Additional Treatments to Speed Up Healing

A. Ayurvedic Oils (for daily massage after Agnikarma or separately):

Jatyadi Taila or Kumkumadi Taila – Promote scar healing and improve skin texture.

Mahanarayan Taila – Useful for stiffness, especially over joints.

Application: Warm slightly and massage gently over the scar for 5–10 minutes daily.

B. Scar Tissue Massage (Post-Healing Phase)

After Agnikarma wounds heal, scar massage breaks up tight collagen.

Use circular and side-to-side motion with oils.

Helps soften and flatten raised scars.

HELLO HIMANSHU,

Hypertrophic scars can be persistent and difficult to treat

SCAR LOCATION- foot SYMPTOMS- tight skin, hypertrophic scar CURRENT THERPAY- agnikarma-therapeutic cautery No chronic illness

AYURVEDIC VIEW SCARS= dushta vrana(chronic wound) with shotha(inflammation) and Granthi like vriddhi(nodular overgrowth) -mainly caused by pitta-raita vitiation and kapha involvement(leading to fibrosis/thickening)

TREATMENT AIMS TO -remove dushti-toxicity -enhance romana-healing -balance tridosha and promotes rakta suddhi

INTERNALLY TAKE

1)GUGGULU TIKTA GHRITA- potent internal medicine for your case -15 ml daily before meals with warm water -Combines the deep detox of tikta(bitter) herbs with guggulu -heals chronic skin conditions, shrinks scar overgrowth by resolving Ama and Granthi -works especially well where Malyatyadi ghrita fails

2)MAHAMANJISTHADI KASHAYA + GANDHAK RASAYANA COMBO -mahamanjisthadi kashaya- 40 ml twice daily -gandhak rasayana- 500mg with warm water twice daily after food

-manjistha purifies blood tissue and clears pigmentation and inflammation -gandhak rasayana is sulphur based , highly rejuvinatize for chronic skin/scar tissue and enhances skin tone, reduces stiffness

3)KANCHANAR GUGGULU- 2 tabs twice a day with warm water -breaks down granthi(hard fibrous tissue) -clears kapha+meda+mamsa dushti causing tissue overgrowth

4)RAKTAMOKSANA -particularly good for scars with redness, inflammation, or itching -clears vitiated rakta and pitta, reducing hardness and improving texture -methods- leech therapy or prachnna at clinic

EXTERNAL APPLICATIONS

1)KUMKUMADI TAILA+MANJISTHA CHURNA PASTE -mix 5 drops of kumkumadi taila+ 1 tsp manjistha powder + rose water -apply to scar daily for 20 minutes , then wash

kumkumadi regenerates cells and removes hyperpigmentation manjistha is a natural blood purifier and scar fading drug

2)TAMRA BHASMA+SHATDHAUTA GHRITA -very potent for fibrous/keloid type scar -antifibrotic, antibacterial , enhances micro circulation

LOCAL PROCEDURES YOU CAN GO FOR IF AGNI KARMA ISN’T ENOUGH WORKING

1)KSHARA KARMA- application of alkaline paste like apamarga kshara -destroys excess tissue in hypertrophi/keloid scars -more effective than agnikarma in fibrous, raised scars

THINGS TO AVOID -avoid trauma, friction, or over scratching the scar -avoid oily, spicy, or junk food -avoid exposure to direct heat or cold on the scar

-GENTLE MASSAGE with warm oil daily -turmeric milk at night with pinch of black pepper drink -yoga to improve blood circulation

TIMELINE TO SEE RESULTS

INTERNALMEDICATIONS- onset of visible effects= 2-3 weeks, full effect= 3 months

EXTERNAL APPLICATIONS- onset of visible effects= 10-15 days, full effect- 2-3 months

AGNIKARMA/KSHARA KARMA- onset of visible effects= 2-4 sessions, full effects= 6-8 weeks

CAUTION -Do not apply raw turmeric , garlic, or harsh scrubs- they worsens scars -dont abruptly stop agnikarma

OPTIONAL MODERN SUPPPORT -SILICON GEL SHEETS OR ONION EXTRACT GELS- for scar flattening -FRACTIONAL LASER/MICRONEEDLING

HOPE THIS MIGHT BE HELPFUL

Agnikarma can indeed be an effective treatment for hypertrophic scars, especially when we consider its benefits mentioned in texts like Sushruta Samhita for managing stubborn skin conditions. It’s considered a powerful procedure for managing local conditions. However, the time it takes to heal varies greatly depending on factors like the size of the scar, your skin type, and overall health. It’s not uncommon for patients to notice improvements over a period of several weeks to months. Remember to follow the instructions provided by your Ayurvedic practitioner closely, as they can offer personalized guidance based on your progress.

