In addressing the persistent skin patches and itching after long-term filariasis and cellulitis, we need to refine the approach from an Ayurvedic perspective. The issues you’ve mentioned indicate a potential Kapha and Pitta dosha imbalance, which may have resulted in the stagnation of lymph and impure blood (Rakta dhatu). Here’s a suggestion for treatment that might align better with the patient’s current concerns.

Firstly, for detoxification and improving blood purity, leveraging the benefits of Triphala Guggulu could be beneficial. It helps in cleansing the lymphatic system and can support the reduction of swelling. Make sure the patient takes it with lukewarm water after meals twice daily.

Considering the itching sensation, internally introducing Chandraprabha Vati can aid in addressing the urinary tract and detoxifying the body, helping with any underlying heat (Pitta) that’s causing the itchiness. 1-2 tablets twice a day after meals can be practical.

For topical application, instead of Kasisadi Taila alone, consider switching to Panchavalkala Kashaya for washing the affected area. It effectively soothes inflammation and reduces patches. This can be followed by an application of Jatyadi Taila, known to assist in skin healing.

In time, Ashwagandha Leham, a herbal jam, might offer rejuvenating effects, aiding tissue repair. 1-2 teaspoons in milk once daily can suffice.

Ensure the patient adheres to a Kapha-Pitta balancing diet, avoiding heavy, spicy, oily foods. Encourage more green vegetables, bitter gourd, and fluids like warm water with lemon. Lifestyle interventions like taking walks, reducing stress through Pranayama, and maintaining a consistent sleep schedule will also play a supportive role.

Do continue monitoring the patient’s progress and make adjustments as needed. If symptoms persist, evaluating the case further with possible bi-dosage diagnostic tools or seeking a second opinion could be wise.
