Seeking Holistic Help for Hidradenitis Suppurativa - #52425

Mohammed
$3.51

I am seeking a consultation for Hidradenitis Suppurativa (HS), a condition I have been managing for 10 years. My situation has recently escalated into a severe, widespread outbreak. I currently have active, painful lesions in four major areas: both armpits, the groin, the buttocks, and the infra-abdominal fold (the area below my stomach). The current flare-up is characterized by multiple interconnected spots within each area, resulting in constant, spontaneous drainage of pus and frequent bleeding. This persistent discharge requires multiple dressing changes daily and indicates the presence of deep sinus tracts. In the past, I have tried a wide range of conventional Western treatments, including Oratane (Isotretinoin) and various antibiotics like Doxycycline. Unfortunately, these medications did not provide lasting relief, and I have become increasingly concerned and fearful regarding their long-term side effects. Consequently, I am now focusing on a more holistic approach. My current daily regimen includes Zinc, Giloy (Guduchi), Amla, and Liv.52 DS. While I am taking these to support my liver and immune health, I am particularly worried about the Giloy. As an immunostimulant, I am concerned it might be counterproductive or even risky for an autoinflammatory condition like HS, potentially worsening the overactive immune response.

How often do you experience flare-ups of your condition?:

- Frequently

What triggers your flare-ups the most?:

- No clear trigger

How would you describe your overall stress level?:

- High

Doctors' responses

Hi! I’m an assistant to the Ask Ayurveda doctors. I’ve carefully reviewed your question.

I will explain your symptoms through the Ayurvedic perspective so you understand what is happening in your body as a whole.

Hidradenitis Suppurativa (HS) is primarily seen as an imbalance involving the Pitta and Kapha doshas. Pitta, which governs heat and inflammation, may be excessively aggravated, leading to the painful, inflamed lesions you are experiencing. Kapha, associated with heaviness and stagnation, can contribute to the formation of sinus tracts and pus drainage, reflecting a buildup of toxins (ama) and fluids in the body.

The interconnected spots and persistent discharge indicate a systemic imbalance rather than isolated skin issues. Ayurveda views this as a manifestation of internal disharmony, possibly linked to digestion, stress, and lifestyle factors.

Your concern about Giloy is valid. While it is known for its immunomodulatory effects, its role in auto-inflammatory conditions like HS needs careful consideration. Ayurveda emphasizes the importance of individualized treatment, and in your case, a comprehensive approach focusing on reducing Pitta and Kapha, enhancing digestion (agni), and eliminating toxins may be beneficial.

To give doctors a precise and effective treatment plan, your answers to the questions below are essential. Without this information, the recommendations will be more general and less accurate.

Please answer each question as clearly as possible.

1. How often do you experience flare-ups of your condition? 2. What triggers your flare-ups the most? 3. How would you describe your overall stress level? 4. Have you noticed any correlation between your diet and the severity of your symptoms? 5. Do you experience any digestive issues like bloating, constipation, or irregular bowel movements? 6. How is your sleep quality, and do you notice any changes in your symptoms with variations in your sleep pattern? 7. Have you tried any specific Ayurvedic treatments or lifestyle modifications in the past, and what were the outcomes?

