Stool changes: Completely unformed, poor texture, and darkish blackish-brown color. It was yellow just a week ago during the active infection, but changed color and texture right after finishing the antibiotics. I do not have severe constipation, but I need a little push even to pass this unformed stool. I have no stomach pain.Bladder changes: My urge to pass urine feels a little dully or numb.Tongue changes: Yellow coating on the backside of my tongue, with less coating on the front.Throat changes: I sometimes experience acid reflux up into my throat.Sexual changes: Absolute zero libido (no sex drive), loose scrotum with poor tone, small soft testicles, and very weak erections if they come at all.Energy levels: Too much persistent body weakness and general physical fatigue . My Vitamin D level is 26, which is insufficient Antibiotic Completed: I successfully completed my 5-day course of Sulfatrim DS 800+160mg for the Shigella bacteria.Testosterone Recovery: My total testosterone was low at 5.17 nmol/L in early March, but it bounced up to 473 ng/dL in mid-March without any medicine. My April Free Testosterone is perfect at 22.80 pg/mL.Hormones & Blood: LH is 3.07 mIU/mL, Prolactin is 7.64 ng/mL, Haemoglobin is perfect at 15.4 g/dl, Vitamin B12 is 379 pmol/L, and Ferritin is 53 ug/L.Scans & Heart: Echocardiogram shows a healthy Ejection Fraction of 59%. Abdominal CT scan, upper endoscopy, and sigmoidoscopy are all completely normal. I ordered these from india after posting last question . I want to know the directions on how to use these to complete my recovery , i m struggling from over few months . Only problem left is general weakness unformed stool tongue coating bad breath and softness in reproductive organs and very weak sexual function For Digestion/Infection: Kutajghan Vati, Kutajarishta, Vidangarishta, Chitrakadi Vati, and Bilwadi Churna. For Strength/Vitality: Ashwagandharishta, Shilajit Pure Resin, and Shatavari Churna. Please let me know how to use these, which one to use or if physio or streches is useful

••First, as a fellow colleague in the medical fraternity, I want to acknowledge how methodically you have tracked your vitals and lab markers. It is excellent that major anatomical complications have been ruled out via the CT scan, endoscopy, and sigmoidoscopy, and that your total testosterone naturally bounced back to normal parameters (473 ng/dL and a healthy free T profile). ••Here is a clinical, step-by-step approach to structuring the formulation regimen you ordered, focusing on Deepana-Pachana (kindling digestion and clearing Ama) first, followed by Rasayana (rejuvenation). Phase 1: Correcting Agni & Restoring the Gut (Weeks 1–3) Do not take all the medicines at once. We must first fix the colon and tongue coating before building up reproductive strength, otherwise, the heavy herbs will cause further Ama and weaken your digestion. The Digestion Lineup ••Chitrakadi Vati: •Dosage: 1 tablet twice a day. •Timing: 15 minutes before lunch and dinner. •Anupana (Vehicle): Warm water. •Purpose: Highly potent for Deepana-Pachana. It will scrape the yellow Ama coating off your tongue, reduce acid reflux, and kindle the metabolic fire. ••Bilwadi Churna + Kutajghan Vati: •Dosage: 2–3 grams of Bilwadi Churna mixed with 1 tablet of Kutajghan Vati (crushed). •Timing: Twice a day, after meals. •Anupana: Warm water or fresh buttermilk (Takra) if it suits you. •Purpose: Kutaja and Bilwa are the premier choices for post-dysentery recovery. They absorb excess moisture, bind the unformed stool, and heal the intestinal mucosa without causing absolute constipation. ••Kutajarishta: •Dosage: 15 ml (approx. 3 teaspoons) mixed with an equal quantity (15 ml) of lukewarm water. •Timing: 20 minutes after lunch and dinner. Purpose: Fermented preparation that acts as a natural probiotic to restore gut flora, toning the colon to help with that “sluggish/need a push” sensation. ••Note on Vidangarishta: Hold off on Vidangarishta for now. It is excellent for clearing residual pathogens or worms, but Kutajarishta is far more specific to your current unformed stool and post-colitis state. Phase 2: Vitality & Vata Restoration (To be introduced gradually) Because your Apana Vata is sluggish and your tissues lack tone, you need subtle nourishment. However, pure Shilajit and Shatavari can be heavy (Guru) to digest. Introduce these starting in Week 2, only if your tongue coating begins to clear and stools start taking a formed shape: ••Ashwagandharishta: •Dosage: 15 ml mixed with 15 ml water. •Timing: Take this combined with your Kutajarishta after meals (you can mix 7.5 ml of each with equal water, or transition to Ashwagandharishta fully once stools are completely normal). •Purpose: Calms the nervous system, relieves general physical fatigue, and addresses the neurological dullness in the bladder. ••Shilajit Pure Resin: •Dosage: A pea-sized amount (approx. 