Hello dear, Your Thyroid Assessment & Lab Review : Your TSH is 4.9 mIU/L (mildly elevated) and Free T4 is 14.1 pmol/L (within the normal reference range). This specific biochemical pattern points toward Subclinical Hypothyroidism. Which is very easily manageable by Ayurveda. Because your peripheral thyroid hormone (FT4) is completely normal, the thyroid gland is functioning, but your pituitary gland is secreting slightly more TSH to keep it running. In standard clinical practice, a TSH below 10 without severe clinical pathology often does not require immediate allopathic hormone replacement (Levothyroxine) and responds well to targeted metabolic interventions. Ayurvedic Perspective on Your Condition From an Ayurvedic standpoint, your presentation is a classic manifestation of Galaganda / Apachi (Thyroid dysfunction) driven fundamentally by Agni Mandya (low metabolic fire) leading to Ama Sanchaya (toxic metabolic accumulation) and a Kapha-Medo Dushti (dysregulation of fat tissue and fluids). This explains the persistent difficulty in losing weight despite intensive resistance training. To restore balance, the therapeutic focus must shift from general wellness to highly specific Deepana (appetizer), Pachana (digestive), and Lekhana (scraping/fat-reducing) modalities that directly act on the thyroid axis and adipose tissue (Medo Dhatu). Medication Audit: Streamlining Your Protocol Your current list of 10 formulations is heavily redundant, causing an unnecessary burden on your digestive tract (Agni), which can paradoxically worsen metabolic sluggishness. What to Discontinue? Stop Immediately: Shankhpushpayadi Churna, Amlapittantak Rasayana, Dasavatar Churan, Rejuva Churna, Go Amrit Bhasm, Punaroday Vati, Dashmool Kwath, and Nari Sakhi. Stop these immediately. We dose not require these. These provide general rejuvenation or target digestive/gynecological avenues that do not directly address the underlying subclinical hypothyroid pathology or your current weight loss blocks. Continue: Granthi Rog Har Guggulu and Tapyadi Loh. Guggulu is a potent Lekhana (scraping) agent crucial for clearing glandular channels, while Tapyadi Loh addresses potential micro-iron deficiencies that limit thyroid hormone conversion and tissue oxygenation. Prescription : 1. Kanchanara Guggulu (Planet Ayurveda) Dose: 2 tablets twice daily with warm water, 30 minutes after meals. Action: Research-backed classical formulation that selectively reduces glandular swelling, modulates elevated TSH, and clears Kapha-Medas blockages. 2. Vara Churna Dose: 1 teaspoon (approx. 3–5g) with warm water at bedtime. Action: Enhances Pachana (digestion), eliminates Ama, and optimizes intestinal absorption to jumpstart weight loss. 3. Guggulutikthakam Kwatham Dose: 15 ml mixed with 45 ml of lukewarm water, twice daily, taken on an empty stomach 1 hour before breakfast and dinner. Action: Deeply purifies the blood (Rakta) and fat (Meda) tissues, correcting the foundational metabolic sluggishness fueling weight retention. 4. Shaddharan churna Dose : take 1 spoon with Luke warm water morning and evening before food. Targeted Weight Loss & Thyroid Optimization Strategy Your dedication to strength training 5 times a week is excellent for maintaining lean muscle mass, but your current physiology requires a shift in metabolic programming to break the weight loss plateau: Thermodynamic Dietary Interventions: Transition strictly to an Agni-vardhaka (metabolism-boosting) diet. Prioritize warm, freshly cooked, light meals. Integrate spices like ginger, black pepper, and cumin (Trikatu) into every meal to enhance thermogenesis. Avoid heavy, damp, or cooling foods like curd, sweets, refined flours, and heavy oils. Optimize Training Recovery: Five days of intensive strength training under a hypothyroid state can elevate baseline cortisol (stress hormone), which actively causes water retention and stalls fat loss. Ensure you get 7–8 hours of deep, restorative sleep to keep cortisol levels low and support thyroid conversion. Introduce Low-Intensity Steady State (LISS) Cardio: Dedicate 20–30 minutes daily to brisk walking outside of your lifting window. This steady, non-stressful aerobic activity directly targets free fatty acid oxidation without overtaxing your endocrine system. You should start these medications and stop previous as I said above as soon as possible… And tentatively 1 month is enough for recover your thyroid gland by these medications and after that once we will check TSH again by your lab test. You can consult me further after one month of medicine and reports.