Along with the ongoing agnikarma, integrating specific dietary and lifestyle support can aid your healing process. Eating a balanced diet that pacifies Pitta dosha might be beneficial, as hypertrophic scars can sometimes be aggravated by excess heat or Pitta imbalance. Consider incorporating cooling and hydrating foods into your meals such as cucumber, cilantro, and mung beans.

Externally, applying balaswagandhadi thailam or similar vata-pitta balancing oils might help in softening the scar tissue. Gently massaging the affected area once a day can improve circulation and promote healing. However, make sure the skin is not raw or inflamed during this procedure.

Practicing gentle yoga or pranayama can enhance overall circulation and oxygenation throughout your body, indirectly supporting skin regeneration. Ensure you continue any regimens or exercises that are advised by healthcare professionals.

Avoid direct excessive exposure to sunlight on the scar, as it might cause further irritation or discoloration. When stepping out, cover the area if possible or use a recommended natural sunscreen.

While these suggestions can complement your ongoing treatment, regular follow-ups with your Ayurvedic physician will be essential to monitor healing and make any necessary adjustments. Supportive treatments can enhance the effects of agnikarma, leading to a comprehensive approach to healing.

HELLO HIMANSHU,

Hypertrophic scars are raised , thickened areas of skin that develop where skin has healed after an injury.

AGNIKARMA FOR HYPERTROPHIC SCAR:-

PURPOSE- reduces hardness, improves circulation and helps flatten the scar tissue

DURATION- typically, visible improvement starts in 4-6 weeks, depending on the depth and chronicity of the scar.

FREQUENCY- weekly or bi-weekly sessions are common

COMPLETE RESULTS- may take 2-3 months for substantial improvement

LOCAL APPLICATIONS

1) JATYADI TAILA- wound healer and tissue regenerator dose- apply twice daily - morning and before bed- over the scar Duration- minimum 6-8 weeks or until scar softens and flattens =promote wound healing, reduce fibrosis and improve skin texture

2) KUMKUMADI TAILA- for skin rejuvenation and discolouration dose- apply at night, after jatyadi taila dries or alternate days duration- continue fr 2-3 months = enhances complexion, reduces pigmentation, softens scar tissue. very effective in post-injury scar aesthetics

3) LUKEWARM CASTOR OIL MASSAGE -small amount, gently massaged for 5-10 minutes before bath =reduces stiffness, deeply moisturises and promotes elasticity

INTERNAL MEDICATIONS these formulations support blood purification , tissue regeneration, and scar remodelling from within

1) MAHAMANJISTHADI KWATHA- 15 ml with water twice daily before meals for 8-12 weeks = detoxifies blood, reduce chronic inflammation, and promote healthy skin formation

2) AROGYAVARDHINI VATI- 1 tab twice daily after meals for 4-6 weeks =potent liver detox and blood purifier supports skin health, balances pitta and rakta vitiation- both crucial in hypertrophic scars

3) GHANDHAK RASAYANA- 1 tab twice daily after meals = helps purify micro channels, supports collagen repair and clears toxins from skin

Since you are already on Agnikarma, this protocol supports and enhances its effect

BEFORE AGNIKARMA- clear with triphala decoction

AFTER AGNIKARMA- apply aloe vera pulp or shatdhauta ghrita to cool and protect the tissue

FREQUENCY- weekly 4-6 sessions, then reaseess

WHY- agnikarma breaks down dense collagen in the scar. the above protocol softens and regenerates the tissue for better results

DIET AND LIFESTYLE

INCLUDE -warm , freshly prepared food -ghee- enhances absorption of herbs and supports immunity -green leafy veggies, pomegranate, turmeric, amla -herbal teas- coriander, cumin, fennel

AVOID -sour and spicy food -fermented items -excessive exercise or friction at scar site

THANK YOU

DR. HEMANSHU MEHTA