150–200 mg). •Timing: Once a day, early morning on an empty stomach. •Anupana: Lukewarm water or lukewarm milk. •Purpose: Directly addresses the tissue laxity (Shathilya) in the scrotum and testicles. It is a powerful Prana provider that will correct the Vitamin D insufficiency (26 ng/mL) by enhancing mineral absorption and boosting mitochondrial energy. ••Shatavari Churna: •Dosage: Half a teaspoon (approx. 2 grams). •Timing: At bedtime. •Anupana: Warm milk. ••Purpose: Nourishes the Shukra Dhatu and acts as a cooling agent to soothe the acid reflux that travels to your throat. Pelvic Floor & Physiotherapy Guidelines Since your abdominal scans are clear, the “numb bladder urge” and “loose scrotal tone” point directly to a functional coordination issue in the pelvic floor muscles, likely triggered by the pelvic inflammation during the Shigella infection. ••Ashwini Mudra / Gentle Kegels: Do not over-strain, but practice mild contraction and release of the perineum muscle (the space between the anus and genitals). Pull inward gently for 3 seconds, then completely relax for 5 seconds. Do this for 5 minutes in the morning. This brings blood flow back to the pelvic plexus and revives the nerve pathways. ••Vajrasana (Thunderbolt Pose): Sit in Vajrasana for 5–10 minutes immediately after eating. It redirects blood circulation from the lower limbs straight to the digestive tract and pelvic floor, aiding both Agni and Apana Vata. ••Mula Bandha / Pelvic Bridge Pose (Setu Bandhasana): Gently strengthens the glutes and lower pelvic floor, helping restore the physical tone of the reproductive organs. Dietary Rules for the Next 14 Days ••Avoid: Raw salads, cold drinks, heavy dairy (cheese/paneer), and deep-fried foods. They will immediately worsen the tongue coating and unformed stool. ••Favor: Warm, freshly cooked, semi-solid food. Use Cumin (Jeera), Grated Ginger, and Asafetida (Hing) in your cooking to aid the Chitrakadi Vati. ••Hydration: Drink only lukewarm water throughout the day to flush out Ama

Based on the information provided, your current picture is very consistent with post-infectious digestive weakness (Agnimandya), residual Ama accumulation, disturbed gut flora after antibiotics, mild Pitta involvement (yellow tongue coating, reflux), and secondary depletion of Ojas/Shukra Dhatu, which is likely contributing to weakness, loss of libido, poor erections, and the feeling of softness in the reproductive organs. The encouraging part is that your CT scan, endoscopy, sigmoidoscopy, echocardiogram, hemoglobin, ferritin, B12, and hormone profile are essentially reassuring. The fact that your testosterone recovered spontaneously also suggests that the primary issue is not severe hormonal failure but rather a systemic recovery problem involving digestion, nutrition assimilation, and tissue replenishment. Ayurvedic Assessment Stage 1: Post-Infection and Post-Antibiotic State The Shigella infection and subsequent antibiotic course likely disturbed: Jatharagni (digestive fire) Grahani function (intestinal absorption) Gut microbial balance Ojas (vital energy) This explains: Unformed stool Tongue coating Bad breath Weakness Reduced appetite regulation Fatigue Stage 2: Dhatu Kshaya Because digestion has been weak for months, proper nourishment of deeper tissues has been impaired. This can manifest as: Low libido Weak erections Loose scrotal tone Lack of sexual thoughts Physical exhaustion Reduced motivation In Ayurveda, this is considered a consequence of weakened nourishment reaching the later dhatus, particularly Shukra Dhatu. Suggested Treatment Strategy Do not start all products together. The biggest mistake is taking many medicines simultaneously, which can worsen Agni and create more Ama. First 4 Weeks: Focus on Digestion Kutajghan Vati 2 tablets twice daily After meals With warm water Purpose: Improves stool formation Supports Grahani Helps normalize bowel function Kutajarishta 15 ml with equal water Twice daily after meals Purpose: Supports recovery after infectious diarrhea Improves intestinal tone Bilwadi Churna 3 grams at bedtime With warm water Purpose: Helps improve stool consistency Reduces residual