••From an Ayurvedic perspective, this case presents a classic, complex manifestation of a Vata-Pitta Pradhan Vyadhi with underlying Agni Mandya (depleted digestive fire) and Dhatu Kshaya (tissue depletion ••Symptom Correlation & Analysis A. Scalp & Hair Changes (Seborrheic Dermatitis & Hair Fall) ••The Paradox (Oily Scalp vs. Dry, Frizzy, Wavy Hair): This occurs when vitiated Vata dries up the internal moisture (Daruna), while a reactive localized Kapha-Pitta imbalance creates an oily, inflamed scalp ecosystem (Seborrheic Dermatitis, akin to Darunaka or Kshudra Roga). ••Porosity Issues: The fact that the hair “doesn’t get easily wet or dry” indicates high structural porosity and a damaged cuticular layer caused by severe Rasa-Asthi Kshaya. The hair has lost its Snigdha (unctuous) and Slakshna (smooth) qualities. B. Nasal Allergies & Blockage Vata-Kapha Pradhan Pratishyaya: Morning sneezing and watery discharge indicate a hypersensitive Pranavaha Srotas. ••Night Blockage: The nocturnal blockage without heavy congestion points toward a Vata-type swelling/reactivity of the nasal mucosa rather than a thick Kapha accumulation. C. Low BMI (40 kg) & LPP History ••Karshya (Emaciation): Skipping lunch directly starved the Agni. When Rasa Dhatu is depleted, weight loss is immediate. ••Lichen Planus Pigmentosus (LPP): LPP is a Bhrajaka Pitta and Rakta vitiation. Even though it is inactive, the underlying Rakta Dusti tendency remains, meaning we must be careful not to use overly heating (Ushna-Tikshna) herbs. ••Agni Deepana & Rasa Prasadana (Igniting Digestion) Before giving heavy nourishing foods or tonics, the digestion must be corrected to prevent ama (toxin) formation. •Panchakola Churna or Ashta Churna: 1 text to 2 grams with warm water 15 minutes before lunch and dinner to kindle the Agni. •Draksharishta: 15 ml with equal water post-meals. It is an excellent Rasa-Rakta builder, improves appetite, and gently pacifies Vata without increasing Pitta. Step 2: Shiro-Roga & Kesha Chikitsa (Scalp & Hair Management) Because the scalp is oily/inflamed (Seborrheic Dermatitis) but the hair strands are extremely dry, strong drying anti-dandruff herbs or heavy oils should not be used simultaneously. Internal: ••Gandhak Rasayan: 250 mg twice daily after food (addresses the fungal/inflammatory element of Seb Derm and ensures LPP stays dormant). ••Saptamrit Lauh or Narasimha Rasayana: Excellent for reversing Kalitya (hair fall) and restoring the smooth, silky texture of the hair strands by strengthening Asthi Dhatu. ••External Scalp Application: Avoid thick, heavy oils while the scalp is actively oily/flaky. Instead, advise a lightweight formulation like Dhordhorpatradi Keratailam or an oil infused with Neem, Karanja, and Triphala. Apply only to the scalp 1 hour before washing, 2–3 times a week. ••Hair Strand Care: Use a gentle, sulfate-free herbal hair cleanser (Shikakai, Reetha, and Amalaki base). A post-wash rinse with a lukewarm Methi (Fenugreek) and Hibiscus leaf decoction will help close the hair cuticles, reducing friziness and correcting the porosity issue. ••Step 3: Pranavaha Srotas Chikitsa (Nasal Allergies) •Haridra Khanda: 1 teaspoon (5 grams) with lukewarm milk or warm water on an empty stomach every morning. It is the gold standard for allergic rhinitis (Pratishyaya). ••Laxmivilas Ras (Nardiya): 1 tablet twice daily to clear the night-time nasal blockages and strengthen respiratory immunity. ••Pratimarsha Nasya: 2 drops of Anu Taila or warm Shadbindu Taila in each nostril, but only in the afternoon when the morning sneezing spell has completely subsided. Step 4: Brimhana (Healthy Weight Gain) Once her digestion stabilizes (usually after 2–3 weeks of Deepana-Pachana): •Ashwagandha Lehyam or Amrit Prasham: 1 teaspoon at bedtime with warm milk to address the low BMI and provide Balya (strength) to the depleted tissues. 4. Pathya-Apathya (Diet & Lifestyle Dietetics) The Golden Rule for her: Regularity is medicine. She must strictly avoid skipping meals. Pathya (Favored): ••Diet: Warm, freshly cooked, easily digestible foods. Ghee (clarified butter) must be included in her daily diet to pacify Vata and nourish the hair roots. Shashtika Shali (red rice), green gram (moong dal), pomegranate, and soaked almonds. ••Lifestyle: Abhyanga (body massage with Sesame oil before bath to combat low BMI and Vata), head covering when exposed to dust/pollution, and maintaining a strict, fixed sleep and meal schedule. ••Apathya (To be Avoided): •Dry, cold, stale, or packaged processed foods (Ruksha-Sheeta Ahar). •Excessive use of spicy, sour, and fermented foods (to prevent re-triggering Lichen Planus Pigmentosus). ••Washing hair with excessively hot water (destroys hair cuticles and worsens frizz). Day sleeping (Diva-Svapna) and staying up late at night (Ratri-Jagarana), which further imbalances Vata and Kapha

