc bulge) suggests: Likely nerve compression (sciatica-type pain) Now aggravated after many years At age 76, best approach is conservative – at present don’t stop any medicine Nd take - SYP Nerwine 2 tsf twice daily after meal - muscalt forte 2 tab twice daily after meal Sudden stopping of pregablin can cause: Pain rebound Sleep disturbance Anxiety —If pain is still severe, you may need: Either restart at lower dose

Lumbagest capsules contain Guggulu. With your slightly raised urine albumin-creatinine ratio, do not start Lumbagest. Ayurvedic medicines with Guggulu can sometimes increase kidney load in elderly patients. ✓Medicines to start Dashmoolarishta – 10 ml + 30 ml warm water twice daily after meals Ashwagandha capsule – 250 mg once daily at night with warm milk Hingwashtak Churna – ½ tsp with first bite of lunch & dinner ✓External Care (Daily) Warm Mahanarayana Taila or Ksheerabala Taila – gentle massage on lower back & right leg 10–15 min, followed by hot water bag for 10 min, twice daily. ✓Daily Must-Do Walk 5–10 minutes → sit and rest → repeat (avoid continuous standing/walking) Vajrasana 5 minutes after meals Legs elevated 15 minutes, 2–3 times a day Light Anulom-Vilom pranayama 5–10 minutes morning ✓Diet Warm, light, easy-to-digest: Moong dal khichdi + 1 tsp ghee, cooked lauki, bottle gourd, pomegranate, thin buttermilk. Avoid: Cold drinks, fried/spicy food, heavy meals at night, excessive salt. ✓Repeat kidney function test & USG after 2 weeks Do not stop Thyronorm or Urimax-D. Find yourself a nearby Panchakarma centre and go for KatiBasti treatment for 10 days Regards Dr Gursimran Jeet Singh MD Panchakarma

Respected Sir, Your case requires careful and cautious management because multiple factors are involved — long-standing lumbar disc pathology, recent severe pain, early kidney parameter changes, hypothyroidism, prostate enlargement, and digestive disturbance. 1. About your kidney report (UACR 35.9 µg/mg): This value is mildly elevated and indicates early kidney stress, but it is not severe kidney failure. However, at your age (76 years), it is important to avoid any unnecessary medication load, whether allopathic or Ayurvedic. 2. Regarding Pregabalin M 75 discontinuation: Stopping it suddenly after prolonged use may sometimes: •Increase nerve pain (rebound) •Cause discomfort or sleep disturbance So pain management should be gradual and supervised. 3. Can you take Lumbagest Capsules? Yes, but with caution , not as a direct substitute. Lumbagest is an Ayurvedic formulation generally used for: •Lumbar spondylosis •Sciatica •Vata-related pain It is not known to directly damage kidneys when taken in proper dose from a reliable source. However in your case: •You have early kidney involvement •You have gastric issues and bloating •You are already on multiple medications Therefore, taking it twice daily directly may be too strong initially. 4. Safer approach (Recommended): Instead of jumping to full dose: •Start with 1 capsule once daily after food •Observe for 5–7 days: •Any increase in bloating? •Burning, heaviness, or discomfort? If well tolerated, then increase to twice daily. Your condition is primarily: •“Vata Vyadhi” (nerve + degenerative issue) •Associated with “Agnimandya” (poor digestion) •And “Mutravaha srotas involvement” (urinary/kidney stress) If digestion is not corrected, any medicine (even Ayurvedic) can aggravate symptoms. a) For back pain: •Daily gentle oil application (Kati Abhyanga) with warm sesame-based oil •Mild hot fomentation b) For digestion & bloating: •Warm water throughout the day Avoid: •Curd at night •Heavy, fried, and gas-forming foods •Take small, light meals c) For kidney safety: •Maintain hydration •Avoid self-medication or multiple combinations Stop the capsule and consult if you notice- •Increased swelling in feet •Reduced urine output •Persistent gastric discomfort If possible, please share: •Exact nature of pain (radiating / localized) •Current creatinine value I can guide you with a more precise Ayurvedic protocol tailored to your condition.

At your age and with your current reports, your case needs a careful and safe approach. You have chronic disc prolapse with severe pain, along with early kidney involvement (raised urine ACR) and gastric issues. Lumbagest capsules are generally safe, but in your case they should be used cautiously and in a controlled dose, as kidney function is slightly compromised. Taking 1 capsule twice daily after food is acceptable, but do not exceed this dose and monitor your kidney parameters regularly. At the same time, pain management should not rely only on capsules. Start Mahayograj Guggulu 1 tablet twice daily after food for nerve and joint support. For local relief, apply Mahanarayan oil with gentle warm massage on the lower back followed by mild heat fomentation daily. This reduces stiffness and improves circulation without burdening the kidneys. Since you have constant bloating and gastric sensitivity, avoid strong or heavy medicines. Add Hingvashtak churna ¼ tsp with meals to improve digestion and reduce gas, which indirectly reduces back discomfort. Continue Thyronorm and Urimax-D as prescribed. Maintain good hydration, avoid high-protein overload, and limit painkillers unless absolutely necessary, as they can further affect kidneys. With this approach, pain starts reducing gradually over 2–3 weeks while keeping kidney function safe. Regular monitoring of urine ACR and creatinine every 2–3 months is important.

Lumbagest capsules has 3 ingredients,: sesame oil, castor oil and nirgundi, none of these can damage your kidney. Can take 1-0-1 after food with water. Also apply myaxyl pain relief oil on affected area twice daily.