Ama Chitrakadi Vati Use only if: Poor appetite Heaviness after meals Bloating Dose: 1 tablet before lunch and dinner Avoid if reflux increases. Vidangarishta At present, I would not prioritize it unless there is evidence of parasitic infestation, persistent Ama symptoms, or a physician specifically advised it. After Stool Improves (Usually 3–4 Weeks) Once stool becomes reasonably formed and tongue coating decreases, begin rebuilding strength. Ashwagandharishta 15–20 ml with equal water Twice daily after meals Benefits: Improves strength Supports recovery Helps nervous system adaptation Improves libido gradually Shatavari Churna ½ teaspoon at night With: Warm milk if tolerated OR Warm water Benefits: Supports tissue nourishment Helps restore depleted dhatus Reduces residual Pitta Pure Shilajit Resin Begin only after digestion stabilizes. Dose: Approximately pea-sized amount (250–300 mg) Morning on empty stomach Mixed in lukewarm water Benefits: Improves energy Supports stamina Supports reproductive function Helps post-illness recovery Diet Plan For the next month: Prefer Moong dal khichdi Rice Cooked vegetables Pomegranate Stewed apple Homemade yogurt (if tolerated) Buttermilk with roasted cumin Small amount of ghee Coconut water Warm water throughout the day Avoid Fried food Excess chili Alcohol Cold drinks Excess coffee Fast food Protein supplements until digestion stabilizes For Acid Reflux Avoid lying down within 2–3 hours after meals. Eat smaller meals. Avoid very spicy foods. Sleep with the head slightly elevated if reflux occurs at night. Physical Activity Yes, gentle exercise is highly recommended. Initially: 20–30 minutes brisk walking daily. Light stretching. Deep breathing exercises. After energy improves: Bodyweight squats. Light resistance training. Yoga including Vajrasana, Bhujangasana, and Setu Bandhasana. Avoid intense workouts until stools and energy normalize. Recovery of Libido and Sexual Function Many people expect libido to return before digestion recovers, but Ayurveda views it differently. The sequence is: Agni → Rasa → Rakta → Mamsa → Meda → Asthi → Majja → Shukra If digestion and absorption remain weak, sexual vitality often remains weak despite normal blood tests. Therefore, focus first on: Stool normalization. Tongue coating reduction. Energy improvement. Weight and strength recovery. Sexual function usually follows as the deeper tissues become nourished.

Since investigations are largely normal and the infection has been treated, focus on restoring digestion first. Avoid taking all the medicines together. Take • Kutajghan Vati – 2 tablets twice daily after food for 4 weeks • Kutajarishta – 15 ml + equal water twice daily after food • Chitrakadi Vati – 1 tablet twice daily 15 minutes before meals for 2 weeks only • Bilwadi Churna – 3 g twice daily after food if stools remain unformed After stool improves: • Ashwagandharishta – 20 ml + equal water twice daily after food • Shilajit Resin – 250–500 mg once daily with warm milk in the morning • Shatavari Churna – 3 g at bedtime with warm milk Avoid Vidangarishta unless there is confirmed worm infestation. Supportive measures: • Daily 30–40 min walking and light stretching/yoga • Avoid junk food, alcohol, excess tea/coffee • Take curd rice, pomegranate, moong dal, and well-cooked foods • Correct Vitamin D deficiency as advised by your physician

▪︎In Ayurveda, your current state reflects a condition of Dhatu Kshaya (depletion of tissues) and Agni Mandya (weakened digestive fire) following the stress of infection and the subsequent use of heavy antibiotics. The antibiotics, while necessary for the Shigella infection, have likely disrupted your Jatharagni (primary digestive fire), leaving behind Ama (toxins), which manifests as the yellow tongue coating, malformed stools, and lethargy. Your reproductive symptoms are viewed as a downstream effect of this systemic depletion; when the body lacks sufficient Ojas(vital essence) and nutrition, the Shukra Dhatu (reproductive tissue) is the last to receive nourishment, leading to the low libido and physical weakness you describe. To begin your recovery, you must prioritize the restoration of your digestion before building strength. ▪︎Rebuilding Digestion (Agni Deepan & Pachan) Focus on clearing the Ama and stabilizing your stools. ▪︎Chitrakadi Vati:Take 1 tablet twice daily after meals with warm water. This acts as a potent digestive stimulant to ignite your Agni. ▪︎Kutajghan Vati & Bilwadi Churna: Use these to firm the stools. Take 1 tablet of Kutajghan