I would like to suggeste you to have a combined approach to the treatment so that we can lower the intencity, the incidence of occurance. Like you mentioned you are currently having an active erruption episode go for 5 days of antibiotic therapy to reduce the intensity and we will pair it with ayurvedic medicines simultaneously as the antibiotics will help reduce the “vyadhibal” Antibiotics i would suggest are Cefuroxime proxitile 500mg twice a day after food, Metronidazole 400mg twice a day, some antacid. Also take t. Bandy plus once in three days at night. Take a dose of 3 tablets which will take you 12days of course. For your ayurvedic regime ?? Medication: ?? Tab. Live 52 DS 1 tab twice a day before food. Tab. Arogyavardhini Vati 2 tabs twice a day before food. Cap. Marvin 2 caps twice a day before food. Cap. Nelsin 2 caps twice a day before food. Syp. Sarivadhyasav + Mahamanjisthadi 2 tsp each twice a day before food. Mahatiktak ghritam 2 tsp twice a day with hot water preferable morning and evening not meal times Castor oil 2 tsp at night with one glass hot water. Use alum crystals as we use soap during bath time twice a day. ?? Do’s: ?? Millet roti Buttermilk (daily include in your meal) Moong dal (green and yellow both are ok) All fruit vegetables Leafy vegetables (except methi and dil) ? Donot’s: ? Hidradenitis Suppurativa your present stage will require combination therapy,of allopathic medicine and parallely ayurvedic medicine is required. From allopathic medicine you need broad spectrum stronger antibiotics which yoou doctor can suggest. From ayurvedic treatment you start with Kishore guggul 1-0-1 after food with water Alsactil 1-0-1 after food with water Kanchanar guggul 1-0-1 after food with water Mahamanjistadi kadha 15 ml twice daily after food with 30ml. water. Continue with tablet Liv-52 1-0-1 after food with water Wash the affected area Twice with neem kwath, dry and apply jatyadi tail. Giloy is not required at present, maybe later as situation improves we can suggest. Avoid sour fermented foods. Eat nutritious and healthy food . Take this treatment for 15days, if you see improvement continue further, if situation is not improving, visit nearby ayurvedic physician ksharsutra expert. Will be able to help you heal. –“Your condition, Hidradenitis Suppurativa, can be understood in Ayurveda as a deep-seated imbalance of Pitta and Kapha dosha, along with Rakta (blood) vitiation. This leads to inflammation, pus formation, and slow healing. Right now, your body is in a highly aggravated state, so instead of stimulating the immune system, we need to focus on शांत (calming) and शोधन (detoxification). Some herbs like Giloy, although generally beneficial, may not be suitable in this active stage because they can stimulate the system. So we will focus more on cooling, anti-inflammatory, and blood-purifying approaches, along with proper wound care and diet correction.” – so except taking online treatment, I advise u to first visit ayurveda practitioner for proper evaluation personally

Namaste Mohammed, Your condition — long-standing hidradenitis suppurativa with sinus tracts, multiple draining lesions, and frequent flare-ups — is a deep-seated, chronic inflammatory condition. In Ayurveda, this resembles pitta-kapha involvement with rakta (blood) dushti and ama, affecting deeper tissues. You are right to think holistically, but this stage needs a structured and careful approach, not just general immunity boosters. ⸻ About your concern: Giloy (Guduchi) is generally safe and anti-inflammatory, but in your case — with active, severe, draining lesions — it may not be the most appropriate primary herb right now. It is not harmful, but it may also not address the core pathology effectively alone. So instead of focusing only on “immunity boosting,” the goal should be: • Reducing inflammation • Supporting proper drainage and healing • Correcting metabolism and toxin load ⸻ What to do now (important shift) Avoid random combinations of multiple supplements. Simplify and target treatment. ⸻ Ayurvedic internal support (structured) Gandhak Rasayan – 1 tablet twice daily after food Kaishore Guggulu – 1 tablet twice daily after food Mahamanjishta Kwath tablet – 2 tablets twice daily before food These are traditionally used to support chronic inflammatory skin conditions, help in detox pathways, and improve healing. ⸻ External care (very important in your case) • Clean lesions gently with neem water (boiled and cooled) • Apply Jatyadi taila or a mild medicated dressing • Keep area dry but not overly irritated • Avoid tight clothing and friction ⸻ Diet & lifestyle (crucial for HS) Avoid: • Dairy excess • Sugar and refined carbs • Fried, spicy, and processed foods Prefer: • Light, anti-inflammatory diet • Warm, freshly cooked food • Adequate hydration ⸻ About your current regimen • Zinc → can continue (supports healing) • Amla → fine in moderate dose • Liv.52 → okay if digestion/liver support needed • Giloy → can pause or reduce for now ⸻ Important reality HS with sinus tracts is not a quick-fix condition. It requires: • Long-term management • Consistency • Sometimes combined medical/surgical care if needed If there is severe pain, spreading infection, or fever, do not avoid medical intervention. ⸻ Key point Your thinking is correct — but the strategy should shift from “immune boosting” to “inflammation control and tissue healing.” ⸻ With the right approach, flare intensity and frequency can be reduced over time. Consultation can help tailor this further to your needs.

Thanks doctor for your reply…for how long should I take this specific regimen?

