आपकी चिंता बिल्कुल समझ में आती है। यदि 6 महीने से पीरियड नहीं आया है, तो केवल दवा बदलते रहने के बजाय कारण को ठीक से समझना बहुत जरूरी है। चूँकि आपको पहले से PCOS की हिस्ट्री है, सबसे अधिक संभावना उसी से संबंधित हार्मोनल असंतुलन की है। 2 महीने होम्योपैथिक दवा लेने के बाद भी मासिक धर्म न आना यह संकेत देता है कि अब पुनः मूल्यांकन की आवश्यकता है। मेरे क्लिनिकल अनुभव में आपके मामले में मुख्य संभावित कारण हैं: PCOS (Polycystic Ovary Syndrome) – सबसे सामान्य कारण, जिसमें अंडोत्सर्जन (ovulation) नियमित नहीं होता और पीरियड कई महीनों तक रुक सकते हैं। वजन बढ़ना, इंसुलिन रेजिस्टेंस या मेटाबॉलिक असंतुलन – PCOS के साथ अक्सर जुड़ा रहता है। मानसिक तनाव, चिंता या नींद की गड़बड़ी – हार्मोनल चक्र को प्रभावित कर सकती है। कम संभावना लेकिन जांच योग्य कारण: थायरॉइड समस्या, प्रोलैक्टिन बढ़ना या गर्भावस्था (यदि विवाहिता/यौन सक्रिय हैं तो पहले गर्भावस्था की जांच अवश्य करें)। आयुर्वेदिक दृष्टि से यह स्थिति प्रायः आर्तववह स्रोतस की अवरोध अवस्था, कफ-वात असंतुलन तथा अग्निमांद्य से संबंधित देखी जाती है, जो PCOS में सामान्य है। उपचार के रूप में Rajahpravartini Vati – 1 गोली दिन में 2 बार भोजन के बाद। Kanchanar Guggulu – 2 गोली दिन में 2 बार भोजन के बाद। Kumaryasava – 15–20 ml बराबर पानी मिलाकर दिन में 2 बार भोजन के बाद। यह उपचार सामान्यतः 6–8 सप्ताह तक दिया जाता है, लेकिन आपकी प्रकृति, वजन और PCOS की गंभीरता के अनुसार समायोजन आवश्यक हो सकता है। आहार में: मैदा, मीठा, कोल्ड ड्रिंक्स, बेकरी आइटम और जंक फूड कम करें। गर्म पानी, हरी सब्जियाँ, दालें, तिल, मेथी और फाइबरयुक्त भोजन बढ़ाएँ। प्रतिदिन 30–45 मिनट तेज चलना या योग करें। पर्याप्त नींद लें और देर रात जागना कम करें। पंचकर्म में उद्वर्तन, विरेचन तथा चयनित मामलों में बस्ती चिकित्सा PCOS में अच्छे परिणाम दे सकती है, लेकिन यह प्रत्यक्ष चिकित्सकीय मूल्यांकन के बाद ही करानी चाहिए। मैं आपको सलाह दूँगा कि जल्द से जल्द ये जांचें कराएँ: Pregnancy test (यदि संभावना हो) Pelvic Ultrasound TSH (थायरॉइड) Prolactin Fasting Blood Sugar / Insulin profile यदि पेट में तेज दर्द, अत्यधिक रक्तस्राव, दूध जैसा स्त्राव, या अचानक वजन बहुत बढ़ रहा हो तो स्त्रीरोग विशेषज्ञ से शीघ्र मिलें। सही कारण पहचानकर उपचार किया जाए तो अधिकांश PCOS रोगियों में 1–3 महीने के भीतर मासिक धर्म नियमित होने लगता है। नियमित फॉलो-अप और जीवनशैली सुधार उपचार का महत्वपूर्ण हिस्सा हैं।
