HELLO, You have Graves disease, an autoimmune condition where the body attacked the thyroid, causing excess hormones. After surgery the thyroid gland was completely removed so -your body cannot produce thyroid hormone naturally -you are fully dependent on thyroxine for life -Your current state is called post thyroidectomy hypothroidism In Ayurveda, this condition is understood as -Agni imbalance (metabolic fire disturbance) -Kapha vata predominance -loss of an important endocrine organ, so treatment focuses on supporting metabolism, immunity, digestion, liver and mind not stimulating the thyroid (because it is absent) NEEM POWDER AND THYROID- CLEARLY Daily neem powder is NOT advised in your case WHHY 1) neem is bitter , dry, and has cooling properties -> in long term daily use, it suppresses metabolism

2) Neem has anti thyroid and immune modulating actions -it may interfere with thyroxine absorption -it can reduce circulating thyroid hormone activty 3) You do not have a thyroid gland -Any herb tha reduces or alters thyroid hormone action can -increase fatigue -worsen brain fog -distrub TSH balance -create hormone instability despite correct thyroxine dose 4) Lon term neem use risk -liver stress -excessive dryness -reduced fertility hormones CONCLUSION -do not take neem powder daily -no tsp dose is recommended for you Oonly short term use is advised if there is specific infection or skin condition TREATMENT GOALS -support metabolism -improve thyroxine utilization -prevent weight gain, fatigue, dryness -support liver and digestion -maintain mental calm and immunity -prevent autoimmune flare tendencies SAFE AND BENEFICIAL MEDICATIONS 1) TRIPHALA CHURNA= 1/2 tsp at night with warm water =improves digestion, prevents constipation, supports gut hormone balance, gentle detox without hormone suppression 2) GUDUCHI CAPSULE= 1 cap once daily after meals =immuno modulator (not suppressant like neem), supports liver and metabolism, prevents autoimmune flare tendency 3) ASHWAGANDHA CAPSULE= 250mg once daily after dinner =supprts energy, improves sleep , helps hormone receptor sensitivity 4) JEERAKA WATER= boil 1/2 tsp cumin seeds in 1 L water- sip warm through the day =improves digestion, reduces bloating, supports metabolism gently EXTERNAL THERAPIES 1) OIL MASSAGE = SESAME OIL 3-4 times/week =reduces dryness, improves circulation, calms vata (common after thyroid removal) YOGA AND PRANAYAM -surya namaskar= 6 -8 rounds -bhujangasana -setu bandhasana -viparita karai Avoid aggressive neck pressure (no long sarvangasana) PRANAYAM -Anulom vilom= 10 min daily -Bhramari= 5 rounds -Ujjayi= helps metabolism DIET -warm, cooked meals -moong dal, massor dal -steamed vegetables -cooked greens -soaked nuts almond, walnut LIMIT -excess raw salads -soy In excess -cold smoothies daily -very bitter herbs- neem, karela daily SPICES -cumin -coriandr -dry ginger -turmeric small amount HOME REMEDIES -morning= warm water + few drops lemon -Weekly= light fasting with khichri -Sleep= fixed sleep wake time Neem is powerful medicine, not a daily supplement In your case, stability is more important than detox Ayurveda works best here as supportive care alongside thyroxine never stop or alter thyroxine Gentle, nourishing, metabolism- supporting herbs are best for you DO FOLLOW HOPE THIS MIGHT BE HELPFUL THANK YOU DR. MAITRI ACHARYA

Since you have no thyroid gland post-surgery (Graves’ disease) and are on stable lifelong thyroxine (150 mcg), your metabolic balance is maintained externally. In Ayurveda, thyroid disorders — especially post-surgical hypothyroid states — involve Vyana Vata and Udana Vata imbalance, along with Agni disturbance. About Neem Powder in Your Case: Neem is: Tikta (bitter), Kashaya (astringent) Sheeta (cold), Ruksha (drying) → These qualities aggravate Vata, especially Vyana Vata, which is already vulnerable in thyroid conditions. So daily neem powder is NOT advisable for you — especially long-term — as it may worsen: Fatigue Dryness Circulatory imbalance Nervous system symptoms Even though neem is excellent for skin, blood purification, and infections, it is not suitable in thyroid-absent patients unless specifically indicated and combined with Vata-balancing herbs. If your goal is: Skin detox / immunity → Prefer Manjistha, Guduchi- 1/4 tsp Before food with warm water General metabolic balance → Triphala (1/4 tsp at night with warm water) Inflammation control → Amalaki Juice or Nisha Amalaki churna or tablet (Churna- 1/2 tsp morning and night before food with warm water or 1 tablet Three times a day Before food.) These are Vata-neutral or Vata-pacifying, unlike neem. Important Continue thyroxine as prescribed — Ayurveda does not replace thyroid hormone replacement in post-thyroidectomy cases.