Atleast 2 month and please follow up after that

1.Haridra khand 1 tsp twice aily with warm milk after meals 2.Triphala guggulu 2 tab twice daily with water after meals 3.Neem capsules 1 cap twice daily with water after meals 4.Mahamanjisthadi kwath 20 ml with 20 ml water twice daily after meals 4.Jatyadi taila -Apply on lesions after cleaning twice daily 5.Nalpamaradi Taila-Apply gently on healed areas once daily Panchakarma & Detox Options— - Virechana (purgation therapy): Clears excess Pitta and toxins from the gut/liver. - Raktamokshana (bloodletting): Useful for severe, pus-filled lesions. - Basti (medicated enema): Balances Vata, reduces pain, and clears systemic toxins. Giloy (Tinospora cordifolia) in HS— - Properties: Giloy is immunomodulatory—it regulates cytokine production and balances immune activity, not just stimulating it. - Implication for HS: In autoinflammatory conditions, Giloy may help reduce excessive immune activation due to its anti-inflammatory and antioxidant effects. However, responses vary, and long-term unsupervised use should be avoided. Lifestyle & Diet Guidance---- - Avoid: Fried, fermented, spicy foods, red meat, alcohol. - Include: Bitter vegetables (karela, neem leaves), detox teas, warm water. - Stress Management: Daily yoga, pranayama, meditation—since stress is a major trigger. - Hygiene: Frequent cleansing with neem/turmeric-based washes; loose cotton clothing to reduce friction.

Hello, It’s severe, long standing HS with sinus tracts, multiple regions involved, and active discharge. From an Ayurvedic view, this is not just a skin issue- it reflects deep imbalance of rakta (blood), mamsa (muscle tissue), meda (fat) and aggravated pitta + kapha with vata involvement (chronicity, tunnelling, pain) Your condition resembles -Vidradhi (deep abscess) -Nadi vrana (sinus tracts) -rakta dushti (impure blood) -kapha-pitta dushti with ama (toxins) What is happening in your body -Kapha-> blockage, pus formation, which discharge -Pitta-> inflammation, burning, bleeding -Vata-> chronicity, pai, sinus tracts -Ama (toxins)-> root cause, poor healing, recurrence HS is essentially a chronic inflammatory + toxin accumulation disorder with blocked channels Important note on Giloy (your concern) Your thinking is correct -giloy is immunomodulatory, not just stimulant -but in active inflammatory conditions with pus, it may –sometimes increase immune reactivity –not ideal in high pitta + active infection phase Recommendations:- -stop Giloy temporarily during active flare with pus + bleeding -can be reconsidered later in remission phase Treatment goals -reduce active inflammation and infection -drain and heal sinus tracts -purify blood -remove ama (toxins) -prevent recurrence Internal medications 1) Kaishor guggulu= 2 tabs thrice daily after meals for 3 months =best for chronic inflammatory skin + pus conditions, works on rakta + pitta + ama 2) Gandhak rasayana= 500mg tab twice daily after meals with warm water =powerful antimicrobial + skin healer, reduces discharge, itching, infection 3) Mahamanjisthadi kashaya= 15ml + equal water twice daily before meals =blood purifier, reduces inflammation, redness, recurrence 4) Triphala guggulu= 2 tabs twice daily after meals =helps drain abscess + reduces sinus tracts, mild detox + improves healing 5) Avipattikar churna= 1 tsp at night =controls pitta , improves digestion very important in HS External treatment 1) Jatyadi taila -apply on lesions after cleansing =promotes wound healing + reduces infection 2) Triphala kashaya wash= wash affected areas 2-3 times daily =cleanses pus, reduces bacteria 3) Turmeric + neem paste -apply thin layer on non open lesions =anti inflammatory + antibacterial 4) Dressing advice -use sterile gauze -avoid tight clothing -keep areas dry but not overdried Lifestyle (critical for HS) Stress (you said high-> major trigger) -HS strongly linked with stress + hormones Do daily -Anulom Vilom= 10 mins -Bhramari= 5-7 mins YOGA -bhujangasana -setu bandhasana -balasana These reduce inflammation via nervous system calming Diet -warm, light food -moong dal, lauki , tori -turmeric, neem, coriander -plenty of warm water -1 tsp turmeric daily -amla continue - good choice Avoid -dairy milk, cheese, panner -sugar and sweets -fried food -bakery items -red meat -fermented food (curd esp, at night) Home remedies -haldi + warm water daily -neem leaves if available= chew 4-5 in morning -warm water + lemon Investigations needed Since HS is systemic -blood sugar (fasting + HbA1c) -lipid profile -CRP/ESR (inflamamtion markers) -Vitamin D -liver function test Expected timeline -2-4 weeks= reduction in discharge and pain -6-8 weeks= fewer new lesions -3-6 months= deeper healing and fewer flare-ups HS requires long term consistency, not quick fixes -Avoid squeezing lesions (worsens sinus tract) -Avoid tight clothing and sweating accumulation -Do not mix too many herbal products randomly Your case is advanced but manageable with discipline Ayurvedic care The key shift is From immune stimulation-> to detox->inflammation control+ tissue healing You are already thinking in the right direction by questioning Giloy- that shows good awareness Do follow Hope this might be helpful Thank you