Vati twice daily. ▪︎Bilwadi Churna can be taken as 1 teaspoon mixed with a little warm water or buttermilk (takra) twice a day. If your stools become too hard, decrease the frequency of these two. ▪︎Kutajarishta & Vidangarishta:These are fermented tonics. Take 15 ml of each, mixed with an equal amount of warm water, twice daily after your meals. The Vidangarishta is specifically beneficial for clearing residual toxicity. ●Building Strength (Rasayana & Vitality) Once your digestion feels more stable (usually after 1–2 weeks), you can introduce the deeper nourishing tonics. ▪︎Ashwagandharishta: Take 20 ml mixed with an equal amount of warm water after dinner. It is a powerful Rasayana that nourishes the nervous system and supports endocrine health. ▪︎Shatavari Churna: Take 1 teaspoon mixed in a cup of warm milk (or water, if milk causes bloating) once daily, preferably in the evening. It is cooling and nourishing, helping to soothe the “heat” and dryness associated with your reflux and fatigue. ▪︎Shilajit (Pure Resin): Start with a very small, pea-sized amount (about 250mg) dissolved in warm water or warm milk in the morning. Shilajit is a potent Yogavahi (catalyst) that enhances the absorption of other herbs and directly supports Shukra Dhatu. ●Lifestyle & Supportive Practices ▪︎Physiotherapy and Movement: While you are feeling weak, avoid heavy lifting or high-intensity workouts. Focus on Yoga specifically for digestion: Pawanmuktasana (Wind-Relieving Pose) and Paschimottanasana (Seated Forward Bend) are excellent for stimulating the abdominal organs and pelvic region. Gentle, consistent walking is better than sporadic, intense exercise right now. ▪︎Dietary Guidance: Eat warm, freshly cooked, and light meals. Avoid raw salads, cold drinks, heavy dairy (unless it is fresh buttermilk), and processed foods for the next few weeks. The yellow coating on your tongue suggests you should avoid heavy oils and complex proteins until your digestion is fully restored. ▪︎Reflux Management:Since you mentioned acid reflux, ensure you do not lie down immediately after meals. Sip on lukewarm water throughout the day rather than cold water. ▪︎Because you are dealing with a complex recovery, please monitor your body closely. If any of these medicines cause bloating, heartburn, or constipation, pause the intake and prioritize simple, easy-to-digest foods like Khichdi (moong dal and rice). If these symptoms persist despite these adjustments, consult here as soon as possible to monitor your Prakriti (constitution) and Vikriti (imbalance) in person is highly advisable.

What you are experiencing is a classic functional collapse of your digestive fire (Agni) and a severe blockage of pelvic nerve circulation (Apana Vata), secondary to the Shigella infection and the heavy antibiotics. The antibiotics wiped out your gut flora, leaving your body unable to process food or absorb nutrients. This creates Ama (metabolic toxins), which is exactly what the yellow tongue coating, bad breath and acid reflux are. Because your gut is inflamed and heavy with toxins, the downward-flowing energy (Apana Vata) that controls the bladder, erections, and bowel movements is blocked and stagnant. This stagnation is why your stools need a push, why your bladder urge is dull, and why your pelvic floor muscles have lost their tone, resulting in soft testicles and zero libido. We must first burn off the toxins, form the stool, and manually stimulate blood flow to the pelvis. ✓Custom Protocol Using Your Medicines 1. Chitrakadi Vati: Take 2 tablets exactly 15 minutes before your lunch and dinner with warm water. This is mandatory to melt the Ama, clear the yellow tongue coating, and stop the acid reflux. Bilwadi Churna: Take 1/2 teaspoon twice a day, 30 minutes after meals. Bilwadi is an astringent that will absorb the excess moisture in your colon and form the dark, unformed stool without completely freezing your bowel movements. Vidangarishta & Ashwagandharishta: Mix 10 ml of each together with 20 ml of warm water. Drink this twice a day, right after taking the Bilwadi Churna. Vidangarishta clears residual infection and toxicity, while Ashwagandharishta begins to gently restore nerve strength and combat your severe fatigue. 