Doctors' responses

Hi! I’m an assistant to the Ask Ayurveda doctors. I’ve carefully reviewed your question.

I will explain your situation through the Ayurvedic perspective so you understand what is happening in your body as a whole.

Ajruna (Terminalia arjuna) is a well-known herb in Ayurveda for supporting heart health. It is traditionally used to balance Pitta and Kapha doshas, which can help manage blood pressure and enhance cardiac function. However, given your specific condition with a pacemaker for Mobitz type II AV block, it is crucial to consider the interaction between Ajruna and your current cardiac management.

In Ayurveda, elevated blood pressure may be linked to an imbalance in Pitta dosha, which manifests as heat and intensity in the body. Ajruna is believed to have cooling and stabilizing effects, potentially supporting the heart by promoting a calming influence on the circulatory system.

However, since you have a pacemaker, it’s essential to ensure that any herbal supplement you take does not interfere with your cardiac device or your overall heart function. Ajruna is generally considered safe, but its interaction with modern medical interventions should be carefully monitored.

To give doctors a precise and effective treatment plan, your answers to the questions below are essential. Without this information, the recommendations will be more general and less accurate.

Please answer each question as clearly as possible:

1. How long have you been taking Ajruna powder? Please specify if it’s been less than a week, 1-4 weeks, 1-6 months, or more than 6 months.

2. What other medications or supplements are you currently taking? Include over-the-counter medications, prescription medications, and herbal supplements.

3. Have you experienced any side effects since starting Ajruna powder? Indicate if you’ve noticed mild gastrointestinal issues, dizziness, lightheadedness, or any other symptoms.

4. How often do you monitor your blood pressure? Is it daily, weekly, monthly, or rarely?

5. What is your typical blood pressure reading? Specify if it’s normal, slightly elevated, high, or if you’re not sure.

6. Have you discussed the use of Ajruna powder with your cardiologist? Indicate if they approved it, had concerns, or if you haven’t asked.

7. How would you describe your overall heart health since the pacemaker was installed? Rate it as excellent, good, fair, or poor, and mention any symptoms you’ve experienced.

Your detailed responses will help ensure that the advice you receive is tailored to your specific needs and circumstances.

This is a critical clinical scenario. In Mobitz Type II Atrioventricular (AV) Block, the conduction system below the AV node is failing intermittently, which is why a pacemaker is the standard of care to prevent sudden cardiac arrest or progression to complete heart block. The Ayurvedic View In Ayurveda, a heart block of this nature is viewed as a profound disturbance in Prana Vayu (the energy governing the heart’s rhythm) and Vyana Vayu (the energy governing circulation). Srotas Avarana (Channel Obstruction): The electrical pathways (Manovaha and Hridaya Srotas) are obstructed, often by a combination of Kapha (structural/heaviness) and Ama (toxins), which slows the “Tejas” or electrical impulse of the heart. Hridaya Daurbalya: The cardiac tissue itself lacks the “Bala” (strength) to conduct impulses effectively. Arjuna’s Role: Arjuna (Terminalia arjuna) is a Hridya (cardiotonic) and a Sandhaniya (healing/joining) herb. It strengthens the cardiac muscle (Hrid-mamsa) and improves stroke volume. Safety with a Pacemaker Yes, it is generally safe. Arjuna does not interfere with the mechanical function of a pacemaker. In fact, since a pacemaker manages the rhythm, Arjuna focuses on the tissue quality and muscular strength of the heart. It acts as a “trophic” support for the myocardium. However, there is a caution: Arjuna has mild “Hridaya-shanti” (calming) and hypotensive properties. Since your pacemaker is specifically managing a conduction block, you must ensure that your heart rate and blood pressure remain stable. As a doctor yourself, you know that while the pacemaker provides the “spark,” Arjuna provides the “fuel” for the cardiac cells. Suggested Management from Kerala Ayurveda For someone with a pacemaker and a history of conduction issues, the focus should be on nourishment without over-sedation. 1. Arjunarishtam This is the most stable way to consume Arjuna. The fermentation process makes the active principles (arjunolic acid and flavonoids) more bioavailable and less likely to cause constipation than the raw powder. Dose: 20 ml mixed with an equal amount of lukewarm water, taken twice daily after meals. 2. Parthadyarishtam This is a specific Kerala Ayurveda variant of Arjunarishtam. It is specifically indicated for “Hridroga” (heart diseases) and helps in maintaining the tone of the cardiac muscles. Dose: 15 to 25 ml twice daily after food with an equal quantity of water. 3. Hridayamrit Vati This is a proprietary blend often used to strengthen the heart and clear the “Avarana” (blockage) in the channels. It supports the electrical stability of the heart. Dose: 1 tablet twice daily after food with warm water. 4. Ksheerabala 101 Capsules While not a “heart” medicine per se, Ksheerabala is a potent Vata-pacifier and nerve tonic. Since the conduction system is neurological in nature, this helps nourish the “Majja” (nerve) element of the heart. Dose: 1 capsule twice daily before food with warm water. Clinical Recommendations for You Avoid Raw Powder (Churna): Arjuna powder can be very “Rooksha” (dry) and “Kashaya” (astringent). In Vata-dominant conditions like a heart block, the fermented Arishta or a Ghritham (medicated ghee) is superior to raw powder as it doesn’t aggravate Vata. Monitor Electrolytes: Ensure your Potassium and Magnesium levels are optimal, as these are the physical counterparts to Prana Vayu in cardiac conduction. Abhyanga: Daily gentle application of Dhanwantharam Thailam on the chest area (over the Hridaya Marma) can help calm the Vyana Vayu and support the pacemaker’s environment.

arjuna powder is not unsafe in general But In your specific condition, it should be used very cautiously and only under supervision arjuna has cardio-active properties It can: Influence heart conduction Mildly slow heart rate in some people Enhance effects of other heart-related medicines Since your natural rhythm previously dropped to the 40s, your heart is electrically sensitive Even though your pacemaker is working well, we should not stress the conduction system unnecessarily So Avoid it for now

With a pacemaker for Mobitz type II AV block, you need to be careful with any herb affecting the heart. Arjuna is generally good for heart support, but in your case it should be taken in controlled dose and with monitoring. Do this: Continue regular follow-up with your cardiologist Monitor BP and pulse regularly (at least 2–3 times per week) Avoid self-increasing dose or combining multiple heart herbs Diet: Reduce salt intake Avoid processed and packaged foods Include fruits, vegetables, garlic, and small amount of ghee Maintain good hydration Lifestyle: Daily walking 30 minutes Avoid stress and overexertion Proper sleep Medicine: Arjuna churna 3 grams once daily with warm water after food (morning) Do not take higher doses. If you notice dizziness, unusual fatigue, very low pulse, or discomfort, stop and consult immediately. Since you already have a pacemaker, priority is stable heart rhythm, not aggressive BP reduction. For proper monitoring and safe combination with your condition, you can consult me.

While Arjuna is an excellent her for general heart health but it is not recommended to use it against the explicit concern of your cardiologist especially with the history of heart block and a active pacemaker I strongly recommended you that your cardiologist understand the electrical mapping of your heart please follow their guidance over general herbal advise rather than folk also focus on the lifestyle and your food management in your daily routine As you current readings are pointing towards pre hypertension or stage1 so correct your Lifestyle alternatives for this that carry less risk of cardiac interference then Arjuna powder Reduce salt intake gentle breathing exercise like anulom vilom everyday for 10 to 15 minutes
