••First, as a fellow colleague in the medical fraternity, I want to acknowledge how methodically you have tracked your vitals and lab markers. It is excellent that major anatomical complications have been ruled out via the CT scan, endoscopy, and sigmoidoscopy, and that your total testosterone naturally bounced back to normal parameters (473 ng/dL and a healthy free T profile). ••Here is a clinical, step-by-step approach to structuring the formulation regimen you ordered, focusing on Deepana-Pachana (kindling digestion and clearing Ama) first, followed by Rasayana (rejuvenation). Phase 1: Correcting Agni & Restoring the Gut (Weeks 1–3) Do not take all the medicines at once. We must first fix the colon and tongue coating before building up reproductive strength, otherwise, the heavy herbs will cause further Ama and weaken your digestion. The Digestion Lineup ••Chitrakadi Vati: •Dosage: 1 tablet twice a day. •Timing: 15 minutes before lunch and dinner. •Anupana (Vehicle): Warm water. •Purpose: Highly potent for Deepana-Pachana. It will scrape the yellow Ama coating off your tongue, reduce acid reflux, and kindle the metabolic fire. ••Bilwadi Churna + Kutajghan Vati: •Dosage: 2–3 grams of Bilwadi Churna mixed with 1 tablet of Kutajghan Vati (crushed). •Timing: Twice a day, after meals. •Anupana: Warm water or fresh buttermilk (Takra) if it suits you. •Purpose: Kutaja and Bilwa are the premier choices for post-dysentery recovery. They absorb excess moisture, bind the unformed stool, and heal the intestinal mucosa without causing absolute constipation. ••Kutajarishta: •Dosage: 15 ml (approx. 3 teaspoons) mixed with an equal quantity (15 ml) of lukewarm water. •Timing: 20 minutes after lunch and dinner. Purpose: Fermented preparation that acts as a natural probiotic to restore gut flora, toning the colon to help with that “sluggish/need a push” sensation. ••Note on Vidangarishta: Hold off on Vidangarishta for now. It is excellent for clearing residual pathogens or worms, but Kutajarishta is far more specific to your current unformed stool and post-colitis state. Phase 2: Vitality & Vata Restoration (To be introduced gradually) Because your Apana Vata is sluggish and your tissues lack tone, you need subtle nourishment. However, pure Shilajit and Shatavari can be heavy (Guru) to digest. Introduce these starting in Week 2, only if your tongue coating begins to clear and stools start taking a formed shape: ••Ashwagandharishta: •Dosage: 15 ml mixed with 15 ml water. •Timing: Take this combined with your Kutajarishta after meals (you can mix 7.5 ml of each with equal water, or transition to Ashwagandharishta fully once stools are completely normal). •Purpose: Calms the nervous system, relieves general physical fatigue, and addresses the neurological dullness in the bladder. ••Shilajit Pure Resin: •Dosage: A pea-sized amount (approx. 150–200 mg). •Timing: Once a day, early morning on an empty stomach. •Anupana: Lukewarm water or lukewarm milk. •Purpose: Directly addresses the tissue laxity (Shathilya) in the scrotum and testicles. It is a powerful Prana provider that will correct the Vitamin D insufficiency (26 ng/mL) by enhancing mineral absorption and boosting mitochondrial energy. ••Shatavari Churna: •Dosage: Half a teaspoon (approx. 2 grams). •Timing: At bedtime. •Anupana: Warm milk. ••Purpose: Nourishes the Shukra Dhatu and acts as a cooling agent to soothe the acid reflux that travels to your throat. Pelvic Floor & Physiotherapy Guidelines Since your abdominal scans are clear, the “numb bladder urge” and “loose scrotal tone” point directly to a functional coordination issue in the pelvic floor muscles, likely triggered by the pelvic inflammation during the Shigella infection. ••Ashwini Mudra / Gentle Kegels: Do not over-strain, but practice mild contraction and release of the perineum muscle (the space between the anus and genitals). Pull inward gently for 3 seconds, then completely relax for 5 seconds. Do this for 5 minutes in the morning. This brings blood flow back to the pelvic plexus and revives the nerve pathways. ••Vajrasana (Thunderbolt Pose): Sit in Vajrasana for 5–10 minutes immediately after eating. It redirects blood circulation from the lower limbs straight to the digestive tract and pelvic floor, aiding both Agni and Apana Vata. ••Mula Bandha / Pelvic Bridge Pose (Setu Bandhasana): Gently strengthens the glutes and lower pelvic floor, helping restore the physical tone of the reproductive organs. Dietary Rules for the Next 14 Days ••Avoid: Raw salads, cold drinks, heavy dairy (cheese/paneer), and deep-fried foods. They will immediately worsen the tongue coating and unformed stool. ••Favor: Warm, freshly cooked, semi-solid food. Use Cumin (Jeera), Grated Ginger, and Asafetida (Hing) in your cooking to aid the Chitrakadi Vati. ••Hydration: Drink only lukewarm water throughout the day to flush out Ama