Your reports suggest borderline / mild hypothyroid tendency (TSH 4.9 with normal Free T4 14.1). In Ayurveda, this is commonly seen as a combination of Mandagni (slow metabolism), Kapha accumulation, mild Ama formation, and hormonal imbalance. Since your periods are regular and you are already doing strength training regularly, your condition is still manageable naturally with proper metabolic correction. The good thing is that your thyroid is not severely disturbed, so focus should be on: Improving metabolism Reducing inflammation and Ama Supporting liver and digestion Managing stress and sleep Sustainable fat loss rather than aggressive dieting Review of Your Current Medicines Your current prescription mainly targets: Digestion & metabolism support Hormonal balance Detoxification Thyroid gland support Some medicines like: Granthi Rog Har Guggulu Tapyadi Loh Dashmool Kwath Punaroday Vati can be useful in mild thyroid imbalance and sluggish metabolism when prescribed according to body type. However, taking too many medicines together for long duration may sometimes overload digestion if Agni is weak. Since you already completed 2 months, continuing another 1–2 months may be reasonable only if you are feeling improvement in energy, digestion, bowel habits, and bloating. If you are not noticing clear improvement, then medicine reassessment is important instead of simply continuing the same combination. Natural Ayurvedic Advice to Normalize Thyroid & Lose Weight Morning Routine Wake up early before 7 am Drink warm water with: ½ lemon OR small piece of dry ginger Avoid sleeping again after waking Diet Guidelines Focus on light, warm, metabolism-boosting foods. Best foods: Moong dal Millets Lauki, tori, parwal, methi Soup-based dinner Jeera, ajwain, black pepper, cinnamon Small quantity of cow ghee Reduce: Sugar and packaged foods Excess curd at night Fried food Cold drinks Heavy bakery items Frequent snacking Excess soy and processed protein powders For Weight Loss Even with exercise, weight loss becomes difficult when metabolism is slow and cortisol/stress are high. Important points: Do not overtrain daily Include 1–2 rest/recovery days weekly Sleep before 11 pm Ensure adequate protein intake Avoid very low-calorie dieting Helpful Ayurvedic Supports commonly helpful herbs include: Kanchanar Guggulu preparations Trikatu Punarnava Guduchi Triphala at night (if constipation/bloating present) Yoga & Lifestyle Very beneficial: Surya Namaskar Ujjayi pranayama Bhramari Neck stretching exercises 15–20 min brisk walking after meals Important Medical Advice Please repeat: TSH Free T3 Free T4 Anti-TPO antibodies after about 8–12 weeks. If: TSH keeps increasing, fatigue worsens, hair fall increases, periods become irregular, then endocrinology evaluation may be needed. With disciplined diet, stress control, proper sleep, and simplified metabolism-focused Ayurvedic treatment, many people with mild thyroid imbalance are able to improve energy, control weight better, and stabilize thyroid function naturally.

Your lab report suggest slightly elevated TSH. Free T4 range is normal. This pattern mainly suggest subclinical hypothyroidism. That means your thyroid is producing enough hormone, but the brain is signalling it to work more If this test is done within a week, then I recommend you to repeat it after 2 weeks. Also recommend to stop all the medicines if you haven’t any other issues. Next about the weight loss, your BMI is 26.8 which is overweight range. Control food by doing some modifications in diet. 3 time meal- 9 inch plate method- enough protein almost 1/2 plate protein enough veggies which is more fibrous - 1/4 plate, less carb. Sugar cut is must. Strength training is perfect. That’s why I introduce 1/2 plate protein. Otherwise change to 1/2 plate veg, 1/4 protein and 1/4 carb. Enough water intake. Good sleep and regular time for sleep wake cycle and meals is compulsory. Otherwise the balance will collapse. Check vit D, vit B12, hb, hba1 c. - Use iodized salt - include egg, fish, nuts etc - manage stress by pranayama, yoga method. Take care, Dr. Shaniba

••In Ayurveda, this early stage of thyroid dysfunction is not viewed merely as a hormone deficiency, but as a state of Agni Mandya (low metabolic fire) leading to Medovaha Srota Dushti (sluggish fat metabolism). This explains why you are struggling to lose weight despite a commendable routine of strength training 5 times a week. The impaired Dhatu Agni (tissue-level metabolism) is causing your body to hold onto weight. ••Since your Free T4 is normal, this is the perfect window to reverse this trend naturally by stimulating your thyroid gland and boosting your basal metabolic rate (BMR). 1.Kanchanar Guggulu: The absolute drug of choice for thyroid glandular dysfunctions. It specifically targets Kapha accumulations in the neck region and scraping (Lekhana) of excess fat. Dosage: 1tablets (250mg each) twice a day, 30 minutes after meals with warm water. 2.Varunadi Kashayam: Excellent for clearing metabolic sluggishness and addressing water retention/weight stalls. Dosage: 15 ml with equal parts warm water on an empty stomach in the morning and evening. 3.Trikatu Churna (Sunthi, Pippali, Maricha): To kindle your Jatharagni (core digestive fire). Dosage: A pinch (approx. 1g) with a teaspoon of raw honey 15 minutes before your heaviest meal. ••Goitrogenic Moderation: Since you are strength training, ensure you aren’t over-consuming raw cruciferous vegetables (broccoli, cabbage, cauliflower) or unfermented soy in your fitness diet. Always cook them thoroughly to inactivate goitrogens. ••Iodine & Selenium Rich Foods: Incorporate subtle sources like soaked walnuts and pumpkin seeds (great for T4 to T3 conversion). ••Use Vardhit Ushna Virya Spices: Cook your meals with cumin, coriander, ginger, and a pinch of black pepper to maintain metabolic momentum. ••Pranayama: Dedicate 10 minutes daily to Ujjayi Pranayama (the psychic breath) and Kapalbhati. Ujjayi exerts a gentle pressure on the carotid sinus and directly stimulates the thyroid gland area through throat contraction