You can take neem cautiously, but: Dose: Start small — ½ to 1 teaspoon daily. Timing: Take at least 3–4 hours after thyroxine. Monitor: Check thyroid levels (TSH) 6–8 weeks after starting. Avoid neem interfering with your thyroid hormone absorption. -Neem may reduce how much thyroxine your body absorbs if taken too close to the medication. This can make your thyroid hormone less effective. - Neem might influence liver enzymes that process medications, which could change how thyroxine works in your body. - so take only in small amount and low dose

Hello I can understand your concern. But dont worry we are here to help you out 😊 YOUR CONCERN – Age: 43 – Problem: No thyroid due to surgery for Graves’ disease – Meds: 150 mcg Thyroxine daily (since '08) – Diet: Vegan/vegetarian, eats healthy – Energy Level: Okay AYURVEDIC UNDERSTANDING Since you don’t have a thyroid, you’re totally dependent on thyroxine from meds. In Ayurveda, this means: – Your metabolic fire (Agni) is off, especially in your tissues and fat. – Pitta and Vata energies are messed up. – Your cells aren’t doing their job metabolically as well as they should. So, anything that slows down your metabolism or messes with your hormones needs to be used carefully. IS NEEM OKAY FOR YOU? Neem is: – Super cooling – A good blood cleanser – Good at calming Pitta and Kapha If you don’t have a thyroid, you shouldn’t take neem every day in large amounts because: – It might further slow down thyroid hormone activity – It could mess with how you absorb thyroxine – It might make you tired, slow down your metabolism, and give you the chills So, I don’t suggest taking neem powder daily. How to Use Neem Safely (If You Need It): If you’re thinking of using नीम for: – Boosting immunity – Improving your skin – Detoxing Then do this: Neem powder: Just ¼ teaspoon Only 2–3 times a week After lunch With warm water ❌ Not every day ❌ Not on an empty stomach ❌ Not at the same time as thyroxine Make sure there’s a 4–5 hour gap between taking neem and thyroxine. Better Options for You: Since you don’t have a thyroid, these herbs are safer and can help more: 1.**Guduchi (Giloy)**1 capsule morning Helps immunity without messing with thyroid levels 2. Amalaki churna 1 tsp with warm water once a day (Supports metabolism and gives you antioxidants) 3.Triphala churna 1 tsp at bed time (Helps detox gently without messing with your hormones) Important To Monitor: Because you’ll be on thyroxine for life: – Get your TSH, T3, and T4 checked every 6 months – If you start any herbal cleanse, get checked after 6–8 weeks My Advice: Don’t take neem powder every day in your situation. You can take it sometimes (2–3 times a week in small doses) if you need it, but safer herbs like Triphala + Guduchi are better for the long haul. Warm Regards Dr. Snehal Vidhate

Neem is not known to interfere directly with thyroxine, but let’s look at it carefully: ⚖️ Suggested General Dosage - Neem powder: ¼ to ½ teaspoon once daily is usually considered sufficient for maintenance. - Anupana (vehicle): Take with lukewarm water or honey. - Best timing: Morning empty stomach or after meals if digestion is sensitive. ✅ Practical Guidance for You - Since you are vegan/vegetarian with a healthy diet, neem can be a supportive addition for skin, digestion, and immunity. - Avoid taking large doses (1–2 tsp daily) long term, as it may cause excessive cooling and weakness. - Monitor how your body responds—if you notice fatigue, loose stools, or excessive dryness, reduce or stop. - Continue regular thyroid monitoring (TSH, T4) as neem does not replace thyroxine. WARM REGARDS DR. ANJALI SEHRAWAT

IT IS GOOD THAT YOUR THYROXINE DOSE IS STABLE AND YOU ARE NOT HAVING SIDE EFFECTS NEEM IS A STRONG HERB AND IT HAS COOLING DETOXIFYING AND METABOLISM AFFECTING ACTION SO IT SHOULD NOT BE TAKEN DAILY FOR LONG TERM IN SOMEONE WHO HAS NO THYROID GLAND AND IS FULLY DEPENDENT ON THYROXINE NEEM CAN INTERFERE WITH METABOLIC BALANCE AND MAY ALTER HOW YOUR BODY UTILIZES THE HORMONE EVEN IF BLOOD REPORTS REMAIN NORMAL FOR SOME TIME IF NEEDED NEEM CAN BE USED ONLY FOR A SHORT DURATION FOR SPECIFIC REASONS LIKE SKIN ISSUES INFECTIONS OR SUGAR IMBALANCE AND EVEN THEN THE DOSE SHOULD BE VERY SMALL USUALLY ONE FOURTH TEASPOON ONCE DAILY AFTER FOOD FOR NOT MORE THAN TWO TO THREE WEEKS IT SHOULD NEVER BE TAKEN ON EMPTY STOMACH AND SHOULD NOT BE CONTINUED DAILY AS A HEALTH TONIC IN YOUR CASE FOR LONG TERM SUPPORT IT IS BETTER TO FOCUS ON DIGESTION LIVER SUPPORT AND NERVOUS SYSTEM RATHER THAN STRONG BITTER HERBS IF YOU WANT IMMUNITY OR CLEANSING SUPPORT THERE ARE GENTLER OPTIONS THAT DO NOT DISTURB THYROID HORMONE ACTION NEEM DAILY AND LONG TERM IS NOT ADVISED IN YOUR CONDITION EVEN WITH A HEALTHY DIET

Yes you can safely take neem powder daily. Neem is cooling (sheet virya), bitter, blood-purifying, anti-inflammatory & supports skin/liver/immunity. It does not interfere with thyroxine absorption or thyroid function when taken in moderate doses (no known interaction). ✓Recommended Dosage ½ – 1 teaspoon (2–4 grams) once daily ,morning or evening Best way: Mix in warm water or add to smoothie/curd/vegetable juice Start with ½ tsp for first 7–10 days then increase to 1 tsp if no stomach upset Take after food (never empty stomach) to avoid mild nausea/acidity ✓Duration Safe for long-term daily use (months to years) at this dose Take 5–6 days/week → 1 day break if desired ✓Tips Drink plenty of water (2.5–3 L) Continue thyroxine exactly as prescribed (take on empty stomach in morning) No need to separate neem & thyroxine timing by hours – fine if taken 2–3 hours apart If any discomfort then reduce or stop & consult doctor.