The combination of low body weight, meal irregularity history, chronic rhinitis, and altered hair texture suggests that nutritional restoration and correction of Vata-Pitta imbalance are essential. Hair fall may improve within 3–6 months, while recovery of hair quality and density usually takes longer (6–12 months). A detailed assessment of digestion (Agni), sleep, menstrual history, stress levels, and current medications would help individualize the Ayurvedic prescription. ----Rx: 1) Tab. Arogyavardhini Vati – 1 tab BD after meals. 2) Cap. Bhringaraj – 1 cap BD after meals. 3) Syr. Draksharishta – 15 ml + equal water BD after meals. 4) Anu Taila Nasya – 2 drops in each nostril morning after face wash. 5) Neelibhringadi Taila – scalp application 2–3 times/week, keep for 1–2 hrs before wash. ----Pathya: Regular meals; avoid skipping food. Include ghee, soaked almonds, black raisins, dates, milk (if suitable). Adequate protein intake and proper sleep. Avoid excess spicy, fried, packaged foods and late nights. ----Advice: CBC, Ferritin, Vitamin D3, Vitamin B12, TSH evaluation if not done recently. Follow up after 4–6 weeks.

This case looks like a combined Kapha–Pitta imbalance with Vata depletion (dhatu kshaya)—reflected as oily seborrheic scalp, hair thinning, dryness/frizz, low BMI, and long-standing allergic rhinitis. In Ayurveda, this is not just a scalp problem; it is a whole-body nutrition + immunity + nasal channel (pranavaha srotas) disturbance. Core Ayurvedic Understanding Oily scalp + dandruff (Seb derm) → excess Kapha + Pitta in scalp Hair fall + dry, weak hair → Rasa & Asthi dhatu depletion (Vata increase) Nasal allergy, morning sneezing, watery discharge → Pratishyaya (Kapha-Vata imbalance) Low BMI (40 kg) → weak digestion (Agni mandya) + poor tissue nourishment So treatment must focus on 3 pillars: Deep nourishment (Brimhana) Detox + scalp balance (Shodhana) Allergy control + nasal healing Ayurvedic Management Plan 1) Internal Healing Build weight, strength, and hair roots from inside. Morning Warm water + 1 tsp Amla powder (or Amla juice diluted) OR Chyawanprash 1–2 tsp daily Diet (very important for BMI & hair) Increase: milk, ghee, rice, dal, banana, dates, almonds, sesame Add: cow milk + turmeric at night Protein: moong dal, paneer, curd (if suitable) Avoid: junk food, cold drinks, excessive tea/coffee, fasting/skipping meals Key rule: No meal skipping again — it directly worsens hair loss in Ayurveda 2) Rx Bhringraj – best for hair fall & follicle strength Amla – cooling, reduces scalp inflammation Ashwagandha – improves weight, stress hair fall Triphala – gut cleansing, improves absorption Yashtimadhu (licorice) – helps nasal allergy & inflammation These work best when digestion improves. 3) Scalp Therapy (Kapha-Pitta balance) Oil massage (2–3 times/week) Neelibringadi Tailam or Bhringraj oil Warm slightly and massage gently for 10–15 min Hair wash Mild herbal shampoo or reetha + shikakai Avoid harsh chemical shampoos (they worsen dryness + frizz) For seborrheic oily scalp Apply paste of multani mitti + neem water (once weekly) Helps reduce excess oil and dandruff naturally 4) Nasal Allergy Treatment Daily Nasya (morning) 2 drops Anu Taila in each nostril Do after warm steam inhalation Steam therapy Inhale steam with ajwain or tulsi leaves 3–4 times/week Herbal support Sitopaladi churna + honey (small dose) for allergy tendency 5) Lifestyle Correction (Break the root cause) Sleep on time (hair regrowth depends on this) Avoid late night screen exposure Regular sunlight exposure (Vitamin D support) Gentle yoga: pranayama (Anulom Vilom, Bhramari)

Based on the symptoms, an Ayurvedic approach may focus on improving nutrition, digestion, and balancing Kapha-Pitta involvement in the scalp and allergic tendencies. Take Arogyavardhini Vati – 1 tablet twice daily after food. Mahamanjisthadi Kwath – 15 ml with equal water twice daily before food. Bhringarajasava – 15 ml with equal water twice daily after food. Anu Taila – 2 drops in each nostril every morning. Bhringraj Taila or Neelibhringadi Taila – apply to scalp 3 times weekly. Diet Do not skip meals. Increase protein intake (milk, ghee, mung dal, paneer, nuts if tolerated). Include amla, black sesame seeds, dates, and leafy vegetables. Avoid excessive spicy, fried, and packaged foods.
