Hello, recently I have been diagnosed with acute cholecystitis with inflamed gallbladder of wall thickness 7.8mm and multiple calculi. After the severe pain I ended with in emergency and prescribes antibiotics. After the course, I did the scan done. Now gallbladder wall is back to 2.3 mm , with one stone at neck of size 6.5mm and largest measuring 7.5 mm in the body. Doctor prescribed surgical removal in one week. Any recommendations?

How long have you been experiencing symptoms related to your gallbladder?:

- 1-4 weeks

How would you describe the severity of your pain during the acute episode?:

- Severe — significant pain

Have you noticed any specific foods that trigger your gallbladder pain?:

- Fatty foods

How is your appetite since the diagnosis?:

- Normal — eating well

Have you experienced any associated symptoms like nausea or bloating?:

- Not sure

What is your current level of physical activity?:

- Moderately active — some exercise

Have you previously tried any alternative treatments or dietary changes?:

- No, this is my first time seeking help

Hello dear, The reduction in your gallbladder wall thickness from 7.8 mm to 2.3 mm is excellent news; it indicates the acute inflammation has subsided, providing an optimal, safer window for your upcoming cholecystectomy. Standard pre-operative management strictly focuses on avoiding another acute attack. Stick to a very low-fat, bland diet, eat small portions, and stay hydrated. If you experience a sudden return of severe upper right quadrant pain, fever, or jaundice before your surgery date, go to the emergency room immediately. In Ayurveda, gallstones are correlated with Pittashmari. The pathology involves vitiated Vata and Kapha drying and crystallizing the aggravated Pitta within the Pittashaya (gallbladder). Since surgery is only a week away, the Ayurvedic goal is not to dissolve the stones which is a long-term protocol but rather palliative care. The focus is on pacifying Pitta, maintaining a balanced Agni (digestive fire), and preventing Aam accumulation to avoid triggering biliary colic before the removal. Ayurveda Medicines : 1. Varanadi Kashayam: 15 ml mixed with 45 ml of warm water, taken twice daily before food. 2. Avipattikar Churna: 3 to 5 grams taken with warm water at bedtime for mild laxation and Pitta pacification. 3. Kumaryasavam: 15 to 20 ml mixed with an equal amount of water, taken twice daily after meals to support general hepatobiliary function.

Thanks doctor. So can I take a chance to take the ayurvedic route instead of removing the gallbladder itself? What is the probability of 6.5mm stone which is sitting at the neck blocking the bile flow and cause another gallbladder attack and eventually an inflamed one? Can i take that risk? Does it happen even i strictly follow the bland diet?

Thanks Doctor. So no surgery required? What are the chances of smaller stones coming out and blocking the bile duct? What are the chances of 6.5mm stone which is at the neck blocking the bile flow and cause another attack? Can I avoid surgery and go with ayurvedic medication you suggested ?

The history which you provided above is pointing towards the condition like pittashmiri which caused by the vitation of Pitta and kapha dosha lead to the formation of stone in gallbladder 1) varunadi kwath- 15 ml with 45 ml you from water 30 minute before breakfast and dinner 2) avipathi churna - one teaspoon at bed time with warm water 3) guggulutiktakam ghritam- one teaspoon early morning on empty stomach only if you can tolerate it 4) chandraprabha vati - two tablet after meal with warm water 5) patolkaturohinyadi kwath- 15 ml with 45 ml leukaum water 30 minutes before lunch 👇👇👇👇👇👍 Food to Avoid Deep-fried items, heavy creams, and excessive butter/oil (crucial, as these trigger gallbladder contractions). Red meat, paneer, and heavy lentils like Rajma or Urad Dal. Excessive chili, pickles, fermented foods, and carbonated drinks. Avoid sedentary behavior immediately after eating; do not suppress natural urges

You had Acute Cholecystitis and still have stones (Gallstones). Surgery (gallbladder removal) is the safest and best option now. Stone at neck (6.5 mm) = high risk of blockage again Can cause severe pain, infection, or emergency Ayurveda cannot safely remove stones quickly in this stage What you can do Before surgery Light diet (khichdi, soups) Avoid oily, fried, fatty food Triphala 1 tsp at night with warm water Arogyavardhini Vati 1 tablet twice daily after food After surgery (recovery) Same medicines for 2–3 weeks Gradually return to normal diet

1.Chandraprabha vati 2 tab twice daily with water after meals 2.Gokshuradi guggulu 2 tab twice daily with water after meals 3.Pashanbhedadi kwath 20 ml with 20 ml water twice daily after meals 4.Punarnava churna 1/2 tsp twice daily with honey after meals Diet Stick to light, easily digestible meals (khichdi, vegetable soups, rice gruel). Avoid fried, oily, fatty foods and heavy dairy. Favor steamed vegetables (bottle gourd, pumpkin, carrots) and mild fruits (papaya, apple, pomegranate). Sip warm water and use mild digestive spices (cumin, coriander, fennel). Lifestyle Eat small, frequent meals instead of large portions. Do gentle walking or yoga; avoid strenuous exercise until recovery. Practice pranayama (Anulom Vilom, Bhramari) for stress relief. Ensure 7–8 hours of sleep; avoid late-night eating. After surgery, reintroduce foods gradually, starting with soups and soft grains.

In your case, since there was acute cholecystitis + stone at neck, the situation is sensitive. Even though inflammation has reduced, the stone at neck can again block and cause severe pain. Ayurveda can support, but it is not advisable to delay surgery in such cases if your doctor has recommended it, especially after an acute attack. You can still follow supportive measures to reduce recurrence risk and support liver–gallbladder function. Diet is most important: Avoid oily, fried, cheese, butter, and heavy foods. Take light, low-fat meals. Eat small frequent meals instead of heavy meals. At home: Warm water regularly. You can take a small amount of turmeric in food. Avoid long fasting. • Arogyavardhini vati – 1 tablet twice daily after food • Punarnava mandoor – 1 tablet twice daily after food These help in liver function and reducing inflammation supportively. If you are planning to postpone surgery, it should be only under close medical supervision, as recurrence can be sudden. After surgery also, Ayurveda can help in digestion and prevention. Regards, Dr Raghuveer (Ayurvedacharya)
