How to taper off labetalol and replace it with muktavati for blood pressure management? - #55085
i take telma 80 at 11 am, labetalol 100 mg at 3:00 pm & at 10:00 pm. i m prescribed muktavati 1 tab in morning and 1 at night while tapering and ultimately replacing the labetalol completely in few days. does muktavati take time to give effect in few days? i observed i need to take muktavati first dose plus labetalol together so that i dont get palpitations. when i take just muktavati at 3:00 pm and dont take labetalol, the bp increases ans so i have to take labetalol to decrease it. what do uou suggest? Bp early morning is around 116/77, at 3:00 pm 135/85, at night 125/85. Plz suggest how i can taper labetalol dose and eventually replace itwith muktavati

is good that your BP is mostly under control, but from your observations it appears that your body is still dependent on the effect of labetalol, especially in the afternoon dose. In Ayurveda, medicines like Muktavati may take some days to stabilize the system because they work gradually on Vata imbalance, stress response, heart function, and vascular relaxation rather than giving an immediate beta-blocker type effect. Do not stop Labetalol suddenly, as it can cause rebound BP rise, palpitations, anxiety, or fast heartbeat. Since you are already getting palpitations when skipping the afternoon dose, tapering should be slower and more gradual. A safer Ayurvedic-style transition can be: Continue Telma 80 as prescribed. Continue Muktavati 1 tablet morning and night regularly after meals. For 5–7 days, continue the same labetalol dose along with Muktavati to allow adaptation. Then reduce only the afternoon labetalol first: 100 mg → 50 mg for 5–7 days if BP remains stable, then stop afternoon dose Keep the night dose unchanged initially. After another 1–2 weeks of stable BP readings, gradually taper the night dose similarly. Monitor: BP twice daily only (too frequent checking can increase anxiety) Palpitations Headache, chest heaviness, dizziness Your current readings (116/77 to 135/85) are reasonably controlled, so avoid rushing the withdrawal. In Ayurveda, stability and gradual reduction are preferred over sudden medicine changes. Supportive measures: Low salt dinner Warm water sipping Early sleep Daily 20–30 min walking Avoid tea/coffee excess and late-night meals Practice slow breathing or Anulom-Vilom regularly
So should i take telma 80 mg and 1 muktavati after breakfast, noon 1 labetalol and night 1 labetalol & muktavati? Or should i take telma 80 after breaksfast, 1 muktavati & labetalol at 3:00 pm & 1 muktavati & labetalol at 10:00 pm ? Most of the times my bo is below 120/80 in early morning and at night. Only in afternoon it is more. I m also put on multivitamins, iron and probiotics by doc which i take in afternoon
Since your BP is mostly normal in morning and night and rises mainly in afternoon, this schedule looks more balanced: morning after breakfast: Telma 80 + 1 Muktavati 3 PM: Labetalol 10 PM: Labetalol + 1 Muktavati Monitor BP and pulse regularly. do not stop Labetalol suddenly. Keep 1–2 hour gap between iron supplements and ayurvedic medicines.
One last question doc, can you please advise how much hour or minutes gap should be kept between telma & muktavati for morning dosa and between labetalol and muktavati for 10:00 pm dose. My current schedule is: Breakfast at 7:30 am Telma 80 at 11:00 am Lunch at 1:00 pm Labetalol at 3:00 pm Dinner at 8:30 pm Labetalol at 10:00 pm before bedtime.
Morning: Take Muktavati after breakfast around 8:00 am, then keep about 2.5–3 hours gap before Telma 80 at 11:00 am. Night: Take Muktavati after dinner around 9:00 pm, then keep about 1–1.5 hours gap before Labetalol at 10:00 pm.
Do not stop or taper Labetalol suddenly on your own. Rebound BP rise and palpitations can happen. Muktavati may take days to weeks for stable effect and may not fully replace labetalol in everyone. Your BP readings are fairly controlled now, so taper only under doctor supervision Take Continue current medicines as prescribed for now. Do not skip the 3 PM labetalol suddenly if BP rises and palpitations occur. Monitor BP twice daily and keep a record. Reduce salt, caffeine, stress, and sleep late nights. Supportive Ayurvedic option: Sarpagandha Vati Only under doctor supervision because it can lower BP further. Immediate medical care if Chest pain Severe headache Breathlessness BP very high Dizziness/fainting