2. The Strictly Avoid for Now List Hold the Shilajit and Shatavari: Do not touch these yet. They are incredibly heavy (Guru). If you take them while you have a yellow tongue coating and unformed stools, they will ferment in your gut, worsen the bad breath, and plunge your energy levels even lower. You can only start these once your tongue is 100% pink and clean. Hold the Kutajghan Vati & Kutajarishta: Kutaj is a brilliant anti-diarrheal, but it is extremely drying (Ruksha). Since your stool is unformed but you already need a push to pass it, Kutaj will trap the stool and cause severe, painful constipation. Save these for emergency liquid diarrhea only. ✓Crucial Pelvic Rehabilitation Yes, physical therapy is absolutely necessary right now. Your pelvic floor is effectively asleep. Ashwini Mudra (Kegels): You must actively pump blood back into your reproductive organs. Squeeze your pelvic floor muscles (the ones you use to stop the flow of urine) and hold for 3 seconds, then completely release. Do this 30 times every morning and evening. This will restore the tone to the scrotum and wake up the dormant bladder nerves. Malasana (Yogi Squat): Spend 2 to 3 minutes a day in a deep squatting position. This physically opens the pelvic floor and mechanically aids the colon in passing stool without unnecessary straining. Fix Your Eating Patterns: Your irregular diet is keeping your stomach confused. Eat warm, freshly cooked, easily digestible meals at the exact same times every day to rebuild your digestive rhythm. Regards, Dr. Gursimranjeet Singh Department - Panchakarma

Hello, Based on the extensive testing you have already done, there is no strong evidence of ongoing serious structural disease. Your current picture is more suggestive of -post infectious gut dysfunction after entamoeba / shigella infection and multiple antibiotics -disturbed gut microbiome -residual malabsorption / weak digestion -mild acid reflux -low vitamin D contributing to fatigue and weakness -secondary reduction in libido and erectile quality due to chronic illness, fatigue, weight loss , stress and poor digestion rather than testosterone deficiency The fact that your testosterone, LH, prolactin, hemoglobin, Ct scan, endoscopy, and sigmoidoscopy are normal is reassuring About medicines you purchased Do not take all of them together. That is common mistake and may worsen digestion Phase 1 (4-6 weeks)= Focus on gut recovery 1) Kutajghan vati= 2 tabs twice daily after meals for 6 weeks =best suited for unformed stools, post infectious bowel irritation, and gut inflammation 2) Kutajarishta= 15ml + equal water twice daily after meals for 6 weeks =supports bowel tone and stool formation 3) Bilwadi churna = 3gm twice daily after meals with lukewarm water for 4 weeks =useful loose, poorly formed stools and gut recovery Avoid for now -Vidangarishta -chitrakadi vati At present you have occassional reflux and weakness, These medicines can be too heating and may aggravate acidity Phase 2 after stool improves Once stool becomes formed and tongue coating reduces 1) Ashwagandharishta= 15ml + equal water twice daily after meals for 3 months = supports recovery, energy , libido, muscle strength, ad nervous function 2) Shilajit resin= start with250 mg daily after breakfast with warm milk increase gradually to 500 mg daily for 3 months =useful for fatigue, recovery stamina, and sexual weakness Diet for the next 4-6 weeks -rice -moong dal -well cooked vegetables -pomegranate -ripe banana -stewed apple -homemade curd -buttermilk with roasted cumin Avoid -alcohol -fried foods -excess fat -protein powders -excess coffee -very spicy food Do not continue restricting all fruits and salads forever. Excess restriction often delays recovery Tongue coating an Bad breath often reflects -gut dysbiosis -reflux -delayed digestion Helpful measures -tongue scraping every morning -warm water throughout the day -triphala gargle at night -dental cleaning if not done recently Sexual weakness your hormones profile does not suggest primary testosterone deficiency The following can improve as digestion recovers -libido -scrotal tone -erectile quality -energy Do not judge sexual recovery week to week. recovery often lags digestive recovery by several weeks Is pelvic floor tightness the main cause? Based on the information provided Not strongly Typical pelvic floor dysfunction usually causes -pelvic pain -painful urination -constipation -difficulty emptying bladder Your dominant features are -post infectious bowel changes -fatigue -reflux -weakness Pelvic floor physiotherapy is not the first priority Vitamin D A level of 26 is insufficient- kindly take supplements as advised This step wise approach is generally more effective than taking every digestive and vitality medicines simulataneously Do follow Hope this might be helpful Thank you