Your TSH being 4 point 9 with a normal Free T4 usually suggests a mild early thyroid imbalance where the thyroid is still functioning but the body may already start showing symptoms like fatigue difficulty losing weight sluggish metabolism water retention mild hair fall or feeling low in energy even when lifestyle is relatively healthy At 152 cm and 62 kg your body is carrying some extra weight and even reducing 5 to 7 kg gradually can make a big difference to thyroid balance insulin sensitivity stamina inflammation and overall hormonal health The positive thing is that you are already doing strength training regularly which is excellent for metabolism and thyroid support but sometimes when the body is under stress poor recovery inadequate sleep or digestive imbalance even good exercise does not translate into weight loss properly Looking at your medicines the intention seems to be improving digestion metabolism hormonal balance thyroid function and reducing Kapha related stagnation especially with medicines like Granthi Rog Har Guggulu Dashmool Kwath and Tapyadi Loh But honestly the combination is quite extensive and when too many medicines are taken together for long periods the body can sometimes become more confused than supported especially if digestion becomes sensitive or if there is underlying Pitta aggravation acidity bloating dryness heat or irregular bowel movements If after 2 months you have not felt noticeable improvement in energy body inches digestion sleep or weight trend then it is reasonable to review the plan instead of continuing exactly the same medicines automatically for several more months I would strongly suggest checking vitamin D vitamin B12 ferritin fasting insulin HbA1c and Anti TPO thyroid antibodies because many women with borderline thyroid issues and stubborn weight gain actually have hidden insulin resistance inflammation nutrient deficiencies or early autoimmune thyroid tendency Your food timing also matters a lot Try to avoid late dinners frequent snacking cold foods refrigerated foods excessive raw salads protein bars or artificial sweeteners because these often slow digestion and metabolism further in thyroid prone bodies even if calories are controlled Warm freshly cooked meals usually work better than restrictive dieting Try to keep lunch as the heaviest meal and dinner lighter and earlier preferably before 8 pm Do not undereat because that can further slow thyroid activity especially in women doing regular workouts Your body needs enough protein minerals and nourishment to maintain muscle and metabolic activity Since you are strength training regularly make sure protein intake is adequate through foods that suit your digestion like moong dal paneer eggs if you take them lightly cooked lentils nuts seeds and balanced home cooked meals Sleep and stress are equally important Even mild chronic stress or sleeping after midnight can keep cortisol elevated which indirectly affects thyroid function fat storage cravings and recovery You can also include simple supportive habits like warm water in the morning gentle stretching breathing practices evening walks and reducing screen exposure at night because nervous system balance plays a major role in hormonal healing Granthi Rog Har Guggulu and similar formulations are commonly used in Ayurveda for glandular swelling metabolic sluggishness thyroid tendency and weight issues but they should always be monitored according to the person’s digestion bowel pattern energy and menstrual response Watch for signs like increased acidity burning loose stools excessive dryness palpitations irritability or disturbed sleep because these can indicate that the medicines are becoming too heating or strong for the body Weight loss in thyroid tendency is usually slower than normal but it is definitely possible with consistency A realistic healthy goal would be gradual fat reduction rather than sudden scale changes because when metabolism improves the body slowly starts responding better Do not get overly worried seeing a TSH of 4 point 9 alone because in many people this stage improves naturally with proper sleep stress reduction digestive correction balanced exercise nutritional support and gradual weight management especially when Free T4 is still within normal range

Your thyroid levels are only mildly elevated, and since Free T4 is normal, this looks closer to subclinical hypothyroid pattern. Weight loss can still be slow due to metabolism and stress response. Your current medicines are quite extensive. Since periods are regular and symptoms are not severe, continuing everything long-term may not always be necessary unless there is a specific diagnosis behind each medicine. Focus more on: Consistent sleep Stress reduction High protein diet Avoid sugar, packaged foods, and frequent snacking Continue strength training + add 30 mins walking For weight loss, calorie balance and sleep quality matter as much as exercise. Medicines: Kanchanar Guggulu – 1 tablet twice daily after food Triphala churna – 1 tsp at bedtime Continue Dashmool kwath if it suits digestion You can discuss with your doctor about simplifying the current regimen gradually instead of adding more medicines. Repeat TSH after 8–12 weeks. Regards, Dr Raghuveer (Ayurvedacharya)
