Hello I get how scary and painful it is to see someone you love in a coma after a brain bleed. It’s especially tough given their age and condition, and you need clear and honest advice. PATIENT INFO – Age: 82 – Diagnosis: Coma (eyes open) from a brain bleed – How long: 2 weeks – Awareness: Eyes open, but not responding – Feeding: Tube – Movement: Very little or none AYURVEDIC VIEW In Ayurveda, this is called Mūrcchita Sannipātaja Avasthā which means: – Big problems with Vata (nerve issues, loss of awareness) – Pitta & Rakta issues (bleeding in the brain) – Kapha blocking things up (which leads to the coma) The brain (Majja Dhatu) and Prana Vayu (life force) are badly affected. WHAT TO EXPECT After 2 weeks in a coma at 82, a full recovery is unlikely. Partial recovery is possible if we start treatment now That could mean they move their eyes, swallow, or start recognizing people and become less dependent. The goals are to: – Stop any more damage to the brain – Get blood flowing to the brain better – Reduce swelling and clots – Help nerves heal – Prevent infections and bedsores AYURVEDIC PLAN This isn’t just about taking medicine. It involves specialized therapies, herbal remedies, and supportive care. 1️⃣ Head & Brain CareShiro Pichu: Cotton pads soaked in special oils (Ksheerabala Taila ) on the scalp for 45 mins a day. – Shiro Abhyanga: Gentle head massage every day to wake up the neurons. INTERNAL MEDICATION (If they can take them through the feeding tube) 1.Brahmi Ghrita 1 tsp morning empty stomach Helps the brain repair 2.Saraswatarishta 30ml twice a day (For awareness)
DIET PLANINCLUDE – Cow’s ghee – Almond paste – Milk – Soft khichdi (rice and lentil dish) – Herbal soups Tube feeding with good liquids is important. Physical Therapy & Senses – Move their arms and legs every day – Talk to them – Play music they know – Gently massage their hands and feet This helps wake up the brain. No treatment can promise a cure, but Ayurveda can offer a better chance when paired with good care. Even if modern medicine says to wait, Ayurveda can sometimes help the nervous system. I hope they recover as much as possible. Warm Regards Dr. Snehal Vidhate

HELLO, From an Ayurvedic view, an eye open coma following intracerebral haemorrhage is considered a severe, life threatening condition, often correlated with prana vata disturbance, associated Majja dhatu kshaya , any obstruction of vital channels , due to rakta involvement. Prognosis in an 82 year old patient after 2 weeks is guarded, and Ayurveda is used only as supportive care, never as replacement for ICU CORE PRINCIPLES -protection of life -pacification of aggravated vata -reduction off cerebral inflamamtion and rakta dushti -neuro supportive rasayana therapy -preventing further degeneration EXTERNALLY These help stabilize vata and support neurologicalrecoery SHIRODHARRA= very mil, medicated oils like brahmi taila SHIROPICHU cottonpad soaked In medicated oil over the crown ABHYANGA (gentle oil massage wit ksheerbala or mahanarayan taila PADA ABHYANGA - FOOT MASSAGE- important for vata control NO STORNG PROCEDURES LIKE SHIROVASTI OR NASYA IN ACUTE OD UNSTABLE PHASE Calm environment gentle chanting or Vedic mantras -family presence WE CANNOT PROMISE -guarantted recovery from coma -reversal of extensive brain damage -quick awakening after hemorrhagic stroke in advanced age Ayurevda focuses on quality of life, stabilisation and possible gradual improvement, depending on -extent of brain damage -location of hemorrhage -overall strength of pateint -speed of modern medical intervention THANK YOU DR. Supportive Systems (Adjunctive, not replacement)— Ayurveda / Integrative care: - Medhya Rasayanas (like Brahmi, Shankhpushpi, Mandukaparni) are traditionally used for brain health, but in coma they are supportive only after stabilization. - Abhyanga (oil massage) and Mridu Swedana (gentle fomentation) may help circulation and prevent stiffness once the patient is stable. -Go for Panchkarma procedures Physiotherapy: Passive limb movements to prevent contractures, chest physiotherapy to avoid pneumonia. Nutrition: Adequate protein, vitamins (especially B‑complex, vitamin D), and hydration. WARM REGARDS DR.ANJALI SEHRAWAT

✓Immediate medical priorities Neuro-ICU / Stroke team monitoring – control ICP (if raised), blood pressure, seizures, pneumonia prevention Tracheostomy + PEG feeding – if not already done (to prevent aspiration & maintain nutrition) Physiotherapy – passive limb movements, chest physio, positioning 2-hourly (prevents contractures, bedsores, pneumonia) Zolpidem trial (if minimally conscious signs) – 5–10 mg single dose under neurologist (paradoxical awakening in some coma cases) Amantadine – 100 mg twice daily (if some response) – increases dopamine, small chance of arousal ✓Ayurvedic supportive care (only complementary – after neurologist approval) Brahmi Ghrita – 5–10 gm morning with warm water (best brain nourisher) Shankhpushpi Syrup – 10 ml twice daily (calms mind & supports nerve repair) Maha Narayana Taila – gentle full-body massage daily 10–15 min (improves circulation, reduces stiffness) Nasya – 2 drops warm Anu Taila each nostril morning (stimulates brain & senses) ✓Family & Caregiver focus Realistic expectation setting – prepare for long-term care or palliative path Comfort first – pain relief, mouth care, turning, emotional support Daily gentle stimulation – talk, play favourite music, touch, family presence (helps if minimal consciousness present) Nutrition – high-protein feeds via PEG (consult dietitian) ✓Most important message At 82 with brain haemorrhage causing eye-open coma , recovery to normal life is extremely unlikely. Focus on preventing complications (infection, bedsores, contractures) and quality of remaining life. Dr Gursimran Jeet Singh MD Panchakarma

I understand how distressing this situation is for the family An eye open coma after brain haemorrhage especially at the age of 82 is a very serious condition and recovery depends on the extent of bleeding the area of brain involved and overall strength of the patient At this stage the first priority is always proper hospital care blood pressure control nutrition through tube and prevention of further complications Ayurveda can only play a supportive role and should be used along with ongoing medical management not as a replacement From an ayurvedic point of view the brain bleed causes severe imbalance of circulation inflammation and nerve tissue weakness The aim is to support brain healing reduce internal inflammation and maintain circulation without increasing pressure For this Brahmi vati 125 mg once daily with honey can be given only if the patient is on tube feed and the treating doctor allows it This is used in very small dose to support brain function and nerve recovery To support overall circulation and reduce clot related stress Arjuna powder 500 mg once daily mixed with warm water can be considered through feeding tube after medical clearance This helps in maintaining circulation and supporting the heart which is very important in elderly brain haemorrhage patients Dose should not be increased and should be stopped if blood pressure fluctuates For general strength and tissue support Ashwagandha powder 500 mg once daily with warm milk or water through tube feed may be used only if digestion is stable This helps in supporting muscle strength immunity and recovery in prolonged bed bound conditions Do not use if there is fever infection or digestive intolerance It is important to understand honestly that at this age and condition Ayurveda cannot guarantee reversal of coma But it can help in stabilizing the body improving chances of neurological response and preventing further weakness Gentle oil application on hands and feet daily passive limb movements proper nursing care and emotional support from family play a very big role Any change in breathing fever blood pressure or consciousness should always be managed by the hospital team first
